I’ve literally witnessed this over the past few years as an informed skincare and social media consumer. If companies can sell you a Retinol, they can sell you even more products and treatments to chase the negligible effects of retinol (in comparison to retinoic acid). My aesthetician literally told me that she was taught to discourage Tretinoin use because it limits what she can do and sell in the treatment room. (She remained wise and informed enough to encourage Tretinoin though), and said that I would notice less of a visible result with professional glycolic acid peels vs. her non-Tretinoin clients because my 45 year-old skin is already in great shape. (And indeed, I noticed NOTHING after a series of six 50% glycolic acid peels). Without direct RAR-gamma activation, retinol is such a time and wallet drainer. I currently use Renova Tretinoin .02% 5 nights a week and Trifarotene 2 nights. Now, the messaging on how Tretinoin is too aggressive and ruining our skin, never intended for long term use, causes calloused skin, “inflammaging”, etc. is running rampant on socials BIG TIME. While all can be the case, it’s usually due to improper usage, concomitant use with too many unnecessary and inflammatory products (e.g. hyaluronic acid), dosage, the number of RARs in the skin, etc. and we all know, the dose makes the poison with any medication and using it on macerated cells seems a recipe for disaster. Fear mongering strikes again. On another note, I’m having trouble getting on with L-ascorbic acid and the data on THD Ascorbate and derivatives is less convincing. I’d be curious as to what you think about ellagic acid. Seems to have some good antioxidant and skin brightening data behind it (more so than the derivatives). Thanks!

Circling back after watching your Vitamin C video. I own the Allies of Skin 35% and struggle to get on with that one as well. If you think the Allie’s ethylated ascorbic + THD far outweigh ellagic acid benefits, I’ll keep on with the Allies but would sincerely appreciate any tips on how to possibly get on with it. I currently use an Aestura Attobarrier Hydrating Essence as a light moisturizer after Tretinoin…wondering if there would be benefit to use it in the morning followed by the Allies. 🤔

You've hit the nail on the head with your comments. Hopefully if my channel becomes more popular, the nonsense companies spread will be restricted. I'd stick with the allies product for now. There may be benefit in ellagic acid, but, long term I doubt there will be a difference in that both will give you the best skin in combo with something like tret/trifarotene. I'd say try both and use whichever you personally prefer. I'm very glad my vid helped and that there are people like you who aren't dumb enough for companies to fool into buying crap after crap ❤️

At 65, my skincare has been the same for 40 years Factor 50 every day of the year I use Altruist it’s available (online) Tretinoin every night Never use moisturiser, it makes my skin lazy But lately , twice a year I have polynucleotide injections to boost my collagen and give me a glow Vaseline on lips every night Castor oil at night all over my neck and hands And that’s it . Simple, very effective and not expensive. I’m proud of my skin it’s in fantastic condition Just paying this forward to the young girls looking for tips.

Wow. I’ve never heard that “moisturizer makes my skin lazy” but it’s definitely something to consider now! Our skin has its own micro biome and healing process and putting so much chemicals on it especially in combination probably makes skin worse in some cases!

I don't like that you put other dermatologists down...they often say tretinoin is way more effective than retinol. I am glad I found your channel, the more information the better but you do not need to throw anyone under the bus.

I think he needs! i have spent so much money over the years on different forms of retinols, some very expensive.They always made my skin drier and made bigger fine lines.I tried it all the ways, small amounts, a day a week so on.Nothing worked. My sister sells Sisley and i had the chance to try products. I just use with comun sense , without overthinking nor listenting influencers and my skin looks great.Other crap is all the forms of vitamin C that give me pimples. The influencer live from this constant posting and advertising, putting pressure on our minds and pockets. I am tired of being dumb!

This is all so fascinating. I’ve been watching all the videos and reading comments over the past couple of days. I’d given up on retinoids as it seemed like my face was coming off and breaking out like never before. Now I want to have another go! Trying to figure out if I’d be better off ordering adapalene (here in the US) or ordering the .005 tret subscription prescription I currently have on hold. Also wondering if I can work through face oil oil supplies or if it’s better to order squalane - definitely going to ditch moisturizers from here on out! Grateful for this no-nonsense biohacking friendly channel. :-)

Adapalene 0.1% is available OTC in the USA and is relatively inexpensive. It is also less sensitizing than tretinoin. Would you recommend it as the ideal, readily available retinoid?

