That would be "extremely fast" with extremely as an adverb, "extreme" can't describe "fast". Since there is no "and" like in "extreme and fast tapping workout" extreme can't describe "tapping" or "tapping workout" either so that only leaves "workout" to describe. It's a fast tapping workout and I think it's safe to say the workout is quite extreme ;)
