I would probably just make that on a Google form, Google doc, or a Google presentation. Have them make a copy of it, and then fill in the lines. And then they could screenshot it and send it back to you. The BETTER thing I would do is use the fill in the blank question type on a quiz in Schoology. You can set specific answers or many different correct answers for the blanks. It will then grade it for you and put it in as a score if you have your gradebook set up.
