Heart failures can be due to an inability to PUMP effectively during systole - SYSTOLIC heart failure, or inability to FILL properly during diastole - DIASTOLIC heart failure.
Heart failure can be right-sided or left-sided .
In systolic heart failure, ventricular contraction is compromised. This may be caused by any condition that weakens the heart muscle:
- Coronary artery disease/Ischemic HD:
- Dilated cardiomyopathy
- Hypertension:
- Valvular heart disease: Damage to the valves, such as stenosis
The effectiveness of ventricular contraction is measured by the EJECTION fraction. The normal range of the ejection fraction is between 50 and 70%. In systolic heart failure, it drops below 40%.
In DIASTOLIC heart failure, the ventricle is filled with LESS blood. This may be because it is smaller than usual, or it has lost the ability to relax. The ejection fraction may be normal, but the blood output is reduced. The ejection fraction is therefore commonly used to differentiate between SYSTOLIC and DIASTOLIC dysfunction.
Examples of conditions that can lead to diastolic heart failure include:
- Hypertrophic cardiomyopathy
- Restrictive cardiomyopathy:
- Hypertension
Regardless of being systolic or diastolic in nature, left-sided heart failures share a common outcome: LESS blood pumped out from the heart. As a result, blood flows back to the lungs, where it came from, causing CONGESTION and INCREASED pulmonary pressure. As this happens, fluid leaks from the blood vessels into the lung tissue, resulting in PULMONARY EDEMA, a hallmark of left-sided heart failure. Accumulation of fluid in the alveoli IMPEDES the gas exchange process, resulting in respiratory symptoms such as shortness of breath, which worsens when lying down, and chest crackles.
RIGHT-sided heart failure is most commonly caused by LEFT-sided heart failure. This is because the INCREASED pulmonary pressure caused by left-sided heart failure makes it harder for the right ventricle to pump INTO the pulmonary artery. This results in SYSTOLIC dysfunction. In compensation, the right ventricle grows thicker to pump harder, which reduces the space available for filling, eventually leading to DIASTOLIC dysfunction. Other common causes of right-sided heart failure include chronic lung diseases which also raise pulmonary blood pressure.
As the right ventricle pumps out less blood, the blood, again, backs up to where it came from, and in this case, the SYSTEMIC circulation. This results in abnormal fluid accumulation in various organs, most notable in the feet when standing, sacral area when lying down, abdominal cavity and liver. The fluid status can be assessed by examining the distension level of the jugular vein.
Heart failure is usually managed by treating the underlying condition, together with a combination of drugs. ACE inhibitors, beta blockers are used to reduce blood pressure in patients with systolic dysfunction. Diuretics are used to reduce water retention.

I have watched numerous video’s on HF trying to learn & understand, as I suffer from this condition, apparently my readings where below 10% & I was still walking around, mind you three steps then I was gasping for air. This is by far the best video on the subject and as well as understanding the problem as well as the possible causes, I also have a better understanding of what the doctors and nurses are talking about. 10/10 Thank you and especially the doctors and nurses. 😊🙏

I may have this- one doctor says " decompensated heart failure" another says "diastolic dysfunction", had cardio- testing this AM- wonder if they are both right, both wrong or 1 correct? I'm 77, so i think stiffened cardiac walls- we all may be right but I'm not planning to "push up daisies" very soon. Somehow i want fixed!😢

Hey, I’m trying to understand diastolic HF a bit better. Why is there eventually an increase in diastolic pressure? I understand that in systolic HF, there is adequate filling, but impaired contraction, resulting in an increasing end-diastolic pressure. Can anyone clarify the process in diastolic HF? Thanks.

Hey, I hope you already got your answer you were looking for but if not, its because the left ventricle becomes thick and rigid and less able to expand. In order for it to expand enough to pump blood your body tries to increase inotropic response and force more blood into the space to force more expansion. But since pushing more blood in doesn't always work if the muscle still can't push it out, the blood backs up into the lungs.

Diastolic pressure is releted with peripheral resistance...when heart can't be relax ..the blood can't enter through right atrium...then the volume of blood will increase in superior and inferior vena cava .. which lead to increase peripheral risestnce..the increase distolic p

I had this in November 29th 2020 in hospital 🏥and was in for 4 days then I went 🏡home.had heartfailure due to the stupid covid viruses that have caused the problem with the virus that has caused the problem with the most recent experience has been doing a great job with a new one and get a good feeling about it and get the vaccines to go through the process of doing things to be able to make a difference to my life as a whole lot of the person who was in a good mood for a little bit to be honest with me and to make sure it is not going to make it to a brain injuries.

Question, since normal range of EF is between 50-70%, systolic EF < 50% makes sense. But in 2:30, it says EF > 50% is diastolic, I get confused. Maybe > 70% is diastolic dysfunction? What’s the real normal range of EF? Thanks, sorry if I asked a stupid question.

Diastolic heart failure has a normal EF. Only cardiac output is reduced in diastolic HF, not EF.