It's $6 in my country. Have been using it for 22 years and I look 15 years younger than my real age. Primarily used it for mild acne on chin. Continued bc I love it. *If you're pregnant or breastfeeding - talk to a doctor and they will more likely than not tell you to stop using it!* *If you're allergic - obviously do not use it!* Otherwise - it's amazing, and I tried EVERYTHING under the sun (tretinoin, dozens of retinols.....evertything).

I used to have adult acne and my dermatologist recommended me “aklief” which is Trifarotene. I love it and it has done wonders for my skin, albeit I still have a little forehead lines (from not taking care of my skin in the past). I’m surprised that the jury is still out about its effectiveness compared to Tretinoin and I thank you for your video to dispel a lot of misinformation on the web.

Hmm I use adapalene, however to give merit to the content creators your mentioned they have elaborated (the information is there, not in the compact format you do , but it's there) about the prescription retinoids , (well adapalene being ex-prescription) . Speaking of two questions: 1. It was from Dr Dray mentioning how adapalne is quite a selective molecule and doesn' t bound to all the receptors.I presume it binds on the alpha? given they are the most abundant ones? I will google afterwards. She was also very reluctant to talk about it's antiaging capacity, since the medical study didn't know show collagen boost. As far as I understand they used echography and it was not an ideal measurement from that start Althoust an elastin index term it was found improved, so i am not sure how one reads that is correlated to a single "yes it does antiaging". Does this index by default means collagen production ?. What is your take? 2. How will you evaluate Korean products? I recently learned about Enprani Retinyl retinoate and my brows raised and got firmed even without using it. Apparently Medik8 is reselling under their brand-name for only 5 times the original product price. You are also gorgeous looking.

To answer each point in turn… 1. Yeah selectivity is crucial. The more modern retinoids are designed like that. With regards to collagen increase - the problem is that you have to define the type of collagen you're creating and then use different methods for that. Morten Karsdal is one of the experts on this topic. For elastin, I don't think it will improve to the extent that something like Sunekos will provide but I'm sure there will be some sort of positive effect. I think as long as we are targetting the RAR-gamma then we are still going after the same HoxB1 gene family. 2. Retinyl-retinoate seems to be a great product and looks like it's far more powerfl than retinol. I agree that Medik8 are selling it at quite high a price but that's the industry for you! I haven't seen a study that converts it into tret or adapalene or trifarotene etc in terms of equivalence. Because of that, it's extremely difficult to compare one to the other, you can only really talk about the selective receptor targetting I guess. 3. Thank you :)

IF hmm maybe i will try my luck for one more! Really fast my eye for bryan johnson taking only oral retinoic acid, accutan is called here. Then a parallel video from a dermatologist comparing oral vitamin A to accutan. That Vit A supplement was rather weaker but still similar. I noticed also how often vitmain A is added in food supplements, small amounts but the ones i happen to take. it's a lot to disentagle here. My question is that oral prescription seems to be more efficient, (I take minoxidil as an example too) , so it could be another direction instead of topical application of retinoids? However is it something a liver would appreciate? and should be such supplements a bit more restricted for pregnancy cases as retinols are? I mean i can feed my friends any vitamin supplement i can reach but when it comes to retinoids are like cats in the water because it can cause teratogenesis. And for oral consumption is vitamin A beta karotin substance in most cases? Could natural sources of this supplement act as good and without worrying for your liver? @@toriwoods9800 Retioate is supposed to be a very advanced retinoid. It has the potency of retinoic acid but also doesn't irriate and dehydrates. So if you use it you will get a very snatched face as a joke. So it's that good that by thinking of it makes me snatched.

My mum used sheabutter for everything when we were little from colds to skin to even drinking for constipation and coconut oil or the palm kernel oil. I lost my way as i grew and gave into skin care but i always stuck with natural i have oily skin and all i use now is vaseline coconut oil and sheabutter. My skin glows no lines acne gone. Money saved 😅

They do sell Adapalene 0.1% over the counter in the US, but mostly marketed for acne. Is this just as good for anti ageing as tretinoin and what is the difference between them? Also, my ophthalmologist has said I should stay away from retinoids around my eyes because it can make my dry eye condition worse. Thoughts please?? Thanks!

