Cricopharyngeal dysfunction is caused by failure of relaxation of the upper esophageal sphincter-cricopharyngeus muscle-during eating. Typically it is solid foods that tend to lodge in the mid-neck area where this muscle is located. The disorder is described in detail within this short instructional videotape.

My symptoms began in 2007. I was hospitalized for weight loss, insomnia, heart-racing, and my body was shutting down from the distress my throat was causing. I was put on a sedative SSRI and clonozepam. I had a scope procedure and was told I had cricopharyngeal spasms and that it would go away. It is now 2022 and I continue to have the "choking" and "lump in my throat" sensation. It has never gone away. I received botox injections which helped in some way, but caused gurgling and other swallowing difficulties. I have been tested for GERD, allergies, and anxiety. I experience a lot of medical gaslighting from doctors, because they did not understand my condition. Now I have developed a rare Killian-Jamieson diverticulum from my throat disorder. It cannot be operated on since the location is difficult and dangerous. I am scheduled for an endoscopy procedure and hope that a myotomy will be an option. This disorder needs more research and recognition by other medical professionals.

I thought botox was supposed to cure the gurgling not cause it? I am so sorry you are dealing with this. I have similar problems.

My son of 30rs old has being going through this over a year now and doctors keeps telling him they don't see anything wrong even though he has 3 1/2 stone because he can't eat solids. Only blended forest very depressed also.

Thank you for a very informative video. The fifth option sounds most appealing: An “esophageal endoscopy” through the mouth to use lazer to release the attachment of the crycopharyngeal muscle from the INSIDE.

That’s a very informative video. I fall right into the cricopharyngeus dysfunction category. I have had my throat stretched numerous times. I feel like I would benefit from the surgery and if my doctor mentions it I am going to do it.

I am not able to swallow solid food, but liquid goes down without a problem. Had a esophageal manometry and the GI said I have cricopharyngeal bar. So when to see an ENT and he send me to have barium swallowing test. He said the test show no problem with my swallowing. Doesn’t recommend anything just continue to try to eat soft food. Don’t know what to do! Still can’t eat. Have lost over 30 pounds in four months.

I'd demand a myotomy, a bar is not common to cause Dysphagia but all experts say it can, find a doctor that takes your worries serious.

I also am having a lot of issues, I use to think it was anxiety eating in public, which just makes it worse, but theres times I feel I have to put forth more effort to swallow the bigger the food is and I feel food residue in the back of my throat. I have almost constant globus sensation especially after eating and although rarely I do sometimes regurititate food at night mostly, waking up feeling i'm choking on something like gum or something then realize it cant be i'm in bed.

Hi Dear...If you heaving choking sensation because of anxiety don't worry you'll get over it. I suffered from it for 2 years. I went to several doctors and they told me that I am totally fine. Then I went to therapist still nothing happened. For this all you need to self confidence. And when you get panic attacks while eating just let it be don't react and do your usual work. Keep yourself busy set a goal and have a trust in yourself. That's how your body will understand that swallowing is not a threat. Look at human body anatomy that's how you'll realise that you'll not choke you can swallow. Thru recovery you might have experience ups and downs don't worry sometimes our body don't feel confident enough. In my journey, go with your friends outside have fun go restaurants keep yourself busy and these things really help for recover.

Thank you so much for posting this video. I have had difficulty swallowing for a long time. I asked my specialist who is helping me with MSIDS about this. He suggested I see an ENT which is exactly what I will be doing very soon. :) This is very helpful info. I will watch other videos too.

You are most welcome, and all the best in your search. If cricopharyngeal myotomy is the solution, be sure you find a surgeon who does this procedure regularly, and who can offer you an endoscopic option.

