Although on pc we can get 2 because they use a box clip to clip inside the act 3 basement without a bear trap and they push the tv in act 3 basement all the way to the gate, and then fling themselves over the gate which isn't possible on console since the devs raised the gate in order to prevent speedrun skips, forcing us to pick up the chair
most of these strats, when perfected are faster than any speedrun strat! edit: oh and btw the letter isn't an item that you pickup, you interact with it and the game starts a cutscene
The letter is counted as an item because for a split second when you pick it up it appears on your item list, and also there are some bugs that basically let you restart the game in old saves (so it looks like you are starting act 3 but you have already picked up the letter) and in order to progress you have to place the letter down and pick it up again
