Here's a good one. Soldier separated (CH 14 Misconduct), offered Voluntary Retirement because of more than 20 yrs AD), Voluntary Retirement (4187) signed and approved but not sent de DA for orders (held back). Command transferred back to Texas, soldier left in Company unit and separated from military after offered and had an Approved Volunteer Retirement. Separation documents were sent to DA instead of Volunteer Retirement docs.