What a great video! I was just diagnosed with Cricopharyngeal achalasia via a Barium Swallow. I have a scheduled endoscopy in three weeks. I would prefer they do a therapeutic procedure rather than a diagnostic one. Specifically, I'd like the laser myotomy since I also heard dilation and botox treatments are temporary. One test I refuse to have is an esophageal manometry due to a tremendous gag reflex and severe panic attacks. I would just not be a candidate for that test. Otherwise, I am all for surgery. Definitely. This video is informational. Anyone with food lodgement, and especially with weight loss, must be evaluated by a personal physician. Sometimes this is via esophagoscopy (looking down the esophagus with a scope); otherwise, using an xray technique to see how barium (the contrast material) flows through the esophagus. Please don't use this as medical advice but see your physician!

Key features of CPS: location at cricoid cartilage, sensation is one of pressure or "golf ball stuck in the throat," but eating makes it feel better (at least you finish). If you match these symptoms and the video, take all of this information to your personal physician for confirmation and problem-solving.

On an "informational" basis, a person with a firm diagnosis of cricopharyngeus dysfunction (not verified by me here) is best managed by cricopharyngeus myotomy. I much prefer endoscopic (through the mouth) myotomy with a laser division of the muscle. Usually one night in hospital suffices. Botox can be used but is not a permanent treatment. Dilation does not typically work, though some say the first or second time it helps briefly...

What a truly unbelievably well presented video! I apologise for the essay below. I have had this problem for approximately 25 years now. I’m 38, and I’ve had a few bad experiences where I have either been on the verge of choking or I’ve been admitted to A&E to get an injection of a muscle relaxant. When I was in my teens I had the problem but it was manageable. In my late twenties it became worse and to date it’s now an issue with virtually every meal. It’s not every meal thankfully but I’d say 70% of all meals I have an issue of some sort with swallowing. Some months it can be worse. Oddly when I have a serious episode, as in the sphincter won’t relax at all, and I’m throwing up to get the food out, to swallow a small amount of water to see if it will relax and I do this until it does relax, oddly I can get a good couple of months with the symptoms being minor. Red meat, chicken, potato (even mash), bread, some fibrous greens, rice (rice is really bad) and peanut butter are all things that like to cause a problem. I’ve had numerous endoscopies and been diagnosed with “Eosinophilic esophagitis” but I could have a swallow of mash potato for example and it’ll go down, then another and it’ll get stuck, then another and it’ll go down. I would have thought it would be consistent/regular. I’ve been so see both NHS (UK) doctors as well as private consultants. No one has ever mentioned what is in this video and yet this video solely resonates with my condition. I’ve since given up with doctors because I just feel it’s a waste of time and if anything I’m just a Guinea pig purchasing pills which never work. I don’t know if it’s relevant but I have quite a strong tongue, I can make shapes with it. I also have symmetrical issues with controlling my face. For example I can blow air into my left cheek, raise my left eyebrow, curl my left upper lip, and smile perfectly on my left, but on my right I have almost a paralysis for these voluntary actions. Perhaps I will consult my doctor again but I feel like it’s a difficult decision for me and just looking for a bit of advice really. I know you’re not able to diagnose but if it were you how would you go about the following? So if I go to my NHS doctor there is a chance he will never have heard of the symptom you have described. I’ll either get fobbed off with their diagnosis, or I’ll have to wait 1-3 years to see a specialist in this field. I’ll probably have to have yet another endoscopy (which I suffer from a lot, honestly I think I’d rather be waterboarded!) And it may still not be a diagnosis and yet more pill trialling. Or I can go private but last time I went private my consultant wanted me to address issues that I didn’t have issues with. For example he wanted me to take this medicine called “fluticasone furoate” which as he puts it is a nasal spray to thin the cartilage in my nose to help be breathe better and to alleviate snoring. I did tell him I didn’t snore at night and every time I interjected he told me the same “when you cough, you cough at 100mph and that causes inflammation of the throat, that and the septum is inflamed, and I should put some bricks under my bed to elevate me when I sleep” so in the end after being told that numerous times I gave up and left. If I go private it seems that even though I’m paying, I’ve got to jump through various hoops instead of tackling the issue. My only issue is I can’t swallow properly. I would love to eat steak again, or even just eat without wondering if I will have to go to A&E. I also now have to take water with me to A&E because the nurses don’t believe I have a spasm, so I have to drink, knowing full well it won’t pass and I run the risk of choking. So I don’t know what to do? How do I get the ball rolling on this? I don’t have any faith in the doctors any more and yet it’s them I need for a diagnosis.

