I believe that the emotional pain really is to much for borderlines to handle in that moment when they attempt suicide or self harm, maybe they do not want to die all the time, but with mood swings in the swing of the mood they may swing into suicide. I think emotions are very powerful they do have physiologucal effect and are felt like other pains like burn a patient may experience. I think it may be harmful to not believe that they are in pain for whatever reason, like if they are cutting it may not be for attention but it may be that release from the act of cutting and the endorphins of the cutting and distraction of the intensity of the act from the intensity of the emotions. That intense fear of abandonment could be driving these behaviours, and a healthy person may see that as manipulative, and it is, but in the disordered mind, it is extremely painful for them to have that perception of abandonment in this world. I cannot put it into words, but I believe it may contribute to stigma and misunderstanding of borderline.
