Robert: Hello my name is Robert Russo, and I'm a rheumatologist at BJC Health. And today I'm gonna ask physiologist Rachael Butterworth, who has expertise in anklosing spondylitis, to take us through the chest expansion test which is used in the assessment of anklosing spondylitis. Thanks Rachael.
Rachael: Thanks Rob. So I'm going to go through the chest expansion test. The first thing we have to do is to find the the fourth intercostal space, and the way to do that is to find the first and second rib space and then come down until your on the fourth intercostal space. The patient then has to get their hands on their head, making sure you still stay on that fourth intercostal space. We get a tape measure, come around the patient, and you need to hold quite firm and measure what that is. Fully expirate, so all the way out, and the full breath in. And all the way out again, and all the way in. And all the way out. And take that measure, the best measure of two tries, making sure your not restricting the patient by holding the tape to tight.
Robert: Hi Rachael. Thank you very much for demonstrating the chest expansion test, which is used in the assessment of anklosing spondylitis. So Rachael how do you determine if the test you measure is abnormal?
Rachael: So what we look at if we use the example of Erol just then, his measure was five centimeters. If we look at his average values for him for age and for gender, we would expect six and a half centimeters. Now restriction is noted if its two and a half centimeters below the expected average, so he still fits within normal.

Good videos - helping my study task for AS case! Can I ask how you would go about this test with a female patient? Would you use the tape a bit higher up the chest or would you just not do it - is there an alternative? Thanks

Hi Peter, thanks for the feedback. Your question is a good one as well. We would explain to the female patient where we would need to measure and ask for permission to perform the measurement. Explain that the tape will need to go over their breast with the reason being that the standard is to have it at that point. Once permission is given, perform the measure. The tricky part of this measure be it whether the patient is male or female is that you are using the same reference points all the time for re-measurement. You could always use different landmarks for females but then you would not be able to compare differences to the so-called "normal" values that exist for the 4th intercostal space. Thanks Wan Kun, The normative values for chest expansion have been released by Medicare Australia (as part of the Department of Health and Ageing). This can be found on the medicare Australia website. RU-vid won't let me place the link up in this reply so just google "chest expansion normative values".

Random question, but would this result be skewed in any way if the patient had a collapsed lung?

I would say yes as the collapsed lung would impact the amount of air that could go into the lungs and hence expand the chest.