I'm so sorry to read about all you've been through. If antegrade cricopharyngeus dysfunction is your issue, it will be diagnosed very simply with a videofluoroscopic swallow study. The technician would need to let you swallow solids more than liquid barium (usually barium paste on a cookie or cracker). A cricopharyngeal bar, (the visual finding of antegrade CPD) would provide an immediate referral for either botox injection (temporary benefit of a few months) or, more definitively, an endoscopic laser cricopharyngeal myotomy. Just had an endoscopy with balloon dilation on Wednesday. Symptoms were the same as yours. The procedure was a piece of cake. They gave me some really good twilight drugs. I can now eat anything once again after suffering for over 3+ year. Go have it done. I did not need the esophageal manometry test (due to a tremendous gag reflex and severe panic attacks). They gave me a solution to gargle with - but it wasn't necessary as I was out of it. Don't torture yourself as I did - have it done. Yes, @Beggin for Mercy - my symptoms were the same as yours - food getting stuck then having to regurgitate it to clear it out. I was getting tired of pureed soups and foods which is all I ate for three YEARS.

So happy for you! Just a comment if I may: If the problem is a web or stricture a dilation can do wonders just as you described. And that single dilation can be definitive and permanent. If on the other hand the problem is cricopharyngeus dysfunction in the forward (swallowing rather than burping) direction, then a dilation can be wonderfully helpful too, but is not typically a "permanent" solution. What I mean by that is that some find that the first dilation helps for 6M (for example); the second for 3 weeks; the third for a few days; and so forth. That is, IF the problem is actual muscle dysfunction, then eventually myotomy seems to become necessary. I hope your problem is one of those stricture circumstances, in which case your problem may be gone forever! All the best to you!

