We’re thrilled to have White House Covid Coordinator Dr. Ashish Jha return to UCSF Medical Grand Rounds. We’ll discuss the current state of Covid, as well as long-range strategies being planned or considered in Washington, D.C. Dr. Jha is currently on leave from his position as dean of the Brown University School of Public Health. He is one of the nation’s preeminent health policy researchers and has been a trusted source of information and insight throughout the pandemic. He was also a medical resident and chief medical resident at UCSF. He’ll be interviewed by DOM Chair Bob Wachter.

Ashish K. Jha, MD is the Biden Administration’s White House Covid-19 Response Coordinator. Before his appointment to the White House, Dr. Jha was dean of the Brown School of Public Health and a professor of Health, Policy, and Practice. Prior to joining Brown, Jha was the faculty director of the Harvard Global Health Institute from 2014 until 2020, and served as the dean for Global Strategy at the Harvard T.H. Chan School of Public Health from 2018 to 2020. Dr. Jha is a practicing physician and an elected member of the National Academy of Medicine.

I have heard of people testing positive a day or two after they tested negative. For that reason I would not trust a negative test prior to an indoor crowded event and would most definitely wear my N95 mask. We have worried about President Biden not being protected enough by his team because so many around him do not wear masks. I am a retired operating room nurse (from UCSF :-) ) and while working I wore masks for 95% of my day, however they were not N95 masks which are admittedly more uncomfortable. I DO believe it is well worth it to wear my N95 mask when I bowl in my league every Tuesday even if I am one of 3 of us doing so since I am now 66 years old. I thankfully have not been sick to this point. Thank you for your interesting discussion.

Really? You don't know how to interpret that as an RN? You know that the early negative test, followed by a positive a few days later, is most likely because early viral load was low, and transmissibility was very likely low as well. Not zero, no perfect transmissibility gauge yet from anything.

@Cathy Thanks for your insight. I am not an infectious disease specialist so I do like to hear what the latest information is. Low load is not zero load so that is why I feel more comfortable wearing an N95 mask. I don’t know the magic recipe for transmissibility but the least amount of exposure is my comfort zone. As a retired RN I realize there is so much we don’t know about the virus. Stay well.

Annals of Internal Medicine had an evaluation of antibody tests compared to PCR, over several days. Their primary concern was telling people whether to quarantine. In the first day of infection, the sensitivity of the antibody test was zero (because the immune system hasn't had time to respond). In day 2 and 3, the sensitivity was 10-20%. The peak sensitivity was around day 3 or 4, of 80%. The sensitivity declined after that. So the antibody test has a high overall rate of failing to detect infection. The argument for it is, it tends to detect infections with higher viral loads, when they are most communicable. It tends to miss infections with lower viral loads, which are asymptomatic and less communicable anyway. (Sorry I couldn't find the citation, but you'd want the latest data anyway.) But I could see utility for a PCR test. If I'm 80 years old (when the case fatality rate matches Russian Roulette), I'd like to know when I'm infected on day 2, so I can start Paxlovid then, rather than day 4 or 5, when my viral load has gone up a few doublings.

My paralyzed mother (stroke), aged 87 with dementia, had C19 twice, including the Delta strain. She’s fine. Low grade fever the first time. That was before vaxxxx. The 2nd time was after 3 of those useless shots and with 100% of her caretakers and visitors wearing masks. Lol.

The 450-550 Americans/day that are dying are overwhelmingly over 75. Fairly good chance that they died "with" COVID pushing them into the grave, rather than just "from" COVID. In my area, the hospitals that serve a greater percentage of the poor have more deaths from COVID.

Constant rebound back to positive test for covid after testing negative when using Paxlovid raises a lot of questions and concerns. I cant understand why a doctor would promote this drug without thoroughly addressing the multiple times people say they were regressing back to a covid positive state after taking Paxlovid. What is actually happening? Why are people regressing back to covid positive state with paxlovid?

@@Cathy-xi8cb nothing about what he said from min 39 is a clear explanation of rebound. Nothing he said is based on thoroughly reviewed varified trustworthy information. He did not address why rebound to covid positive happens and he did not address why it happens with paxlovid specifically. He gave some flimsy non trustworthy response about people tweeting and he talked about some unknown small study which also did not address the reason for rebound to positive for covid. His response was like the typical dismisive and evasive politician.

@@soccersprint Rebound is seen in many viral illnesses, without any vaccination for that virus, and without any antiviral treatment (we have antivirals for flu, for example). It doesn't happen only with Paxlovid. That is untrue and isn't news to any medical provider. Plenty of information on medical management of symptom rebound.

Because Paxlovid reduces hospitalization by 50%, and among those who are hospitalized, it reduces death by 95%, according to a recent study in The Lancet.

re drug interactions: friend was sick, asked doctor about paxlovid. doctor looked at drug interaction list and said no because he was on a blood pressure medication. also, many people with risk factors take statins. some research online says you can stop the other drug while taking paxlovid. the pfizer site says ask your doctor. but the doctors don't know anything and are afraid to prescribe. what is the real info and where can i find it? (I just looked at IDSA and didn't find a clear answer...)

Victoria Australia Department of Health claimed that Molnupiravir and Paxlovid given in one day are far more effective than 5 days . Molnupiravir is predeployed in Nursing Homes to ensure the most rapid ascess .

My 93 year old Mother is in a Nursing Home i never allowed her to get any Covid Jabs and she was the only one on her floor that never got Covid. She has been fine ever since i will never allow her to get these poisonous jabs.

Well, um, there's nothing to stop you from wearing a mask if you think that will help you. I was personally rather pleased that the issue of masks didn't come up, because people just aren't into that and public health authorities immediately lose their audience once the M word crosses their lips.

Haha!! Clearly you live in fear. WTH wear this mask?? I taught infection control at a college for 33 years and I now realize the huge mistakes they made forcing masks, especially on kids. WHAT about VITAMIN D3???? Or change eating habits- no seed oils or sugar in every product??? The advertising of all the junk that is mislabeled as food is hideous. I love it when "masked people" go to the store and buy a cartload of junk, hahaha!! None of the agencies want us healthy; they want to have us eat horrible "food", get obese, go to their "healthcare" and get the jaaaabs. Reprehensible but they are getting rich off us so why would they stop???????

So my wife says, he looks different in this video. I said who? She said Jay. I said Jay who? She said, you know, Dr, Bhattacharya. I said, that's because it's not him! She has cataract surgery Monday morning and not an instant too soon. ;) Not long ago she told me there was a dog lying on the pavement in one video. I told her, no, it's part of the pavement! It's all pavement! No dog. I hope these extra non Medicare covered lenses she wants are worth the dough I could get half a fancy new molar for one of those. I could go on, but you get the idea.
