In March 2019 a Boeing 737 MAX crashed shortly after takeoff in Ethiopia, killing all 157 people on board. What went wrong? Did Boeing hush up concerns about the safety of the aircraft? And what role did US aviation regulators play?
The crash involving Ethiopian Airlines Flight 302 came just four months after a Boeing 737 Max had plunged into the sea in Indonesia killing all 189 passengers and crew. The aircraft was grounded following the second crash and remains so today. The shockwaves reverberated throughout the industry, raising questions about Boeing and the regulatory authorities.
Both tragedies have been blamed on faulty software - software that the pilots didn’t even know existed. The documentary shows that malfunctioning software was just a small part of the picture. Instead, it alleges widespread systemic failure and blames market forces for the fatal errors. Did Boeing care more about profit than safety? Could other models produced by the company turn out to be affected?
The makers of this investigative report show just how much the production structures of commercial aircraft have to change, if we are to avoid similar crashes in the future.
Update November 2020: The FAA has lifted the order suspending 737 operations for airlines. However, it is unlikely that all airlines will carry passengers again before 2021.

How can you use SOFTWARE fix HARDWARE problem? Good luck with the MCAS 2.0. 🔴 *The Engines are still dragging on the FLOOR!!* It's aerodynamically unstable whose nose keeps on pushes up and require software constantly to keep it down. The 60 year-old physical structure still unchanged!! How can you use SOFTWARE fix HARDWARE problem - they still use MCAS. Now 2.0 - To save money!! Collusion between FAA and profit of the self-certified Boeing has no bound.

I cannot understand people willing to board machine corrected with autonomic software solution proved to be fatally flawed in the dark past of 737MAX. New and big and powerful engines mounted on 60years old airframe, insufficient amount of sensors, very poor quality control and overall low quality of the final product with F.O.D and other trash and tools, rugs and hidden junk, left and forgotten in the bodies of the MAXimum failure. *You cannot put modern F1 engine to 1964 Pontiac GTO with serious manufacturing issues* *:D* *Does not look safe to me in any way* :) It is strong *NO* from me In my personal opinion MAX will always be a fatally flawed aircraft patched with ad-hoc fixes and walkaround solutions where due to hideous and ruthless corporate desire to enrich and increase profits, 345 people died. I just read that Canada is not going to follow FAA and they will not certifie the MAX for now. They will focus on their own assessment. I don't blame them :D EASA same. They will eventually approve MAX accordingly to their own assessment. *FAA said that MAX is safe already 2years ago* ...

My professor lost her granddaughter (who was only 24) on that flight. This story gets a lot sadder when you know someone personally affected and seeing how angry they are that they can't get justice.

One should never take legal matters such as this into their own hands, but I can certainly understand the temptation. Anyone involved in allowing these aircraft to be certified and fly, from engineers to executives, should be tried for premeditated murder. Boeing and the FAA knew the MCAS software was an issue, yet chose profits over passengers lives. To me, that is murder.

If so, what do you base that on? II read somewhere that in the aviation industry Airbus' planes are nowadays considered to be of better quality, but they are also more expensive.

It makes me wonder how many plane crashes are blamed on “pilot error” when in reality they are due to defects that should have never been allowed to leave the factory.

Makes me sick knowing Boeing tried to blame those crashes on poor training and pilot error. I mean, yeah, that poor training was an iPad course that didn't qualify anyone to fly that plane. The Lion Air pilot, albeit too late, allegedly managed to take control during the final moments. And the Ethiopian Pilots were trying to read and follow a checklist while they were moments from smashing into the Earth. That's ridiculous! And the damn checklist never accounted for MCAS kicking in every 10 seconds. Only accounted for it happening once. People believed Boeing calling it poor training on Lion Air's part. But when Ethiopian Airlines, which is a pretty safe airline, crashed in a similar way with their pilots' knowledge of MCAS, and the checklist hadn't worked, people began to realize Boeing had more to do with this than they thought

Plane crashes are investigated very thoroughly so it is unlikely a pilot error is anything else but. Actually most crashes are human error which is why all this automation exists. But there is no excuse rushing to market without proper testing and certification. And using one sensor is the stupidest idea Boeing ever had. Unless you consider NOT telling the pilots about MCAS. Not sure which one is dumber.

