From what I saw she is and she isn't coming back. They have a lame character who just a cliche angle type. She jokes around and has fun with itodori just like nobara did. I understood gojos that made sense, this one truly frustrates me because it was good writing with reason up till this point... now though it was stupid and terrible writing he basically only killed her off because he wanted to implement a new character because he got bored of her development. He didn't say that personally I'm just assuming with the information I know.

Depends. I mean if the attack landed properly her whole head would have exploded at least. Possible she blew off half her own face and survived. Story is very vague about her fate, perhaps intentionally so.
