Calcium forms the basic structural component of bones in the form of calcium hydroxyapetite. Major function of calcium inlude cellular signalling, neuronal transmission, contraction of soft and skeletal muscles.
Only 1 percent of the calcium is present in the circulation. Of that around 50 percent is free while the rest is bound to albumin and organic anions . The part of the calcium which is free is called as ionized calcium . It is the active form of calcium.
Reference range of
Ionised calcium : 4.6 to 5.8 mg/dL (1.15 and 1.30 mmol/L)
Total Calcium : 8.5 to 10.5 mg/dL
Normally total calcium reflects the amount of ionized calcium hence testing of ionized calcium is not routinely required unless in special conditions involving parathyroid disorders.
Corrected calcium (mg/dL) = measured total calcium (mg/dL) + 0.8 (4 - serum Alb g/dL)
Corrected calcium is an estimate of the total calcium concentration, had serum protein (albumin) concentration been normal. Although "corrected" total calcium reflects ionized calcium more accurately than uncorrected total calcium in patients with abnormal serum protein concentration, none of the correction formulae are entirely reliable for all patients.
The two principal clinical situations in which total calcium concentration does not sufficiently accurately reflect ionized calcium activity are:
1.Those in which the patient's serum protein concentration is abnormal
2.Those in which the patient is suffering disturbance of the acid-base balance

Is serum calcium test a dependable method to assess whether the person needs to take Calcium supplement or not?

Serum calcium can help in diagnosis of calcium deficiency or excess, especially in the presence of clinical symptoms . For considering the need for supplements several other factors which includes vit D Levels, Bone density and use of medications like steroids all come into play . Mechanism is like, during the initial deficiency of dietary calcium, it will be moved from the bone into blood to compensate. So even in the presence of calcium deficiency the serum levels can still be normal. Bone density test can give u a good idea. When density is decreased it indicates calcium supplementation Dr KMD

No both are different. Calcium while in circulation is bound to protein . That part of calcium which is active is called as ionic calcium . Regular calcium test value is indicative of bound calcium + ionic calcium Dr Kasi MD

yes its still normal 2.15-2.50 mmol/L (8.6-10.0 mg/dL)