Yes, it is even not possible to imagine the terror of the people on board of the flight knowing what's about to happen to them. Majority of these accidents are a result of a greedy industry, from aircraft manufacturers, airlines to regulatory bodies. Yet, most of the time they manage to find an easy scapegoat to blame, that is the piloting crew.

Boeing executives could have easily come out and said, "Look, we screwed up, we want to make things right. We want to make sure something like this never happens again." The fact that they covered all of this up and wouldn't even reach out to these victim's families says a lot about Boeing.

i lost my mother in this damned aircraft. Lion Air crash in Indonesia. up to this day neither lion nor boeing personally apologize to us. or openly admitting their fault. its been 2 year and boeing and Lion air did not get what they deserve. most of the victims still did not get any money from neither.

Sue them! Really, use the justice system. It will not reduce your pain but will cause real pain to these companies. Don't use the US legal system because there even if you win you'll have to pay the legal fees - something that usually does not exist in most countries (losers pay all the fees).

It was extraordinary and epic when senator Ted Cruz roasted Boeing CEO Dennis Muilenburg. Yet this criminal walked away from Boeing with almost 80 USD millions of compensation. In China this inept and corrupt CEO would've been sent to the firing wall for good.

The shipping industry is the exact same way. They push the ships way beyond its scrapping point, because rust repairs among other repairs are more expensive then keeping the ship in service until it just breaks apart. I'm relieved that my last vacation, the airline used an A319 even though 95% of their fleet is all Boeing. It was NOT reassuring to hear that the route I flew (DFW/MCO) is frequently covered by the 737. The flight I took there before the A319 flight was on a 737.

I agree! Boeing is so riled by Airbus that they will cut any corners they can to be first or get ahead of Airbus- even when they shouldn't or it can cost lives! Even their move to the Carolinas! Someone asked one of the supervisors if he would fly a plane built in that plant and he said NO! AND HE WORKS THERE!

There was a problem with an A320 so the software engineer was called to look at the issue. When examining the software he was heard to say _I didn't know it did that!_ If the software expert doesn't know what the software can do then what the hell chance does the pilot have. Also, the software is designed to *stop* the pilot over stressing the aircraft. That means in an emergency the pilot *cannot* over stress the machine while they try and save to lives of the passengers. Instead the software says *no,* I will not let you do that. I must save the machine from some damage even if the humans have to die. After all the scrutiny Boeing has undergone, I would rather fly on their machines than the software driven Airbus tree mowers. Just look at what Airbus/Air France did to that highly experience Airbus pilot over AF296. Airbus and Boeing are both bloody dishonest companies just like the automotive ones who find t cheaper to pay out on wrongful deaths instead of spending money to actually fix the problem.

@@techhelpportal7778 I'm pretty damn sure that editing the aircraft controls software to compensate for the changed flight characteristics due to the new engines, and adding new operational parameters is okay. However keeping the agreement with the customers that the MAX didn't need any additional crew training over the previous 737s (gattaca save that dollar), is where we start to see the faults. The crews of the crashed planes reacted to the seemingly abnormal behaviour of the new plane. The plane new what it was doing but the pilots compromised it due to not knowing how the new feature set worked. If the MAX required any additional training, the customer airlines would rather go buy Airbus's new Neo range. Airbus unlike Boeing expected larger diameter engines to hit the market sooner or later. That's why the sizes that now exist in the Neo range always had longer landing gear than necessary, while 737 had their low bypass engines almost licking the ground (this matters because the engineers was forced to push the engines up and forward, an unideal placement). Boeing didn't have time to develop and FAA certify everything the engineers would've liked to make things work properly. Why? It's because many high rankings in Boeing were to be given massive bonus checks if they made the deadline set by the preordering carriers and got the sales. In addition Wall Street has been pushing Boeing lately as Boeing was very been profitable for them earlier in the century, and wanted to continue the trend. And what did they accomplish? Every thing went to shit. Small carriers in especially developing countries that needed the reliability and low fuel expenses went to shit. Trust in the industry went to shit. Trust in FAA (which apparently was bribed by Boeing to play this game) went to shit. Boeing's reputation has an almost unhealable gash in it. Lastly, some 500 lives were lost in the conundrum. 500 lives lost in order for people that's already doing very well in life to earn another big bonus check. I don't think that money tasted so succulent when the news started rolling in...

