In this video, Dr Sanjay Gupta, consultant cardiologist explains a heart condition called atrial flutter. Atrial flutter is an abnormal heart rhythm which can cause symptoms of heart palpitations. In atrial flutter,the atria contract at 300 beats per minute and every other impulse goes down into the ventricles and therefore the ventricles beat at 150 beats per minute. In patients who are taking medications such as beta blockers or calcium blockers the impulses reaching the ventricles may be reduced even more so the heart rate can be even lower than 150/min but basically as doctors whenever we see a heart rate of about 150 in a patient complaining of heart palpitations, we think of atrial flutter as the likely underlying heart rhythm disturbance.

Such a calm, perfectly reasoned and utterly clear presentation of this condition: delivered in a way that is easily understood by patients, yet never failing to incorporate all the clinical details which are so important. Explanations of medical conditions, delivered with this degree of clarity, are extremely rare, and make Dr. Gupta a very special person, and a very special doctor.

Very well said! Last year I went into heart failure from prolonged a-fib. They think I was going in and out of a-fib for several months. While I was in heart failure, I had several runs of a-flutter. Worst feeling I’ve ever had in my life. Luckily, the a-flutter passed, I was given medication to control the a-fib, and I’m having an ablation next month to hopefully keep it from happening again. Don’t ignore this stuff. I’m lucky that my heart failure was reversed. According to my doctor, most people are not that lucky

Just wanted to say thank you so much. We had a call on the phone a few months ago about my non-stop palpitations, and after your reassurance they all disappeared within 24 hours and haven't been back since. How are you doing today? I have flutter and wear a Fitbit. I don't feel any different at all when my pulse is 140 or 70, but I'm frequently checking the watch. Friends have said that I should lose it.

Such a clear explanation of atrial flutter. I had sudden onset of AFL on a Friday afternoon and was taken to hospital where IV Amiodorone x 2 was unsuccessful. I was admitted to coronary care and the next day had a cardioversion which was successful, and it was so good to have a quiet heart again. My cardiologist refused me an ablation since I hadn’t ‘earned’ it.😡 My GP referred me to a new cardiologist who performed the ablation procedure and I had no more AFL. I should say that I had open heart surgery at 11 to repair an ASD which caused scarring on the heart and predisposed me to arrhythmias later in life. Two years later I developed sick sinus syndrome and had a permanent pacemaker implanted the day after the Holter monitor results came back. Within six months I developed a DVT in the left innominate vein where the pacemaker leads were. Shortness of breath, swelling of the left side of my neck and jugular vein distension were all plain to see. My ‘new’ cardiologist said my SOB was due to de-conditioning and referred me to a psychiatrist. Subsequently I developed SVC syndrome (missed by this cardiologist) when the thrombus in the vein extended into the SVC. I had urgent surgery to correct this with a graft from the saphenous vein to restore the diameter of the vessel. Still I was told that doing a bypass of the blocked vein would not help my breathlessness, which was causing a decline in my quality of life and was disabling me. I had given up my teaching career and we had moved to a one storey apartment as I could no longer cope with stairs. I planned my funeral as I thought that I would die before I found a solution. My by now new cardiologist never gave up on me. He sought a second opinion and we found a brilliant surgeon who deals with difficult cases. A formal venogram showed a central venous obstruction of the left innominate vein which was now a ‘stump’ and not suitable for percutaneous stenting. My new surgeon performed a left innominate vein bypass with a Gor-tex pipe and by the afternoon of surgery the redness and swelling had subsided. My shortness of breath disappeared. I am now really well, going to the gym and enjoying life, and planning an overseas trip. I’m sorry this is such a long story but I thought you might be interested in hearing about my case which is apparently quite rare. I’ve had a hard time with cardiologists but my psychiatrist was responsible for referring me to my current cardiologist who actually listens to what I say and doesn’t have preconceived ideas. Just because I didn’t fit the usual picture he kept working on my case. I love your videos and your clear explanations. I have had more problems with my heart than most cardiac patients and I always learn something new from you. Thank you for what you do.

Pam Greatorex- I ‘enjoyed’ reading your entry only because, though it was well written, clear, and interesting; it helped me to feel that someone else has experienced some reservations about the cardiologist they are working with here and there. I moved so had to find a new one. Based on my experience with the sourpuss god complex of this new one I will be getting a new one. Thank you, you, you give me confidence.