Dear Dr. Laryngopedia (I am sorry sir, I do not know your name). I want to thank you for your series of videos on swallowing disorders. Since you were kind enough to reply to my recent procedure which corrected the swallowing issue I had for three YEARS, I felt necessary to send you my procedure report. I believe it will also help others who are FEARFUL and may avoid eating and/or having a procedure performed since it sounds frightening. I had my procedure done on September 18, 2019. You presented your information in a very calming and informative manner. Without your videos, I probably would have lived with the swallowing disorder. I owe you, sir! Others reading this, get help. Don't avoid having it looked into as I did. There is absolutely NO NEED to suffer. There are NO gaging issues. You will know nothing under the "twilight" anesthesia. I went to sleep having a swallowing disorder. I woke up being free to once again enjoy food as I did - and as others do. Please, do yourself a favor - make that appointment as soon as you can. Once again, you will NOT have a gag issue and you will not remember the procedure at all. Now, here is my Gastroenterologists report on my procedure. Please print it out and look at it and realize, your suffering is over. (If Dr. - you feel this post does not belong here, feel free to delete it) Christmas, as of this writing, is two days away. I look forward to Prime rib, mashed potatoes & gravy, vegetables, "bread", cranberries, bread pudding, and many desserts. ...and the best part, it all goes down smoothly! Thank you a billion times Dr. Laryngopedia - you're the best! # # # PROCEDURE: EGD with esophageal balloon dilatation and antral biopsy to rule out Helicobacter pylori. INDICATIONS: A 65-year-old gentleman complaining of esophageal dysphagia. He has had dysphagia to solids for a very long time and had food bolus impaction when he was 10 years old. He had an upper GI series that suggested cricopharyngeal achalasia with tertiary esophageal contractions and a benign-appearing stricture suspected at gastroesophageal junction with a small sliding hiatal hernia. PREMEDICATION: Monitored anesthesia care. OPERATION: The risks, benefits, and alternatives of the procedure were reviewed with the patient. Risk factors mentioned included but were not limited to cardiopulmonary complications from anesthesia, aspiration, hemorrhage, perforation and a pathology misread of 5% to 10%. Informed consent obtained prior to starting the procedure. Olympus video gastroscope was introduced in the oropharynx under direct vision advanced over esophageal sphincter into proximal esophagus. The scope was maneuvered into distal esophagus. There was no increase in resistance to passage of scope through upper esophageal sphincter. I did not appreciate a Zenker's diverticulum or achalasia of upper esophageal sphincter or increase in cricopharyngeus muscle tone. Furthermore, there was no increase in esophageal rings or furrows. There was no stasis change and no evidence of traction diverticulum. The squamocolumnar junction was well-demarcated at 36 cm and there was a tight stricture at this level suggestive of a tight Schatzki's ring measuring no more than 9 mm in diameter. Efforts to transgress the Schatzki's ring were not successful initially and scope caused circumferential trauma and scant oozing. However, with sustained pressure, I was able to get through Schatzki's ring into a very small hiatal hernia measuring no more than 1 cm and then into gastric lumen. Gastric mucosa suggested patchy erythema in the peripyloric area with some scarring and there was functional pyloric stenosis, however, pylorus could be easily intubated with the adult gastroscope. Descending duodenal mucosa was unremarkable and the scope was advanced to second portion of duodenum. No evidence of duodenitis or arteriovenous malformations or inflammation of duodenum. The scope was then withdrawn in the antral portion and antral biopsies were obtained for H. pylori. Retroflexion suggested mild bleeding from the passage of scope through a tight Schatzki's ring. No additional findings were noted. The scope was then withdrawn in the distal esophagus and using controlled radial expansion through the scope balloon Schatzki's ring was engaged with a 12 mm balloon and dilated sequentially to 13.5 mm and then to 15 mm with deflation set at every stage inspect the effectiveness of dilatation. At 15 mma mucosal tear was seen at 7 o'clock position. There was heme effect. A smaller tear was also noted at 2 o'clock position. Thereafter, stomach was decompressed, scope withdrawn and procedure terminated. Patient tolerated the procedure well without difficulties or complications. IMPRESSIONS: 1. No evidence of increase in upper esophageal sphincter tone, Zenker's diverticulum, traction diverticulum, stasis change or increase in esophageal rings or furrows. 2. Tight stricture noted at 36 cm coinciding with squamocolumnar junction suggestive of a tight Schatzki's ring that was dilated first with the passage of the adult gastroscope and then with controlled radial expansion through the scope balloon to 12 mm, 13.5 mm and 15 mm with mucosal tears and heme effect. 3. Barely 1 cm hiatal hernia. 4. Mild antral gastritis in the vicinity of pylorus with scarring and erythema, status post antral biopsy for Helicobacter pylori. 5. Some narrowing of the pyloric opening noted. However, the adult gastroscope was advanced without difficulty. 6. Normal duodenal mucosa. RECOMMENDATIONS: 1. Await pathology. (UPDATE: Pathology report came back as normal) 2. Cut food fine and chew well to minimize chance of dysphagia. 3.Anti-reflux measures. 4.Repeat endoscopy as necessary based on patient's symptoms

Happens every single time I eat steak, and sometimes when I try to swallow larger pills. I can't swallow, a lump forms in my throat that keeps moving up and down, and my mouth begins to fill with saliva. Eventually I end up with my face in the toilet trying to puke up what's in my throat. Not my stomach, just the throat. I've never heard of anything like this before, and didn't think I would ever find out what was happening. It got to the point now where I am afraid to eat out in a public place or with other people. So relieved to have found articles and this video on it. I'm hoping I won't have to give up steak, it's my favorite!

