What is conjugated bilirubin?
When bilirubin binds with glucuronic acid, then it is called conjugated bilirubin.
What is the first Principle of conjugation of bilirubin?
If mononuclear cells release bilirubin in circulation, then this molecule should be trapped in circulation so that this molecule may not diffuse back into the tissues of the body. In this way, it will be handled by the liver when it passes by the liver later on.
What are the ways to keep and trap bilirubin in circulation?
There are special bilirubin trapping proteins synthesized by the liver, which, when come into circulation, bind with the bilirubin. These are called Bilirubin Binding proteins released by the hepatocytes.
What are the two properties of bilirubin when banded with the plasma proteins?
1- It can not diffuse through microcirculation back into many body tissues.
2- It cannot filter into the urine, so this form will not produce bilirubinuria.
What is Kernicterus?
It is a type of brain damage. If bilirubin goes into CNS in a heavy amount in babies, it will cause brain damage. If a baby survives this condition, he will develop mental retardation or motor dysfunction.
Why is there a great risk that high levels of unconjugated bilirubin can go into the CNS of newborns?
There are two reasons:
1- A newborn's Blood-Brain barrier is not so well developed.
2- There is a shortage of bilirubin binding proteins compared to unconjugated bilirubin.
How can newborns be prevented from developing Kernicterus?
There are two ways:
1- Phototherapy
2- Tin Protoporphyrin Therapy
