Quick question , if a resident identifies that a particular medication is being given to a specific type of patients in the ward although the recommendations (guidelines) have changed and the resident points it out to the seniors. Is it a kind of an audit? How can one go with that. I'd be very grateful for your help !

And once we are done with the audit and recommend changes to improve the audit is it necessary to re-audit and document all the changes again and present the improvement again? (means audit cycle is mandatory to be completed? Or just we can recommend the changes and then sustain them?
