People don't drive around because they don't need to, they can just reset and choose to spawn in another city and people rarely use other cities because there's no one there, if they made spawns between the cities, a gas station or something, newcomers would eventually go to different places like eastdike, also there is too much map for 45 people servers, so everything will be empty even if people spread, and most anomic players are non car guys so cars are just ultility for them.
