The "small person" was used as a prop to paint the doctor as a bad person ahead of time so they can more easily convince the audience he was in the wrong with the pronoun issue. They loaded the viewer with a preconceived notion using something everyone would find wrong to propagandise ideology. I'd say that shows the show writers in a much worse light than the character.
