As a Server COPD sufferer aged 62 who has never smoked in his life, it's helpful to understand how my body is failing. I can only conclude i must have inhaled other destructive lung function pollutants throughout my working life in the aircraft manufacture industry.

On CBC you can see: hypoxia-> EPO->secondary polycythemia 2) Emphysema-> IL-6, TNF alpha, INF gamma -> increased hepcidin -> decreased liver ferroportin-> decreased transferrin -> decreased Fe to red bone marrow -> decreased erythropoesis -> Anemia of Chronic disease So you can see on CBC either low( Anemia of chronic disease) or high(secondary polycythemia)

As far as I'm aware, swing between polycythaemia and anaemia in COPD depends on the balance between the hypoxaemia and the inflammatory process. From what I've read, nowadays, the swing is more often towards the anaemia as the hypoxaemia is usually corrected in the clinical setting.

I think the one that should not be used alone are the long ating beta-agonists (LABA) rather than the inhaled corticosteroids (ICS), because it is usually the ICS which are used as monotherapy especially in mild persistent cases.

Amazing videos man. One question, I'm currently a second year medical student and at the moment studying respiration. In obstructive diseases, we find that spirometery has different results to the graphs you showed here. Due to the obstructive nature, FEV1 is reduced while FVC remains the same, in your graphs it displays an extended period of time to reach FVC (which is correct) however it shows that the FEV1 remains the same, which it doesn't. What I'm trying to say is that the amount of air exhaled in 1 second should be reduced however your graph doesn't show this. Love your videos man, really helpful :)

Thank you. But what about the aspect of COPD where patient's "drive to breathe" changes from O2 to CO2...in other words for some COPD patients if you increase the O2 too high then you remove their drive to breathe because CO2 is now the main drive? Hope this makes sense.

Hi, I just wanted to clarify. You mentioned ICS are not used alone but used with LABAs. Shouldn't it be that LABA should not be used alone but always with an ICS. Thank you for your videos. They are really helpful =) You're really talented.
@krissy4265
@krissy4265 7 лет назад
Hi, RT student here. LABAs and ICSs can both be taken separately or together. There are several LABAs, ICSs and Combo drugs of the two!
@dhverma4769
@dhverma4769 7 лет назад
yes, ICS can be used alone. using LABA as a mono therapy can have severe adverse effects, thus there are also drugs like seroflo, which is combination of both LABA & ICS. and if no benefits are seen after adding LABA, it should be stopped.

I love your explanations, can you possibly explain at a cellular level how do vitamin c and e and elderberry and zinc help build immunity in the cellular level and how can it work against viruses we encounter? Also the mechanism of green tea and Irish sea moss in the lungs. Thanks just a hopeful request.

I've been watching your series of videos - very helpful in understanding. Question: I have Pulmonary Hypertension which is being managed with my Pul Doc. Due to first PFT, (which I think I just didn't understand fully what I was supposed to do) it was claimed I had COPD due to the DLCO being low at 70%. 2nd PFT a year later showed much improvement with DLCO at 90. FVC, FEV1, FEV1/FVD, TLC - all within normal range. Have never been short on 02 even when exercising. Just SOB. Wondering why now they still want to call my SOB and PH Class III due to COPD rather than PAH, or Idiopathing PH.

My group has to do a learning resource for lysosomal storage diseases and I suggested using handwritten tutorials as the way to go. Just wondering is the paper you are writing on stuck to a wall or is it placed on a table? And do you surround the paper with lamps that illuminate the sheet? And how do you fix the camera in such a way that allows the whole page to show up in the video without shadows? I would really be grateful if you replied. Thank You and have a great day :)