I can relate very much. I have eaten almost no meat, especially steak in months because I just can't swallow it. I like it, but I don't want to choke so I just don't eat it. I have been able to get down mashed potatoes very carefully, applesauce, lots of smoothies, almond milk... Spicy food is easier. I don't normally eat spicy foods, but I've been eating spicy Doritos because I can get them down. My mouth waters more with them, so they slide down better.

Im having the same situation, actually I’ve been eaten very soft meals ( baby like meals) almost for a month. Sometimes I had a bite and gave up with meals, is very sad. It happens time to time, but never is “ present” when the Drs appointment finally come. I know is not just stress, the tissue in my throat gets little “ bubbles” and gets very very dry, and I can see the saliva pretty dense getting stuck in my throat.

@@sammylpt9076 25 years? wow. I never actually did anything to resolve it. I just ate steak a lot less and stopped taking the large pills. Over time it just kind of improved on it's own. I haven't had any episodes quiet like that for a while now I'm glad to say.

Achalasia more commonly refers to difficutly lower in the esophagus. We are talking about non-relaxation of the upper esophageal sphincter (cricopharyngeus). Myotomy is a major treatment, either endoscopically (through mouth) with laser, or via small low neck incision.

Food getting stuck/not going down can also be a problem with the esphogus not functioning properly which causes the lower sphincter muscle (LES) not to relax and stays closed not allowing food to go into the stomach which is Achalasia. There are more and more people being diagnosed with Achalasia today.

I ate fine as a kid no conscious issues at all perfectly normal. Problems started at age 11. Either following trauma from choking on a fish bone or I am beginning to think that something was triggered or damaged during an old fashioned 1970s stomach pumping in hospitalat age 6 after taking a load of my mothers pills.......swallowing seems easier with spicy foods. Key problem is the occasional feeling of the swallow just stopping mid flow resulting in a split second panic due to a feeling that I might be about to choke but it quickly passes. All tests negaitive so no physical issues. Could my brain now be holding back at certain stages of the swallow process.....very hard to explain. Now into my 50s and its no different to when I was young. No worse no better. Liquids fine. Solids are the issue.....preferring to chew excessively and only swallowing small boluses. Yes, some people with CPD develop a pouch called a Zenker’s diverticulum. The considerable forces of swallowing arrive at the unyielding CPM and, over time, a weakness in the wall of the throat allows this bulge. Across several years’ time, the pouch becomes large enough to retain swallowed material. The undigested food in the pouch may burp back up into the throat on occasion. Not sure whats going on with my throat, is it CPD? about 3-4 years ago started having very seldom issues with swallowing food, especially in public restaraunt settings(more stress) Started out certain meats and breads. I ignored it for years as it was very rare and primarily only in public. Well I had an EGD done when I started feeling it even more in restaraunts and now most solid foods that are not liquid or softer. I’m on Buspar for anxiety and omeprazole because the EGD showed very mild gerd. Current symptoms - 1. Sometimes I feel like I cannot swallow bigger bites without a lot of effort or even smaller ones if not liquid, also things like biscuits and certain bread or crumbling foods I feel tiny pieces stuck at the back of my throat like it cannot clear without coughing it up or swallowing more. Sometimes its like my swallowing reflex does not work which could be stress + tight muscle soreness maybe? 2. For almost a week and a 1.2 straight it felt like food stuck under my adams apple and I kept coughing up clearish mucus. This has recently calmed down a tiny bit but certain foods still cause the sensation off and on it also feels like pills stuck in my throat. 3. I have sensations of waking up coughing or choking like something is stuck in my adams apple almost regurtiating or that I have to concentrate to get a big swallow at night to releive pressure turning on my side helps sometimes. Sometimes it presents as nightmares im choking on gum or have a quarter stuck near my adams apple and upper throat but after I calm down and realize it either clears or I swallow what I regurgitated. 4. I have pretty much no issue with liquids, but sometimes thicker liquids like applesauce or mashed potatoes I do. I’ve been on omeprazole for almost 5 weeks and buspars for roughly 3 1/2. I have little to no heartburn if any and no stomch or chest discomfort. EGD showed no ulcers, H polori or any of that and no EOE. My questions 1. Can this be CPD and its slowly progressing? 2. Does this sound like how it reacts symptom wise? 3. Can a Barium swallow detect these? PLEASE any help I can get would be much appreciated. I’m spiraling into madness here.