Absolutely it is! I made a video explaining them all earlier 😊 They can irritate any skin if the skin isn't used to it. All my patients use them around the eyes slowly and work up and there's no dryness at all.

This is so helpful. I am 46 and have stubborn hyperpigmentation from the pill.. I feel cheated cause ive been taking care of my skin since i was a teen and ive always been proud of my flawless skin. Hi Dr, Is Isotretinoin ok to use instead of tretinoin, i picked up a non perscription tube from a pharmacy in greece this year. My skin is perfectly fine with it, no reactions. But wondered if i should go back to tretinoin instead? I do have oily skin with sebaceous filaments, so thought Isotretinoin would be a good and cheaper alternative while i could buy it.

Uh, isotretinoin is sold under the brand name Acutane. It’s an oral medication used for recalcitrant acne vulgaris. It basically shuts your sebaceous glands down, so no more acne. Tretinoin is a topical cream or ointment that helps with acne AND is an excellent fine line/wrinkle remover/collagen promotor. I think you might be confused.

Lol I just bought Medik8 0.06% Retinaldehyde from using adapalene 0.1% before because I thought I wanted it to be a little stronger. Should i go back to the adapalene? 😂 . I heard that adapalene hasn't been studied much for anti-aging results , its known to be used for acne . So for long term & anti-aging purpose, what product do you think is the best for sensitive skin?

I'd go back to adapalene. In terms of antiageing etc it's basically just vitamin a which we know is used to help skin ageing. Wrt sensitivity, I'd say is it depends on the person because sometimes the sensitivity is linked to things like digestion issues too so hard to say over a RU-vid comment

It's an answer that's different for everyone to be honest. It'd ideally revolve around skincare and some sort of clinical treatment together. Skincare that has vitamin A, some exfoliation to remove old pigment, pigment production inhibition and suncream would be ideal. Then a clinical device like Sciton BBL and/or Moxi would be the cherry on the cake. Hope that helps

I used the retinol products by Paula’s Choice, Drunk Elephant, Perricone, Neutrogena, The Ordinary and Medik8. None of them made a difference to my skin. Some stained my pillow case as well so had to place an old t-shirt on top to stop that.

Don't use glycolic acid, I used 8% glycolic acid and it ruined my skin, it took 1 years to recover with nevia moisturizer, and then started olay nigh regenerative retinol moisturizer 3 times week and it did well

It's not that retinol is bad, it's that the OTC products with retinol are too weak. High concentration retinol products are actually effective, cheap and less aggressive than prescription retinoic acid products. Not everyone needs prescription medication!

I'm it sure about the insurance, sorry! Usually they don't because it's cosmetic. Tretinoin/vitamin A can be used for total skin health anywhere on the body really. But for laxity there's only so much any kind of skincare will do if I'm honest. For that, skincare combined with clinical treatment is probably best.

First of all, don't worry too much. It's expected and will slowly subside unless there's information I'm missing. You can drop the frequency of usage down and also increase use of squalane oil at the same time to compensate (TheOrdinary sell it for very cheap).

Started adapalene at the age of 21. That was 22 years ago. It's a revolutionary ingredient. Not expensive either ($6 in my country). Regardless: if it cost $200 - I would pay $200 for it. If you're pregnant or breastfeeding - talk to a doctor and they will more likely than not tell you to stop using it! If you're allergic - obviously do not use it!

Pretty sure I read collagen production takes a nosedive and you start ageing much faster once you hit 25, so 28 is absolutely not too soon to start with the anti-ageing products.

I am using Dermatica's Tretinoin 0.05% with 4% niacinamide. Well that is what it says on the tube which is sold for £26 15ml per month. Is it any good? I am Fitzpatrick skin type 02.

Is there a % on the Tretinoin you would recommend? I have been using for years, using the 0.1% now, but still get the flaky skin after a few days? Would you recommend using daily since I’ve used for some time? And would you recommend a moisturizer to diminish the flakiness ? If so any suggestions as I saw your video on moisturizers.

If you don't push through the flaky stage it'll be life long. Go daily and then push through - you'll never need to do it again after that. I'd recommend squalane oil instead of a moisturiser.

