It's correct that the event horizon does not necessarily represent the geometrical boundary of the energy-matter the black hole consists of, but we do not know if its size is Plank length. No, the actual size of a BH depends on its mass(schwarzschild radius).

Not just a graphic representation, that's more or less what you would see should you gaze onto a black hole (which I wholeheartedly hope you never will). The light can't escape the event horizon (one of its many, many fucked up properties) and you can say those black circles are (from our perspective) a cross section of more or less spherical volume that the horizon surrounds.

Violent star formation feeds tons of gas and debris into the accretion disk of the black hole, which creates a quasar. The intense radiation from the quasar blows gas out from the core of the galaxy, which regulates/restricts star formation and creates a calm galaxy (like a carburettor on an engine).
