I was reading in Harrison this subject and it said that 50-60% of patients in development countries are diagnosed with systematic blood tests and they usually have no symptoms other than maybe fatigue, because of the anemia, so my question is: in the blood test what makes you suspect of CML? Is high wbc >100,000/uL and anemia (like you mentioned in the video) what arises the suspicion of CML?

when this video appeared in April 2020 I was recovering from a Micro Perfed Colon a side effect of Bosulif,My PCR had risen to 0.17 while taking CBD Gummies for inflammation, I was prescribed dasatinib but told to wait for the next blood test to resume treatment. in the interim I went to resupply my CBD Gummies,but due to supply was offered 500 Mg Tincture, I got 4 vials and consumed 1 per day and then it was time for my next blood draw, and the Nurse called and said "your results are good"

What to do if patient has taken every generation of TKI without long term sustainable effectiveness? What if a patients body/blood has been resistant to all these generations of tki going on about 6 years?

For the 3rd line therapy, asciminib has shown more effective than bosutinib. Updated results from the phase 3 ASCEMBL trial confirmed the superior efficacy and tolerability of asciminib over bosutinib in patients with chronic myeloid leukemia in chronic phase after treatment with two or more tyrosine kinase inhibitors. The updated data, which were presented at the 2022 ASCO Annual Meeting, also showed that a higher proportion of patients assigned to asciminib achieved a BCR-ABL1IS ≤ 1%, which was associated with better survival outcomes. In the ASCEMBL study, patients with CML in chronic phase were randomly assigned to twice-daily asciminib (40 mg), a BCL-ABL1 inhibitor targeting the ABL Myristoyl Pocket, or to once-daily bosutinib (500 mg). In total, 157 patients were assigned to asciminib, and 76 patients were assigned to bosutinib. With a median follow-up of 2.3 years, the updated data presented the major molecular response rate at 96 weeks, a key secondary endpoint of the study. At the time of reporting, 53.5% (n = 84) of patients in the asciminib arm and 19.7% (n = 15) of patients in the bosutinib arm continued to receive treatment. In both arms, lack of efficacy served as the most common reason for treatment cessation (ascimib, 24.2%; bosutinib, 35.5%). The major molecular response rate in the intent-to-treat population favored the asciminib arm (37.5% vs 15.8%). When the researchers adjusted for major cytogenetic response status at baseline (Philadelphia chromosome-positive metaphases ≤ 35%), which served as a stratifying factor, they observed a difference of 21.7% (95% CI, 10.5-33; 2-sided P = .001). A greater percentage of patients in the asciminib arm achieved BCR-ABL1IS ≤ 1% at 96 weeks (45.1% vs19.4%). The probability of maintaining major molecular response for 72 weeks or longer was 96.7% (95% CI, 87.4-99.2) for patients assigned asciminib and 92.9% (95% CI, 59.1-99) for patients assigned bosutinib. The probability of maintaining BCR-ABL1IS ≤ 1% for 72 weeks or greater were 94.6% (95% CI, 86.2-97.9) for asciminib and 95% (95% CI, 69.5-99.3) for bosutinib. The median duration of response favored treatment with asciminib (103.1 weeks vs 30.5 weeks), and although patients assigned asciminib had longer intervals on treatment, they experienced fewer toxicities. Compared with bosutinib, patients assigned to asciminib had fewer adverse events (any grade, 91% vs 97.4%; grade ≥ 3, 56.4% vs 68.4%) and adverse events leading to discontinuation (any grade, 7.7% vs 26.3%; grade ≥ 3, 7.7% vs 19.7%). The most observed grade 3 or higher adverse events were thrombocytopenia (asciminib vs bosutinib, 22.4% vs 9.2%), neutropenia (18.6% vs 14.5%), diarrhea (0% vs 10.5%), and increased alanine aminotransferase (0.6% vs 14.5%). meetings.asco.org/abstracts-presentations/208188

Hi doctor I am 74 yrs. Earlier I was taking dasatinib but all the times side effects. Now I am taking Imatinib 400 but blood counts decrease every month I.e. below the normal parameters. Should I stop taking medicine?

The FDA has approved asciminib to treat patients with chronic Philadelphia chromosome-positive chronic myeloid leukemia (CML). A release from the manufacturer described asciminib as “the first FDA-approved CML treatment that works by binding to the ABL myristoyl pocket.” The approval is for patients treated with two or more tyrosine kinase inhibitors and those with the T315I mutation, according to the release. The approval follows results from the phase 3 ASCEMBL trial, which compared patients treated with either 40 mg asciminib twice daily or 500 mg of bosutinib once daily, and the phase 1 NCT02081378 study that included patients with the mutation. The primary efficacy outcome of the ASCEMBL trial was major molecular response (MMR) at 24 weeks, with an MMR rate of 25% (95% CI: 13-33) in the asciminib arm compared to 13% (95% CI: 6.5-23; p=0.029) in the bosutinib group. The release noted that with a median duration of follow-up of 20 months, the trial had not reached a median MMR.