Thank you for stressing that "treating the underlying cause" is key; when I recently asked my GP "don't you want to identify the CAUSE of the Afib?" He replied "often we can't do that - besides, body parts wear out over time". This statement, without even TRYING to identify any cause, save for a thyroid test. Listening to Dr. Gupta's videos, I have identified several potential triggers and am working to eliminate/reduce those. Additionally, I recognize that the Vagus nerve is a culprit in my condition... so I am investigating that avenue. As always, Dr. Gupta is an amazing resource in our journey with Afib.

This was an excellent look at Atrial Flutter. I was diagnosed with Atrial flutter after my heart was staying at 140-170 bpm. I went to the emergency room when my heart rate would not come down. They did the necessary tests and decided to do medicines which were successful, but within hours after discharge, my heart took off again. I went back to the ER and I was kept in the ER for two days under constant monitoring. They then decided to do cardio conversion and that seemed to work and I was discharged. The follow-up with cardiology lead to holter monitoring/EKG where they saw the the flutter and that lead to Atrial Flutter Ablation, which solved the issue, but on another EKG, they saw that I was now dealing with SVT and it was back to the cath lab for SVT ablation. Both procedures were painless and both ablations have been successful. You discussion helped me validate what I learned on the net and what my doctors told me. Thank you for your wonderful talks and you certainly helped me through my 2 procedures.

What I like about Dr. Gupta is that he always tries to reassure patients that the worst is not the worst, so basically, he says:" X can happen but...". This is always so positive for people who are suffering any of these scary symptoms. Really great video (as the others are).

Thank you for this info/advice. As someone who has both Fib & Flutter I find your advice extremely helpful AND reassuring. Having played sport into to my 60’s I was disappointed to become unwell. However I’ve got confidence & continue with an exercise regime & active life. I now manage my BP more effectively, limit myself to 10 units of alcohol per week & try to get 7-8 hrs sleep. Result: significant reduction in episodes 😊

Had 4 surgeries for athletic injuries in 9 months. Prior to the 4th surgery I told the anestheia folks I was concerned about too much anesthesia in a short period of time. I was on no drugs, healthy and normal blood labs at 56 years old. After surgery they said all was well and I could go home. I was dressed and they came back and said they recommended an exit EKG... I was curious why, but agreed. Well I was in A Flutter. Off to the ER, totally asymptomatic. 4 days later I was cardioverted and back in sinus rhythm. They could not find anything wrong at all and the Dx was ANESTHESIA induced A Flutter. WOW... Still only have an Ejection Fraction of 40-45 but feel great as always. No meds and no flutter or abnormal rhythms for 2 years now. This may help folks headed for surgery somehow.

I had atrial flutter three weeks ago and cardiologist who have seen me recommended ablation. I believe that my atrial flutter was triggered by botched bridge replacement by the dentist which resulted in my face swelling 3 weeks after bridge was replaced. I had to use antibiotics, the swelling disappeared in about 10 days, but 5 weeks after the bridge replacement (2 weeks after use of antibiotics) I had atrial flutter (114/80/157) and ended in emergency.

I've lived with your wonderful videos since I was diagnosed with AFIB about 5 years ago. Seems like its a permanent thing for me but I have few symptoms. Fear of blood clots and strokes are always there and serves as a reminder to take the meds and exercise as instructed. I wish you were my Cardiologist!

I think the correlation between sleep apnea and a-flutter and a-fib is spot on. Once the amount of time wearing my C-pap improved so did the number of a-fib episodes. I had a cardio aversion a yr ago and so far no more flutter. If a-fib does start it’s shorter & so far, doesn’t require a trip to the ER. I just tolerate that and the ectopic beats and it subsides usually under an hr. I do have asthma and have it under control with beudesonide in a nebulizer. Thank you for addressing a-flutter and mentioning how important anti coagulants are.

Dr. Gupta, after taking plavix for 30 yrs after 1st stroke, missed 5 days of plavix and had a stroke. 1week later, the patient had 1 episode of A flutter. During this time, patient was dehydrated, sitting outside in hot weather, an untreated infection, possibly 3rd spacing and 50mg of Meclizine. Plavix was discontinued and prescribed 2.5mg x2 of eliquis. No episodes of AF since, but recently a very "small stroke" but not TIA. Finally found a doctor willing to prescribe zio patch. 11 days, no AF events. Only PVC. 1 year 4 months post 1st event, and no other episodes. The fear is not having plavix now. Something that worked for 30 years. Is it possible that the infection, and contributing factors I mentioned cause an isolated AF event? Eliquis causes leg weakness and instability. Currently has CKD stage 3. Are you aware of any studies that followed patients with an isolated AF event, return to plavix? Any isolated events survive with anticoagulant instead of Eliquis or other anticoagulant?