Another thing I get time to time, sticky foods or dry or bread like chik fi la biscuit for instance once I swallow I feel tiny residue in the back of my throat a lot I have to either reswallow or cough out.

Have you made any progress with this? I too have the same symptoms, I'm 38 now and had this from the age of around 13, and it's progressively got worse. I think now, touch wood, It's at a standstill as in for the past few years it doesn't seem much different. I have to be careful regardless of eating food. I do eat meat, but I have given up steak and pulled pork. I eat rice, but I eat small portions of it and mix it with vegetables to try to hide my throat from it. It kinda works. Bread is a problem and while I do eat it, I try to be seldom with it. Peanut butter and mashed potato trigger the issue as well. I tend to limit my diet now and get my calories from things like olives, avocados, creams, protein shakes, etc. Luckily for me, touch wood, I have not had issues with weight loss in all of this time, as one thing I can eat crisps all day and it will never have an issue. Not sure why, but yeah, crisps all day long. No issues. Do you have any facial paralysis I was wondering? I only say it because of my face, not sure if this is the case for most people but I can independently move muscles in the left side of my face but not my right. For example, on my left, I can raise an eyebrow, curl my lip, blow air into my cheek, and smile completely. In my right, I can't do the above. It's never caused a problem but I notice I slur my words. I was wondering if facial muscles and imbalances in the face may affect swallowing. But yeah I'd be keen to hear what resolve you have had over this?

@gallantsword4u Cricopharyngeus dysfunction (non-relaxation) creates a projection into the lumen and this narrows the barium stream. That's because the muscle won't "let go" so you see this asymmetrical hourglass contour of the esophagus exactly where the muscle is located. If this dx is verified, then primary option is cricopharyngeus myotomy. There are others, so check with a physician who does this kind of problem a lot. Be sure to take the barium images (not just the report) with you.

Thank you for this informative video! I am a 24-year old female and developed a swallowing problem in February. It started when I was laughing, which turned into a sharp cough--when the cough was over, I had a swelling/lump feeling in my throat. Since then, I feel that food gets "stuck" in my throat and I am on a soft diet. I have lost much weight, my life is on hold, and no doctor can diagnose the issue. Can a coughing fit trigger a cricopharyngeal problem, lasting months?

This sounds like my exact symptoms. But would this condition cause any irritation in the throat? I now have some pain and irritation but for months it was only food feeling like it was getting stuck going down, especially to one side. Definitely a gradual problem. Is the barium swallow test the same as the x-ray you were speaking about? I had one done and it only shows slight narrowing. Now they want me to do a ct. My ENT thinks it might just be tension. I'm not sure if Cricopharyngeal Dysfunction is what he was referring to. Everyone seems a bit stumped. I can no longer eat at all but liquids are ok.

I have had some difficulty swallowing for about 9 months now. It just came out of no where, I use to be able to eat and swallow very well. I have had some acid reflux in the past but no major until 9 months ago. It’s a very scary feeling. Sometimes I don’t want to eat because of the feeling of choking. It feels like a muscle spasm while eating. It comes and goes. So I went to a gastroenterologist and did a barium swallow test and it showed I had very bad acid reflux. So I’m on some pills now to help with that but it seems like it’s not working out that great. Still have problems but not as bad but it’s still there :(

I’m the exact same as you! Probably around 9 months myself, It’s so discomforting! I also have been experiencing acid reflux and I too got put onto some pills that also didn’t help. They tried 2 different types of medication but they both had no real effect that seemed to help. I can relate 💯percent

