Bundle branch blocks happen when there is an obstruction in one of the bundle branches. The names “left bundle branch block” and “right bundle branch block” indicate the side that is affected.
In a normal heart, the two ventricles are depolarized simultaneously by the two bundles and contract at the same time. In bundle branch blocks, the UN-affected ventricle depolarizes first. The electrical impulses THEN move through the myocardium to the other side. This results in a DELAYED and SLOWED depolarization of the affected ventricle, hence a broader QRS complex - typically longer than 120 milliseconds; and a loss in ventricular synchrony.
Left and right bundle branch blocks are diagnosed and differentiated by looking at ECG recordings obtained from the CHEST leads, which register signal movements in a horizontal plane. Of these, the most useful are leads V1 and V6 as they are best located to detect impulses moving between the left and right ventricles.
Activation of the ventricles starts with the interventricular septum. In normal conduction, depolarization of the septum is initiated from the left bundle going to the right, TOWARD V1 and AWAY from V6. This results in a small positive deflection in V1 and a negative deflection in V6. The signals then move both directions to the two ventricles, but as the left ventricle is usually much larger, the NET movement is to the left, AWAY from V1, TOWARD V6. This corresponds to a negative wave in V1 and a positive wave in V6.
In RIGHT bundle branch block the initial septal activation is unchanged. The left ventricle depolarizes NORMALLY toward V6, away from V1, producing a positive deflection in V6, negative in V1. The impulses then REVERSE the direction spreading to the right ventricle, hence a subsequent negative wave in V6, positive in V1. Lead V1 gives a characteristic M shape with a terminal R wave, while V6 sees a broader S wave.
In LEFT bundle branch block septal depolarization is REVERSED, from right to left, giving a negative wave in V1. The right ventricle activates first, with the signals moving to the right, generating a small upward deflection. Depolarization then spreads to the larger left ventricle, resulting in a large downward deflection. Lead V6 sees the opposite, producing a wide, characteristic “bunny ears” QRS complex with two R waves. In some cases, right ventricular depolarization may not be visible.
Some people with bundle branch blocks are born with this condition. They usually do not have any symptoms and do not require treatments. Others acquire it as a consequence of another heart disease. These patients need monitoring, and in severe cases, a pacemaker may be required to restore ventricular synchrony

Wow! That animation and explanation explained it far better for me than the hours of reading I did. What helped the most was showing the depolarization path with the ECG in real time, and the location of the two leads.

Leannyly If I am not mistaken you see 🐰 ears in RBBB but since those notches can be seen in LBBB too, you should check slurred S wave in V6 first rather than "bunny ears".

I found out I had a RBBB because of near-syncope. I've now had four EKG tests, and the last three have shown RBBB. It's interesting to me that as soon as this video explained what they looked like on an EKG, I recognized it immediately because I was curious and looked at the EKG readout while in the ER.

@@Luminous.Dynamics Personally, I had to cut out caffeine above 30mg/day or 100mg/week (i.e. a cup of black tea per day, 3 days a week, or 6 cups of decaf coffee in a day, and 20 in a week), shy away from any cardio workouts, and be prepared as a fall risk. I haven't had a serious fall in over a year, and tend to only faint about once or twice a month. The RBBB was secondary to POTS and some other circulatory problems (a mitral valve prolapse iirc), so your mileage may vary on these changes.

@@wildhunt2235 No, thankfully. It does increase risk of syncope, and correlates heavily with other cardiac issues (such as those with orthostatic tachycardia), but it in and of itself is *mostly* harmless.

I have several questions if anyone is able to answer them. 1- Why are the magnitudes for depolarisation in V1 and V6 not the same? Is it to do with the RV size compared to the LV? This will help my understanding with some of the other questions most likely. 2- In NORMAL - why is it not an RSR pattern in V1 to mirror the QRS pattern in V6? 3- In RBBB - why is the S wave in V6 so rounded and wide? I do not understand why it is a rounded shape, should it not be a sharp point? 4- In RBBB - why is the R' wave in V1 larger than the S wave in V6? Shouldnt the large electrical shift toward V1 be reflected in V6? 5- Not to do with blocks - Why is the hearts ejection fraction not 100%? Why does it keep the % in the LV post contraction?

1 - The left ventricule is much bigger and thus has more muscle tissue which means more depolarization/repolarization. 2 - In normal you have a rS or RS because at the end of the contraction the cell comes back to normal and thus it appears as isoelectric line (its not another depolarization, its just the end of the cell contraction) 5 - There is always some residual blood into the LV, it is called the end-systolic volume just like in the lungs, there is always still some air or it would collapse. Since the ejection fraction = stroke volume / end diastolic volume and stroke volume = EDV - ESV, the ESV is never equal to 0, so the SV will never be equal to EDV, so when you calculate the EF your stroke volume will be inferior to the EDV, so < 1. Can't help you for 3 and 4

Well I live with rbbb and idk if I have anything else but I have like trouble showering with warm water and sometimes I feel faint for about 3 secs and my head feels hot I have trouble doing a lot of tasks that other ppl wouldn't and my heart goes very fast I think I have a atrium abnormality too though im 19 lol hope this helped I don't mind answering some stuff

My mother has left bundle branch block from 2016 till date ..we noticed 2020. Due to blood pressure check up is 160. Even if doctor prescribe medicine bp no change. Not decreasing . Doxtor changing medicine for upcoming two months .. I dont know it isnserious issue or my mom adopted this condition .. Im confused

My boyfriend is 55 and he just had an ekg as part of his physical he had no preexisting heart issues this is the first time hearing anything. Our concern is this is causing his ED and Aldo should he be followed by cardiology? If anyone can answer this. His doctor keeps saying it's nothing to worry about but he's lost a ton of weight after eating extremely unhealthy diet for years and now he's cleaning up his life. Any thoughts?

What's the side affects of drinking alcohol with lbbb? I have got calcium score of 0 plus i done a stress test and also had an eccocardigram and my cardiologist was happy said he will see me in 3 years.. I'm 41 and im quite fit i love the gym and eat clean but if i do drink which is not regularly i am literally unable to do anything for a week i don't have a headache or feel an upset stomach sick i just have shortness of breath and chest pain on my left side every time, Can anyone on here help me with any advice I'd really appreciate it.. Ps I have been stressed alot the last 2 years and my cholesterol went to 7.3 but since lock down ended its now 5.8 my cardiolst did want to put me on statins the lowest dose but i decided ill do it by cutting out red meat eggs etc but the high cholesterol thing is only recent... 👍

R wave is the positive wave in the QRS complex, S is the negative wave. Usually the QRS complex ends with a negative wave - S wave, but in RBBB the QRS complex has a M shape - 2 positive waves - so two R waves, one at the beginning (first, initial R) and the other at the end of the QRS complex - second R wave or terminal R wave.

Some people call the M shape rSR' in V1 of RBBB "bunny ears" , while some people don't (us included) because the 2 "ears" are very much different in size. It's really doesn't mater what you call it, the important thing to remember is the shape.

No as long as you don't have fainting spells or chest hurts you can live a long time, I'm 52 and have had it most of my life, I played high school football with no problems...it's an easy fix though if it gets serious, my doctor told me a pace maker would take care of the problem, which he doesn't see any major problems for me yet, says some people can live a normal life without problems.
