In this arc-flash simulation, the electrician died for three reasons.... 1. He failed to make sure that the spanner wrench would never, ever drop into the conductor, usually by using wrist harness bungees that will keep the spanner in place. 2. Sadly, he failed to make redundant or backup confirmations that the equipment was de-energized for sure, and my guess is that he failed to find a second or backup disconnect to really de-energize anything at all. The mantra - everything needs to be de-energized before doing anything! Not just a portion! 3. He also failed to wear the proper arc flash protection. His electrical outfit had no chance against the calcium-produced arc-flash heat of the accident. He should have worn PPE designed for arc flashes that had a rating of 40 or more.

I just came from a video where a switchgear can flashed over even though it was closed and no-one was there working on it. In that case, there was no tool to be dropped onto live conductors. Any idea what may cause an arc flash in such an incident?

All arc flash events require an initiating event, a bit like with arc welding, some event has to strike the arc, after which the arc can sustain through the gases produced by the initial arc providing there is sufficient arc fault current. It is difficult to determine what the initiating event was in the video you reference but we would suggest that one failure mechanism could be insulation failure either internally within the breaker or at or near the breaker terminals. An arc flash can occur under various conditions, including: 1) Opening of an isolating device under load conditions which is not rated for interrupting load current 2) Failure of insulation 3) Accidental contact, such as dropping tools into panels etc. Although many arc flash incidents occur due to worker activities, maloperation of switchgear and deterioration of insulation can also cause an arc flash to occur, potentially with covers in place and the switchgear appearing to be in a normal condition. Every situation is different, and there are many factors to take into account. This is something that we are able to assist with.

Hello @ja_adam_, 1. Yes, however single-phase circuits (240Vac) are not part of our arc flash studies as it can be demonstrated that the arc flash hazard is sufficiently low as not to present a significant risk of injury due to arc flash. 2. Yes, three-phase 400Vac circuits are included in our arc flash studies as the arc flash hazard can be significant on these types of circuits, in fact very often the hazard at 400Vac is often higher than at above 1000Vac. However, it should be noted that modern LV switchgear is very often designed not to expose operators to arc flash hazard during normal operation of the switchgear, provided the equipment is in good condition, properly maintained and operated as per the manufacturer’s instructions. Looks like the spanner dropped into the live side of the main switch, the consumer side of switch is shut off but live is still going to switch.

If John shut the power off, then why would dropping a wrench between phases create such a horrific incident? If there's no power, where would all that energy be coming from?

The switch gear is still being fed from somewhere else that is still energized. Same way if you turn of the master breaker in your home panel the lines coming from the street into the lugs of your panel are still energized.

John was working downstream from the disconnect where he had isolated the power but the junction where the spanner made contact was upstream where contacts were still energised. When you isolate something to work on it there can still be energised systems all around it that you must be aware of.

So, for example, you are a qualified electrician, but if you do not pay attention to the electrical safety rules, laws, and regulations of your company or country, and you work on high-voltage switches, breakers and capacitors while intoxicated or not at complete concentration, you can end up severely burned, injured, or dead from an arc flash. Lots of ways arc flash accidents can happen, but one way is using a magnetic large screwdriver on an energized load fuse you work on, and then you suddenly drop the screwdriver, and the metal part of that too hits the energized stuff on the ground, sets off air-gap sparking and causes a phase-to-ground strike of extremely large, explosive electric current/heat (the "arc flash" itself) in as fast as 1/8 of a second!!! The injurious/fatal force of the flash can be equivalent to 1 (or a bit more) stick of dynamite blowing up in front of your chest or face!