I am a 64 yr. old male. Approx. 6 months ago, quite suddenly, I started having swallowing problems. This happened shortly after I had a severe cold. My symptoms were: 1. There always appears to be saliva at the back of my throat that I can never swallow. 2. Food and pills get stuck at the bottom of my throat. Sometimes the next bite of food, or some water would chase this food down. 3. I would sometimes regurgitate completely undigested food or a pill, several hours after I ate or took the pill. 4. I occasionally feel a large lump in my throat. 5. Much throat clearing, and sometimes a gurgling sound in my throat. 6. When drinking a glass of water, there is still some water left in my throat after I have complete the swallow. A doctor suspected Zencker's diverticulum, but I had a barium swallow test that showed there was no Zencker's present. Questions: Does this indicate CP dysfunction? Do you suspect that my doctor is not familiar with CP dysfunction?

I have to say this video has got me down,im a 13 year old kid and i had some trouble swallowing certain foods like bread and whatnot,but anyways i had to cope with not feeling comfortable swallowing but few days had passed and i had almost recovered with my swallowing problems which i could now swallow bread and meat.im glad that my swallowing problems are not as bad for now.However i had check on yahoo answers and what i had discovered is sleeping on your left side helps with swallowing problems but im not certain that this actually works and im not sure it had worked for me or it was just my body that recovered itself but anyways im done expressing myself.

Please one question!!! Is an endoscopically swallowing test (a tube down the nose) as good as X-ray swallowing test to verify CPD? I have done a gastroscopi and an endoscopically swallowing test. Both were fine. I have a lump in my throat and I can feel that it takes several swallowing to swallow the food down and until it feels clear in my throat. Sometimes, very rarely, I have had small pieces of food lodged in my throat there for an hour and have then got down. I don't know if I have CPS or CPD...

Mine was caused after radiation treatments. It's been about 2 years now. I only eat mashed potatoes and well well cooked broccoli or coliflower mashed into a paste. This i have every day now for over 1 1/2 years. Went from 165 to 105 lbs. Do you by chance think the last solution you give here might work for me ? I just had the x ray swallow test done. She said even water is getting somewhat stuck. Although at point I don't feel it. Where are you located ? As maybe your institute could help ? I live in Michigan and any suggested surgeon in my area would be extreamly appreciated. She wants me to go to her clinic for throat exercises. Think that could help ? Great video !

Very interesting video. For many years I've had problems swallowing food (solids), especially meat, potatoes, rice, bread, etc. Things have grown worse in recent years. I chew my food meticulously, but still it's usually not enough. When food gets stuck, even liquids can't come down. Or if they do, they come back up after a few seconds. I also have acid reflux for the duration of the "choke". Once it happened that this lasted for six hours and was extremely unpleasant. Everything I swallowed just came back up, even water. Laryngoscopy didn't reveal anything wrong. No physician so far had any idea what was causing the problem. Could this be UES related?

CPD means that the UES is failing to fail to relax appropriately each time you swallow. So, early in the disorder, it is occasional and mild, and might only affect a large vitamin pill or piece of steak. As the years pass, the episodes of lodgment at the mid-to-low neck become more frequent. Gradually, the person migrates diet (via food choice and/ or prolonged mastication) towards softer, more slippery, and eventually more puree-like consistencies. A simple videofluoroscopic swallow study shows a cricopharyngeus bar with or without Zenker’s diverticulum. The problem is resolved or diminished after successful cricopharyngeal myotomy. My suggestion for finding a specialist in your area is this: call the ENT department of a nearby university or medical center. I was astounded by the precision in your well presented educational view on this subject matter, thank you. Unfortunately for me living in the UK, there is not a lot of expert availability in this area of medicine. I had a barium meal swallow that showed Cricopharyngeal spasm, pouch formation and pooling also asperating. I then had a manamotary and 24hrs PH testing that showed 13-37 pressure upon swallow and something about peristalsis (none?) This has been going on 5yrs with multiple tests and one offer of botox which I refused on the basis of it maybe this or maybe that. Achalasia has been discussed. I'm very lost I must admit but I now have your video I just do not know where to take it but thank you very much. Your explication in great and understandable detail gives me hope.

