As You mentioned at the beginning this is a cover of a Meat Puppets song, the lead and rhythm guitars are being played by Cris and Curt Kirkwood of Meat Puppets. MTV didn't want Nirvana to do this song live, but Kurt Cobain told them too bad it's already in the set.

At this point, Kurt was tired of the whole thing. His addiction and relationship with his wife was taking a toll on him. He also didn’t want to go back on another tour, but was pressured by the record company to promote the “In Utero” album while it was still hot. A lot to read, but I think you’ll see why this context is so important when you see his demeanor during the performance despite having one of his best versions of this song. Best rock and punk songs are basic like this and under 2 minutes. Buddy Holly started the "no more than 2 minutes rule" , The Ramones and most punk bands mainly adhlhereed to it. Pixies resurrected it after 20 years of excesses. Cobain idolized Pixies.