Hi Eugene: isn’t it strange, we have business men with no technical expertise whatsoever running engineering companies, but people would be outraged if engineers were running banks and finance companies. .... ..... what is more important to life and limb? ....... ...... i’m pretty sure engineers would do a better job running the banks too, by the way. ...... my experience as an engineer with the MBA class, and I have a lot of experience with them, is that they are of a morally and intellectually lower standard. A lot of these people wouldn’t have even qualified for a college education 70 years ago. And now they run the world.

SADLY, the Boeing Board of Directors relied on CEO Dennis Mullenburg, a graduate Aeronautical Engineer with excellent credentials, to maintain Boeing's dominance and success; reportedly, Dennis became enamored with his Boeing stock portfolio and the profits he was making by pushing the 737MAX deadlines to bring it to market before its rival -- the Airbus A320 with the bigger, fuel-efficient engines; but those engines would not fit under the Boeing 737MAX Wing, so Dennis approved an ILLEGAL MODIFICATION OF ITS ENGINE MOUNTS, MOVING THE ENGINES UPWARD BY FIVE INCHES, WITHOUT REQUIRED FAA APPROVAL OF A SUPPLEMENTAL TYPE CERTIFICATE. So the Board fired Mullenburg at the end of 2019; although he was forced to surrender his Boeing stock, HE WAS STILL REWARDED FOR HIS FAILURE WITH A "GOLDEN PARACHUTE" OF $62 MILLION DOLLARS!

Remember how Boeing tried to blame the pilots and that the airline was in Africa and 'backwards' - meanwhile the investigation showed that the pilots had managed to override the computer and manually fly it, but it was too low and too late. In the other crashes, the plane computer couldn't be override, and the pilots fought against the plane (as seen on the flight trajectory, gaining altitude and then falling several times) . Insane to blame and use "third world country" meanwhile they are BRAND NEW planes lmao

The very fact that the system could not be overwritten as it was designed to be is clear evidence we haven't been told everything there is to know from this story. Developing conclusions when all the facts are unknown to us is foolishness. What we know is that there were technological failures, the causes of those failures and whom to blame are the real questions. Do any of us really believe a plane designer would build a plane with no redundancy for the failure of a single aot sensor? Or that mcas would somehow develop an additional bug which prevented pilots from disabling it precisely during the moment when that very system needed to be disabled? I watched that too and also the episode from 60 minutes australia about the same topic. Pretty bizarre how they did'nt ground the 737 max after the first crash and they knew mcas could cause disaster. Family members of those who died even suggested them to ground the planes but all they thought about was making profit and they didnt really seem to care about safety. They even tried to blame the pilots while it was a software issue and they didnt even inform the pilots about the new system nor did they feel the need to train them. With the second crash, the pilots were aware of the system and disabled it but still it reconnected and took the plane down and nothing they could do. As you could see in the flight simulators the system would have a mind of its own. Must have been terrifying for the pilots , flight crew and all the passengers that died in both crashes. Can't even imagine how their last moments must have been like on both occassions. It's infuriating to learn that both disasters and all deaths could have been avoided. RIP to all victims and strenght to all family and friends that lost their loved ones on both crashes. 346 lives lost....

According to 20/20, the engine not being designed for the airframe created a flaw where the plane would tip upwards. The MCAS, in theory, was supposed to correct that flaw by automatically tipping the plane safely back down. The flaw the MCAS was supposed to fix was not necessarily a fatal one and perhaps the pilots would've been able to tip the nose down safely but it was an obvious design flaw nevertheless. So Boeing already had a major design flaw, but rather than starting from scratch and correct this design flaw, they decided to cut corners and create a software that was supposed to correct that design flaw. A software that was so badly designed, it carried fatal risk. So Boeing created a design flaw, went ahead with it, created a software to window dress that design flaw, and badly designed the software meant to correct the lazy design flaw to where the software created fatal risk. And at no point did the higher ups just try to fix any of the issues that were brought up with what they created, they basically just put "tape" over everything and didn't realize, or care, they were putting human lives on the line.

The defective software (only used one of two sensors for detecting the angle of the aircraft and ignored pilot correcting it) was put in because the larger more fuel efficient engines put on a low landing gear 1960s aircraft required special mounting and caused the plane to pitch up on occasion.