Thanks so much for a great and detailed explanation on A FL. It makes sense to me now why my patients after getting cardizem and metoprolol po went from sinus tachycardia 140’s to A FL 60 -75. Patient was in A fib a day ago.

Thank you. I was diagnosed once again with this condition. I’ve had two ablations. One for atrial flutter and the other for atrial fibrillation. They put me on an anticoagulant until I can see the specialist next week.

I would like to share that TESTOSTERONE is helping me tremendously (male 50's). I was doing all kinds of suggested things but my energy just would not come back. None of my doctors mentioned hormones but a friend of mine suggested a blood test. My testosterone tested 7.2 out of an "in range" scale of 7 to 15 (U.S. LabCorp blood test) With testosterone, I am finally starting to be able to exercise consistently because my energy is coming back. I now go days without aFib symptoms and good blood pressure readings.

Thanks Doc! Your channel is really helpful! Just to show how many variables can exist, I have AFIB controlled with Flecanide, had flutter occur, cardioverted, flutter returned within 3 weeks, scheduled for ablation next week, it will be both Cryo balloon for the afib, and radio frequency for the flutter. Surgeon says my profile puts me at a 60% success rate for first ablation. My flutter feels more like intermittent (persistent) palpitations with a slow heart rate (54-79), rather than fast heart beats, I presume this is due to the flecanide and metropolol I am on. I have sleep apnea controlled with a bi-pap machine, high bp, and diabetes. Diabetes controlled well with a 6.1 A1C, bp controlled with Lisinopril & metropolol.

@@akilahfaiz6468 Hi Atrial fibrillation and atrial flutter last for some time and tend to be consistent, ectopic heartbeats are like skipped heartbeats or an extra heartbeat that you notice as a thump or stronger heart beat that can last minutes or hours.

I just viewed your white coat syndrome video.at the end of the video my BP was 150/86 83.i got away from that video because I have white coat.the middle of this video I'm at 121/71 74.this is true.its been less than 20 minutes

I’ve had atrial flutter and Tachycardia bradycardia all my life.. caffeine chocolate stress alcohol definitely aggravate it. I was also diagnosed with sick sinus syndrome AKA SSS I’m 63 looking at a pacemaker before I’m 70. Make sure your electrolytes are good have them checked I’ve hade over 100 eKGs over the years never find .. anything my father hade A-fib he had the ablation done 10 years ago it worked for him.. my heart flutter only lasts 3 to 4 seconds but it something you never get used to.. I’m now taking Magnesium taurate and COQ10 it helps.. and atenolol to control my heart rhythm.. I have Bad wiring china wiring in my heart.. lol thx DOC your input is greatly appreciated..👍🏻

I'm like you my friend I'm 50 now and had off and on Atrial Fibrillation most of my life along with anxiety and panic disorders since young my Atrial Fibrillation is pretty well controlled but I myself have been having alot more PACS and the doctors say it's your probably worrying to much about your problem because of all this virus and stuff around so latley the flutters sometimes last few seconds maybe even around say 25 seconds just wondering if you have had any issues of flutter like that thanks and take care of yourself.

I have flutters once a week, or so. My heart is structurally normal. My lungs are very large and normal. I lose weight overnight and wonder why.. and I have nightmares, which is stressful. Ugh this sucks when it happens from just leaning over! I’m only 41 years old. My head also hurts daily- nightly it’s pressure. I’m on no medication because they can’t find anything wrong. If I take medicine my body does the flutter. My flutter is like a birds wings it shakes my whole midsection and it’s visible and I lay down to make it go away. I feel normal during this.

