if it's mostly not actually under the muscle why would surgeons recommend a method that affects bodily anatomy just for it to not actually make much difference?

I’ve had 4 children and breastfed them all. I’ve been seen for a consultation which the doctor wanted to do a lift with implants under the muscle for my desired outcome. My question is must I do a lift and under the muscle? I truly just want the volume back and feel that the lift and under the muscle is going to be major healing. My youngest son is 3 and I need a quick recovery time as I have a very active lifestyle.
I recently had my 20yr old perfectly round 425cc leaking silicone implants (under armpit) removed. My new surgeon did a lift and put in 615cc gummy implants under the muscle. I am VERY unhappy with these new ones. My breasts are smaller now?? They have loose skin that hangs separately away from the implant from the nipple to bottom of my breast. My boobs were always round, now they are triangular/natural looking which is soo ugly to me. He said he cant remove excess skin from the side or above the nipple to make them taut, because that will make the tissue die?? Is this true? How do I get them taut and ROUND? He ALSO said that if he moves the same implants ABOVE the muscle, they will be bigger? Is this correct? He didn’t even use a mesh sling to keep everything together.
Im a naturally a muscular girl who lift a lot of heavy objects at work (underground miner) and is extremely active. My surgeon said to go over the muscle due to body type and life style. Im the 95% who wants to go under and am worried about my perky size D boobs sagging over time if we go over the muscle. HELP
