To begin an ACDF procedure an incision is made in the neck and the muscles are retracted. The trachea and the esophagus are exposed and moved out of the way by retraction. The injured disc is then removed and a graft is placed in the disc space. A plate is then secured with screws to the side of the neck and the incision is then closed.
In some cases, these procedures do bleed. Typical small hematoma's are not a big deal and only cause little pressure on the esophagus and no pressure on the trachea. When the drain does not function or is not recognized the hematoma can grow causing pressure on the trachea and esophagus. This pressure causes airway obstruction and hinders the ability to swallow. In that case an endotracheal tube is put down the airway so that the patient can breath.
One other problem associated with hematoma is blood gathering in the head. The red arteries bring blood up to the head and the blue veins bring blood away form the head. The veins are a low pressure system and the arteries are a high pressure system. A hematoma can compress the veins, but not the arteries, so the arteries will continue to pump blood into the head and eventually into the neck and shoulders.

I had a hematoma after my c4-c6 acdf. It started to develop during the night and on the following morning I was about to suffocate. Fortunately enough I was still in hospital when it happened and the staff's prompt reaction saved my life. I was induced into coma, they drained the hematoma and brought me back 4 days after. I reckon I was lucky.

Thanks for the model of where each vertabrae is. I've been going nuts trying to figure out if my issue is C3-C4 or C2-C3. I keep conflicting skeletal models but this one on a human head and neck make more sense to me. I'm post op 3 1/2 years C5-C7 and 2 years C3-C4 ADR and I swear I am so hypermobile and can't function normal with my neck because of the ADR (It's a Mobi-C). I know this video isn't about that but it did help me a bit, and I appreciate it... an appreciate talks of complications here (I've been fortunate enough to really have none of this from either Op)

I had a c3-c6 ACDF done in Scottsdale and developed this complication of the posterior hematoma, I'm 35 days post surgery, spent 3 weeks xtra in the hospital and rehab. However they did not remove it, I'm still unable to consume food other than liquids, 12 cans of Ensure a day, lost 35 pounds.And have bad acid reflux, I was told maybe three months to resolve, then my first post op consult told me 9 months. I went to my other doc and he was upset that more aggressive actions were not taken to remove the mass 6 cm, and keeping my epiglottis open with retrograde liquid entering trachea. He said the acute swelling should be resolved the the mass will probably not resolve as it has turned to a dried mass of cells and whatever. What's your opinion how would you address this complication?

Christy Wright Christy, I'm now almost one year post ACDF, they will not tell you after the surgery you will become ENDUSED Scoliosis, I'm 48 degrees starring at the ground I STRONGLY REGRET THIS SURGERY....I WOULD NEVER RECOMMEND TO ANYONE THIS ACDF. However, search for a doctor in San Francisco, new procedure, does not fixate your vertabrae it's new technology. The doc actually reversed a fusion of ACDF an placed and articulating replacement vertabre and returned the patient who was fixed and fused back to full motion. Please communicate with me before you opt for the old style ACDF

Khamsin First, I was wondering if you found out what was causing your symptoms and if you've ever had it resolved. I hope u have and are doing well now. I am having the same problems! I feel like someone has their hands around my neck 24/7, choking me. I have Dysphagia, SOB, choking on food! I have inhaled food on multiple occasions and can't inhale until I relax and try to inhale through my nose and forceful coughing before food will come out of my airway and lungs. I had 2 very Big Surgeries on my entire Cervical spine both Anterior and Posterior surgery after it was discovered that my spinal cord was 99% compressed and 1 wrong move would've caused me to become a quadraplegic. I had surgery 8/1/16. They replaced several of my vertebre with my bone and every inch of my Cervical spine is full of hardware from the back of my head to my thoracic spine! A few months later, 1 of my vertebre was almost puncturing my skin! You could see the color of the bone and shape. Eventually, the hardware pushed out more and was millimeters for breaking the skin. On 9/26/18 they cut the protruding hardware making it shorter. Now I feel like someone's hands are around my throat. I'm sl

I ‘ve had 2 ACDF’s surgeries and now have a throat blockage. I’ve been to an ENT doc, had swallowing tests all confirm there is a blockage- it looks like the hematoma but they are thinking it’s scar tissue since it’s been 10 years from my last surgery going in thru the front (I had a 3rd but they cut thru the back super painful). I came here looking to see if there was any info on scar tissue blocking throat. I go for more xrays tomorrow but my ent & ortho isn’t thinking like this problem is important .

I'm sorry for U😕, it seems that they may not be listening to "ur vailid concern" regarding ur health. NEVER FEEL BAD EXPRESSING UR CONCERNS IF U FEEL U ARE NOT BEING HEARD OR FEEL BAD SEEKING A SECOND OPINION. This is ur health. It took 20+ years (many breaks w/out med coverage) before any doctor took me seriously enough to push for an MRI (only x-rays which never showed where pain was originating from) regarding my chronic back pain n eventual chronic neck pain. The sad n ugly truth is many Drs. are quick to write a band aid Rx to cover up a possible legitimate issue because it's EXTRA WORK, make U feel like it's really nothing and possibly over exaggerating or quick to say it comes with age etc. All because it's easier than taking the extra 5-10 min listening to valid concerns, suggesting extra treatment options, or just plain doing their job to at least make an attempted referral 😡 When the need arises and it has, I've had no problem telling a few....Listen, I know it's no big deal to U because ur not feeling my discomfort, it's not like ur paying my medical bills so can we make this easy n just do ur job n make a referral at least or I'll find another who will take me seriously after I complain that U are failing to address my concerns sorry🤦 It upsets me when Drs do this I HOPE U FIND THE SOLUTION TO UR HEALTH CONCERNS. I WISH U BETTER HEALTH SOON N I WILL KEEP U IN MY POSITIVE THOUGHTS. BEST OF LUCK To U....TAKE CARE🙏🙏

The healthcare system is a scam and these doctors should be ashamed of themselves for still continuing with this surgery. It ruins peoples lives. I am 4 months post op after a C6 7 T1 fusion. I have so many more problems then I had before surgery. My surgeon did not tell me I have a reverse curve before the surgery and that it is what caused the discs to degenerate in the first place. Having excessive force on the discs is so painful. Now I can not return to work and have applied for disability. I really hope one day they stop this procedure and more people start to use the laser disc repair like Deukspine institute in Florida.

Shit, i feel i have problem now after 6 weeks to swallow. Must bite the food very good so i can take it down. c5c6c7,after 8 week I take away the collar.