@@mrsx7944 actually no Engineers at Boeing wanted more time and money to create the MAX to invest more into fixing the MCAS before it's Certification during 2013 but everytime they where either shut up or threatened by the Hegheir Ups of Boeing if they had just let the Engineers do the damn job all of this would not have happened plus the Engineers at Boeing's Puget Sound Plant were moved to Renton because they were "spending too much" according to a Boeing Whistleblower Engineer at Puget Sound and the MCAS to save cost was made by a 3rd Party Manafacturer from India to get great deals with Indian Investors once the Max came out have some Sympathy for the Engineers of the MAX the plane never had a pitch up issue and if it did it would only be slight the MAX without MCAS is safe but it was added to make the MAX handle similarly to the 737NG to once again "save costs" I think we can see a pattern in the MAX disaster they Cut way too many Corners and they loose $9,000,000,000 in the Process

As a 737 pilot, the main reason I have heard why the MCAS system was installed was to compensate for unusually high nose up moment tendencies created at high thrust setting by the new more powerful LEAP engines. If that is case it means the engines needed to be derated to a lower maximum thrust setting, or the thrust line of the powerplants need to be reconfigured by engineers to produce more manageable flying characteristics. Either way the 737 Max design violated some very basic long established rules. MCAS was installed to cure a basic design flaw that should never have been allowed to leave the engineering drawing board in my opinion. It's like a doctor prescribing medicine to reduce anxiety, and then give the patient an upper pill to cure their resulting depression.

Ryanair was going to buy Russian Jets the US forced the cancellation of the deal. They put engines too big on the 737 rather than raise the landing gear height they extended the Pylons holding them. This caused the centre of gravity to move forward VITAL to balance the aircraft and for Elevator control. To combat it they programmed the software to keep a constant nose up attitude when it failed the 737 did exactly what it was designed to do go nose down and fly into the ground. Which they tried to hide by not informing pilots about when the software failed they had no idea what was happening.

In the ethiopian crash they managed to cut off the electric trim wheel switches which disabled the MCAS nose down trim inputs, but were unable to manually adjust the horizontal stabiliser due to the high airspeed and aerodynamic forces on the horizontal stabiliser (due to leaving autothrust on climb power). So they switched the electric trim switches which reconnected MCAS causing further nose down trim which caused it to descend at high speed into terrain.

@@techhelpportal7778 American companies are famous for doing this, look at the Auto industry for example, GM pulled out of many markets like Europe, Australia and India for example for creating cheaply made trash at high prices and not catering to the markets' need, part of the reason the Japanese slaps America in the face in the Auto industry

Having learnt to fly under the FAA & EASA i can tell you the FAA safety standards are very poor in comparison to EASA. I'd feel much safer flying in Europe than around USA.

I did ride the same flight 2 days before same plane crashed. It was overwhelming situation.. i was unable to perform my day to day activities for few days.. i was afraid even to drive on the street.. not purposely to be lazy.. But, some casually told me that i should celebrate because i dodge the bullet.. or some say i overthink things.. i was not..

Reading that email around the 35 minute mark was so similar to a construction company I recently quit working for, who had recently hired a former Boeing employee. We were building a multi-million dollar house in which I was the project superintendent, very complex, he had NO experience in this industry (I have 40 years), yet he was always pushing unrealistic schedules, disagreeing with things that I said, and hiring subs that didn't know what they were doing. I couldn't stand it! I often wondered what part of this Boeing debacle, if any, he played.

When I was a Quality Assurance Manager and Audit Team leader I would tell CEOs and VIPs that we can make lots of money safely and that "effective" QA will save the company millions in the long run. I also coined the phrase, "If you think safety is expensive. Have an accident." Boeing is paying now for taking short cuts. Mean while the FAA is hiding and laying low.

how can these top management sleep at night? knowing what exactly they did? In an older documentary, it has been reported about their quality/inspection before these 737MAX accidents. In any topics on Boeing Vs Airbus, there will be thousands of comments defending Boeing, most likely from it's employees.

The sad thing about it is that we all know very well that those executives and managers who ignored safety protocols will do just fine while many workers who had no other choice but to comply with those ridiculous orders will be fired. Not saying all workers are innocent or all managers are evil though.

I was an Aircraft Electrician for four years with the USAF and seven more for the Dept. of the Navy. I never worked for Boeing, but I worked on many Boeing aircraft. I am terribly sorry to hear of your loss. It is a mechanic's worst nightmare to think that something he did caused a crash. I never worked on one that crashed, but I did help re-build two CH-46D's that had crashed beforehand.

