overall good, only thing I would supplement is "ventilation", it is important to provide constant fresh air to confined space, especially when breathing apparatus is not available
Great Effort, citing mostly the chemical and biological hazards but more work has to be done on the control measures for mechanical (engulfment by liquids and solids), electrical, light etc. very good work
If you are in a company that doesn't do follow this then you are in a company that is risking a tragic and deadly accident. Ventilation may be required and should be considered for safety in circumstances.
1. Using pure oxygen as ventilation method inside the confined space is very dangerous and not advisable. 2. Oxygen should not be used as ventilation because it will the atmosphere flammable.