Im the one out of a 100 that had nothing wrong with my heart but got a flutter anyway outta the blue, my cardiologist says an ablation will cure it, we'll see. I've done all the tests, twice, this happened about two months ago, they have me on beta blockers calcium blockers blood thinners, even though my score for stroke risk is 0, I'm gonna have the ablation and pray I'm done with taking these medications and done with an outta control heart, I have plenty of scars around my heart so what's one more

Can you give me some references about the success and side effects of atrial flutter ablation? There are lots of citations for ablation for afib. I see a lot of statistics thrown around but cannot find a citation on Pubmed or elsewhere. There is one citation but it is in German.

I just cant to excercise anymore. Feels like anxity and panic attacks have made my heart weak. I get paplitations, feeling faint when i do excercise. I used to be a runner before my anxiety and panic episodes.

Kartik Doonite feel just the same.! Used to be a runner too... I also suffer from stomach issues, bloated stomach, reflux, and constipation. When my stomach is very bad, I get alot off pvc’s.. i have tried for a couple of weeks to eat one meal a Day. That have helped on my stomach issues, and my pvc’s are almost gone. From about 2000 a day. To 10-20 a day.! No doubt that some stomach issues Can get pvc’s going.!

I've been on Eliquis since May and my most recent blood test had elevated liver enzymes and very low platelets. I'm also on 1.25 mg Cardiloc. Since May my resting pulse has decreased from the 130s to the 80s (usually) all the while still on the same medications. The cardiologist says I have atrial flutter and wants to do an ablation. I feel that the medications have decreased my quality of health and wish to get off them. Other than that my quality of life is good. If getting an ablation does not reduce the risk of stroke, is a relatively risky invasive procedure, and I'm not suffering quality of life issues, then why do it? I don't want pills and procedures. I went all through my 50s without so much as taking headache medication. I think my current situation is due to chronic long term stress coupled with inadequate sleep which I am working to correct.

I have had some vibrations in my chest when I lay down feels like it’s comes from my heart. I have been going to a UCLA Cardiologist for heart related issues after a year of tests he said function is fine besides the skipped beats here & there. I have learned to live with the thumps all night however the vibrations are keeping me up at night? Thought it was atrial flutter what could it be

Do you still have these? I feel like I get the same thing. Like a pounding or vibration at night. My heart rate goes up but only between 90-110. But I feel like my chest/back is pounding/vibrating/shaking faster.

Thankyou for explaining, i just went for an ablation for atrial flutter,but he said he could not do it as i was in perminent afib not flutters ,opp was stopped,thay are now going to put a pacemaker in,i have never had a cardioversion but dont no why.when i had atrial flutters it was 260bpm,nearly passed out.i could not stand up or my heart rate would emediatly go up.he said thay gave me bisoprolol x10 to stop flutters.

I’m one of those AFL patients with a pretty normal heart. History of AF and a good ablation in LA 11 years ago, with a small to moderately enlarged LA, but otherwise stucturally robust. Ablation in RA has not stopped it so far, nor has flecainide. Hoping dofetilide (in 3 weeks) will restore NSR. My AFL comes in 2:1 or 3:1 bursts of a few seconds to a minute, then turns into AF making it hard for an EP to map.

I have atrial flutter. Since this is a problem of the right atrium wouldn't a blood clot most likely lodge in the lungs instead of making it all the way to the brain? I know that this could kill you but it is not as much of a stroke risk as a clot from the left atrium, right? My problem at the moment is that I had to be taken off of all blood thinners due to being anemic. Hopefully the anemia will improve so that I can go back on anti-coagulant therapy. I get regular ectopics, skipped and extra beats, which I've accepted and they don't bother me too much anymore. But occasionally I get a sensation that I have yet to see or hear described. It only lasts maybe one or two seconds but it's a light squeezing sensation in my heart. I wouldn't describe it as pain but it's uncomfortable, a feeling that somethings not quite right. When this occurs I have no pulse, then an irregular pulse for a few seconds and everything goes back to normal. The pulse rate doesn't raise in these incidents. These episodes can happen two or three times in a minute and then I might not get one for a few more days or even a few weeks. There's no obvious triggers. It happens when I'm at rest, when walking and when exercising. I've had all the tests done and nothing ever showed up. My heart structure is normal and I've been assured that there's nothing wrong. Any ideas?

I have very irregular heart beats too. Especially when I exercise. I also usually get them in the morning. It can be scary. Even though they are not painful, it feels scary because it feels like my heart is going to stop beating. I get very weak skipped beats or flutters. But I began to take a drug called Mildronate and it helps.

