Thanks Prof. At 47:24 you don't follow steps to recognize respiratory or metabolic acidosis as the PH is decreased and PCO2 is increased( above 40 ) so the primary disorder is respiratory acidosis not metabolic acidosis

Sir.. at 46:42 it has to mentioned as anion gap is increased by 2.5 times for each 1 mg decline in plasma albumin right? It's written as anion gap is reduced . I think it's wrong..

Thanks very much again. I noticed that on your slide causes of high anion gap metabolic acidosis 39 minute in the presentation . the table of causes you listed 4 groups. I think there should be another group is inborn errors of metabolism ( separated from the lactic acidosis).

السلام عليكم ،، My inquiry about the rang of plus or minus ,,,for example if l have a patient with metabolic alkalosis and when calculate the expected value of pco2 he should to have pco2 =45 to be compensated ,,but the given value is 46...so can l consider this case compensated or no ???thanks a lot for greet effort ,,,

This will depend if acute or chronic, the age, the respiratory rate, the clinical scenario. History, physical exam, and labs all together will make you able to understand the case. All the formulas for compensation were explained in the lecture with multiple clinical cases. Thanks.

31:13 It's respiratory alklosis not acidosis as CO2 is 20(low) so it's partially compensated metabolic acidosis...

Yes, I agree with you the CO2 is low and less than 40, but the appropriate compensation is 14 and his actual CO2 is 20, his CO2 is higher than expected because of respiratory depression as a result of his intoxication, and he is not able to hyperventilate to compensate better. According to this formula. 1.5x4 +8= 14 -->the actual is 20 --> inappropriate compensation, retaining more CO2 than expected-not able to hyperventilate---> Respiratory acidosis

Hello Dr. Osama My inquiry about the calculation of anion gap. In some books, it is calculated as follow: Anion gap= Na + K - ( Cl +HCO3) > Which is more accurate ???Thanks a lot for your great efforts and best regards.

With or without K is okay, since the value of K is not much of a deal, this will not make a much difference in the total value or the treatment plan. I usually do not bother with the K level and I do not use it.