I started sleeping with tape across my forehead in a T shape in 2014 as an experiment because I was getting frown lines. Sleeping with the tape there trains your muscles to stay relaxed most of the rest of the time, even when thinking or emotional (as in dreaming/making faces asleep). I am gluten intolerant and at that time the frownies product contained wheat so I tried blue painters tape and it does not irritate my skin. It is firm without being too firm and now, 10 years and counting, at 62, I have been wrinkle free there! I use the width that covers most of my forehead and trim it to avoid my eyebrows. Then I cut a piece to lay on top that comes down between my eyebrows to the top of my nose. I use the 1.88inch Scotchblue version. No residue, and if I have applied skincare, and gently wipe off any oiliness it still adheres (doesn’t have to be squeaky clean). I have used brown medical paper tape but it is a bit more difficult to remove, although less startling to a bed partner, lol.

I personally use high strength retinoid everyday tbh. Wrt scarring, usually something clinical is needed alongside like a laser as an example. It's obviously more effort but imagine if u did it years ago and never had to worry about it for the rest of your life - some things are worth investing in. I'd say find a clinic that offers something like Sciton Halo and Sciton BBL near you and invest in yourself 😊

@@Dr_Abs really appreciate the reply doc. I am in my 2nd years on alternating fractional CO2, microneedling and subcision atm. The result is quite good. But, since the acnes keep coming and going, I feel like it's all in vain. Still optimistic tho'. I lately found friendly products for my face and I am now trying tret 0,1%. Wish me luck :) Btw, will Halo and BBL have better result compare to treatments i've done? Haven't checked the treatment cost but i'll consider if I could afford it. Thanks again doc ☺️

I probably wouldn't keep having CO2 repeatedly, I'm not 100% convinced that much trauma again and again will be good long term (personally). You need to stop treating scars if the acne is still active - settle it down first then you have a limited number of scars to treat instead of constant new ones from new acne. Wrt the treatments it's impossible to say without a proper consultation unfortunately

I feel like this is what most retinol RU-vid dermatologists have been saying though, that a prescription retinoid show results faster but haven’t studies shown that OTC retinols also will work well in the long long term? So in a nutshell are you saying any OTC retinol is bullsh*t like retinyl palmitate, etc? What about Altreno? I have been looking to get a retinol prescription so I might check out your link. Actually it says “page cannot be found”, man you trying to give me a virus 😂 what a load of 💩

I suppose if you microneedled before application you could get them to penetrate. But the question isn't just about penetration - even if they work by improving the lifespan slightly, if you still smoke or drink or have huge protein intake or don't exercise most days etc, how much is it really going to help? Life longevity will improve for free if you just have a healthier lifestyle.

Bakuchiol is like an alternative to retinoids - it's used by several of my colleagues or women that are pregnant and don't want to use retinoids. Granactive retiniods seem to be more powerful than retinol in vitamin A activity but still pale in comparison to those mentioned in the video.

Otherwise doctor, I recently stopped taking tretinoin (my skin tolerated it well, it's a shame) because it made me more tired and more vulnerable to certain viruses (often sick, especially after the 3 month purge phase ) it's a shame... I have a fragile body but perfect skin.... Have you ever heard of this type of side effect or problem with this medication? Sincerely Jack

Sorry for being a bit thick but aren't these prescribed solely for acne issues ? You seem to be saying that these are also have anti-aging benefits, which can be used by people with no acne at all.

Yeah they can be antiageing too. Think about it this way if it helps... the molecular interaction is independent of why you got the product. So, whether you use it for acne or aging, the interaction will still be the same. Therefore, if it improves skin health why wouldn't it do it any time you use it? Hope that makes sense 😊

Sunekos is better. Polynucleotides generally make collagen 1 3 4 7. 1 and 3 aren't difficult to make. 4 and 7 are crucial in antiageing etc but are very difficult to make. Sunekos makes them (+ elastin + fibronectin) in the maximal amount hence it's granted a patent for it. The fact no PN company markets by saying it makes the most says it all. Often, it's what the company doesn't say that's most important. Check out my antiageing lecture for a full and complete understanding of this.

Red light therapy and copper ions.are the best for the skin. I use 1/8 tsp of copper sulfate pentahydrate dissolved in 2 oz of water inside a 2 oz spray bottle. I themz spray 2-3 pumps in a small amount of lotion or gel and rub itbon my face and neck. Then, repeat the process woth legs, arms, etc.