I''ve suffered from a-fib for many years and 12 months ago started suffering extreme amounts of flutter 48 to 60 hours in every 72. I've had ablation in both a-fib and flutter but the flutter has returned. It's not as bad as before but l'm in and out all day long and any large heart rate rise will trigger flutter.

Very helpful as usual and well-timed for me. I had persistent AFib and on meds, but omitted Elequis in error for a few days and am back into AFib. Needed a refresher course on potential for stroke and why it occurs, so will call doc tomorrow to get my electrical system back in order. Am on diuretics, but if they want to add digoxin, are there any alternatives? I had a horrible GI reaction to it and don't want to go thru that again.

Sir may I ask you a question on heart palpitations? I'm 23 years old and I know what the cause of it is, but I need to know how can I get rid of em? Today at work I was doing the usual landscaping and I was taking out a heavy amount of dirt and grass with a wheel barrel. Well I tried to carry the wheel barrel over a side walk, but My stomach had this weird reaction and It feels like I walked a whole mile when in reality it was my stomach reacting to me trying to lift the wheel barrel over the side walk.

The sudden onset and sudden end to atrial flutter sounds exactly like PSVT which can also be 150 or higher bpm. I guess the big difference is PSVT is usually in a structurally normal heart. If caught on EKG, then whether such a tachycardia is a SVT or flutter , then it can be diagnosed, correct?

Question about Atrial Flutter in relation to a missing or damaged sinus node due to myxoma removal open heart surgery I’ve had 2 continuous episodes of atrial flutter in the year following open heart surgery that went on for a week+ the first was treated with beta blockers which I did not tolerate well and then another the second went on for 7 days At that time I had an ablation and they cut 2 flutter circuits. Wondering what the likelihood of these being a life long problem?

Chronic Butterfly. I also have EDS. Was fitted with a Pace Maker last year. I still feel awful. Fatigue is great. Heart races to 130/140 all the time. I lecture and I feel I’ve run a race afterwards.

Chronic Butterfly and Lorraine Roach, I also have Ehlers-Danlos Syndrome (Hypermobility Type). I was diagnosed with atrial flutter a year ago via a 24 hr holster monitor and stress test. I was prescribed Metoprolol, a beta blocker that I take twice a day. It’s been a wonder drug and I carry a few tablets when I go out as I occasionally need a dose earlier than usual. It’s given me peace of mind, especially when I had a bad episode away from home which took only 10 minutes to settle down after I took the Metoprolol. I wish you both good luck with your heart health and of course with your EDS. I only found out afterwards that heart conduction abnormalities can be a feature of EDS. I also recommend that you read this article about the mycobacterium Chlamydia Pneumoniae which is airborne (not the same as the better-known STD). The website www.cpnhelp.org has a lot of great medical information and the list of associated conditions are mostly the same as some of the EDS comorbidities. I think your cardiologist would be interested to read the Physicians’ Page of this website as C.Pn is known to affect the heart.

Right AF is an easy fix. Left AF not. I had both. Right ablated 2012. Gone. 2021 left AF. First ablation did not last 6 weeks. I webt to Austin TX aug of 21. LAA isolation. Fixed it

Dear doctor Gupta; I have severe anxiety and panic attacks and I was wondering if you could reassure me about my palps, I'm 27 male now, I used to exercise a lot and do a good job at it, but then panic attacks caused me to have to stop so I got out of shape, and now, SOMETIMES when I exercise I get thuds or flutters (mostly thuds), but sometimes it doesn't even happen. And when it does happen it's mostly like 1 or two for the whole time (around 40 minutes to an hour). It happened today like twice (one wasn't that bad), but I just calmed myself down and finished exercising with no problem besides the panic it caused. My heart rate was even higher after I continued exercising than it was when they happened and apparently I was fine because they happened at the beginning mostly. I don't get out of breath or have pain, it just sometimes feels like I got the wind knocked out of me like I got punched in the chest or something. I really think it's related to the stomach though because whenever it happens I feel like there's so much trapped gas in my stomach and chest, and when I burp it feels amazing. I still have to make an appointment with my doctor though, but a serious heart issue can't just develop over such a short time like a year, can it? I think I'm going to try some gas pills or something, I think I'm eating pretty healthy, my diet is more or less paleo. Love your videos, thanks.

Sounds like your arteries are clogged. Take some serrapeptase twice a day for a few weeks. Also, you may need more iron in your diet.
