You'd be surprised how a freestyle can go, spitting off the top is literally killing shit that you yourself have never said heard or thought of. Mostly they are rhymes and words that you use alot and rearrange how you spit for the free, but when it's off the top sometimes it gets more fluid and doper shit comes out that's completely fresh. I've seen a few artists that seem to have it and I definitely have spit some one off shit

The thing that amazes me about accomplished freestylers is the depth of their vocabulary and how “fast their RAM chips” can access that. I have yet to hear a single word I haven’t previously heard or didn’t know. But, for the life me, I could never pull out one word rhymes like this guy is pulling out whole lines.