Don't be rediculous. Insurance companies don't loose money. When claims exceed planned what they budgeted, they just raise the rates. If they don't they aren't going to be around for very long. So, it's going to bite Boeing in the ass anyway, even if they are self insured.

I want to see the son of Akio Toyoda dies in a Boeing 737 MAX when the MAX crashes in Japan. Even tough Akio Toyoda is the CEO and owner of Toyota but their is one thing that Boeing has done in 1996. Back in 1996 Boeing paid a Dutch airplane company to go out of business because that Dutch airplane company was running their business like Toyota runs it’s business that was the reason why. The name of the company was Fokker. So for this I bet someone related to Anthony Fokker lives in Japan and he is probably dating the daughter of Akio Toyoda on what will happen if the son of Akio Toyoda dies in a Boeing 737 MAX to see if Toyota brings back the Fokker name. So what will happened to Boeing after the son of Akio Toyoda dies in a Boeing 737 MAX after they find out that Boeing paid lobbyist to put a Dutch airplane company to go out of business because that company was running their business like Toyota runs it’s business?

The last thing the engineers forgot was that the boeing 737 flight computers thinking that the plane was going to stall even at a decent and good speed. The computers were like dive dive dive.

Its not wrong, planes are fly by wire because many require it now. Stealth planes and fighters were deliberately made or inherently unstable. They failed by not disclosing the requirement for more training.

I saw that in review, as Boeing likes to butts in a lot as what Boeing has done back in 1996 is that they paid money to a company based in The Netherlands to go out of business because that Dutch airplane company are running their business similar to Toyota runs it’s business that is the reason why.

I have a friend who flies the 787 dreamliner recently upgrading from the 757 and 767 which he flew for 20 plus years. When I asked him if he wanted to fly the 737 his words were, 'I don't want anything to do with no 737. Enough said and in the end he's happy as hell flying long hauls in the dreamliner...said the flight deck is like stepping into a starship

How about Chinese pumping water in meat to make it heavier so they can charge you for more. A specific store in my town soaks ginger in water to keep it plump so it looks 'fresh'. My mom worked with the sister of the owner and that's how we know.

I watched one of these from panorama on BBC Iplayer and I’m no plane mechanic, engineer, designer, etc. But even I could see the flaws. They were so obvious. What made it worse was that Boeing made no plans to change them.

Lets correct a little bit of miss-information here. I'll make it quick. MCAS was developed and implemented because when the aircraft is at a high angle of attack, the engine nacelles would help produce additional (and at times unwanted) lift. The 737 MAX did not meet certification requirements for control 'feel' when at high angles of attack during a wind-up turn maneuver. The controls shall become heavier as the pilot adds more elevator control back pressure while airspeed decays. In order to meet this requirement 'they' decided to use the horizontal stabilizer and trim the required control surface 'feel' into the solution. At the time only one sensor was used as input to one flight control computer because that's how the previous 737's did it.

short answer: Boeing had to make the engine bigger for more efficiency. To accomodate that bigger engine, they had to place it more up front and higher than the previous generation so it wouldn't hit the ground during landing. The drawbacks of that, the new Max center of lift is more forward. It makes it harder to recover from stall. Boeing knows this problem so they develop a new system called MCAS to help the pilot to avoid stall by giving MCAS to override the control of the trim and elevator at the back. One big weakness of this system is that it relies heavily on other sensors, such as air speed sensors and AOA. There are 2 of it for each kind on the Max. If the sensors are failing, MCAS will dive the nose of the aircraft (because MCAS thinks that the airplane's nose is too high from the input of the faulty AOA sensor). The pilot and copilot have to turn it off and switch it into manual trim mode to override the MCAS system. Both Lion and Ethiopian incidents are related to the sensors and MCAS.

The wing is overloaded from decades of add-ons. 62% increase in takeoff weight, 25% increase in wing area. The new engines MUST run at full thrust because they need airspeed to generate enough lift.

As an inspector at a machine shop I would say that the max incident was not a wake up call, but the tip of the iceberg. Metrology is not just about studying measuring tools, but also psychology with regards to the people that use the tools. When you streamline a process, it is the fact that 1 out of 20 people actually want to see and/or cause a crash that gets left out of the equation.