Sudden onset of dysphagia can have various causes, including: stroke; larynx/pharynx surgery; severe dehydration; etc. As for CPD, most cases don't progress abruptly, but gradually--perhaps from occasional episodes of lodgement of pills or solid food to, years later, being a daily problem. Hi Thanks for the video information. I can relate to the diagnosis’s mentioned. I’ve had a stricture in my esophagus since 2006. I’ve always been treated for it with an esophageal dilation. Sometimes the dysphasia comes back after 6 months of the dilation. After watching your video I can relate it to my GI Dr. I have an endoscopy & a pill cam scheduled for July 24 . Thanks Georgi

Or a different idea: some people have a coughing problem based on a sensory disturbance--like a tickle or dripping sensation. The act of swallowing can elicit that disturbance and cause coughing. In that case, the swallowing itself is normal but excites the aberrant nerve endings to trigger the sensory disturbance. @redbutterflynine Verification of diagnosis comes first from your personal physician. If the diagnosis is verified with a videofluoroscopic swallow study, the primary option is cricopharyngeus myotomy. Find someone who deals with this problem a lot and go from there.

I underwent C-spine surgery in 2011 [C6-C7], I developed what seems to be this problem. Then in Jan 2018 a Chiropractor [who knew all about my C-spine history] wanted to do an adjustment on my neck [typically called a rotation] I told him NO!, he insisted on doing it anyways and now my existing problems are ten fold to say the least- with just a half an hour of any exertion I can barley talk without getting radical spasms and cramps that scare the hell out of me. It happens everyday and has now been one year with no relief in sight. Do you believe I have this problem described in your video,and if so what should I do- living with this is horrible-thank you

CPD is only noticeable when swallowing, e.g., a pill gets stuck in your throat. CPS, conversely, tends to cause a more continuous sensation of a lump or pressure. Relief comes primarily while eating; there are no episodes of lodgment. Benzodiazepines (Valium, Xanax) have a muscle relaxant effect and can give temporary, partial relief from CPS, but I don't recommend ongoing use of them. I can't swallow pills, i have been having trouble since i was 14 swallowing solid foods and for years i was doing fine but 2 months ago i choked on pasta while i was alone at home so now for the past 2 months i haven't ate anything solid... :/ I'm starving and I've tried eating noodles but i cant bare it... my throat just feels tight and even small pieces of food is hard.... im depressed and dealing with this anxiety is ruining my life cuz now i dont eat alone and if im going to try eating anything solid i have to have someone watching me... i use to eat alot and fast and i had no problem and now its just a huge part of my life i wish i could just get rid of i want everything to fo back to normal

+Ashkem GamZ I am doing.better now I started eating healthier and it worked, now I'm a very slow eater but at least I'm eating now, I just am very safe now I don't really eat much meat, but I also do alot of neck stretches to losen the tightness around my throat, and I had a doctor stretch my throat so it could help with my problem I have throat problems and get sick.easily, I'm probably going to get my tonsils removed their my main problem but I am doing good better now and I.hope everyone else gets better, just try and eat healthier, take small bites. :)

The dilation I had last year was a game changer for me. I had zero problems eating for the first time in 15 years. But the issue returned after about eight months. SO i went in for a second dilation but it didn't have the same results. Now my doctor wants to do it again (after 6 weeks) but I'm reluctant. He also keeps telling me to take pills that basically keep your natural stomach acids from forming. I can't see that that's a good idea. Other information I've looked at suggests natural remedies...

Based upon long experience with a large caseload: Dilation for CPD is expected to help (maybe) the first time; a little bit (maybe) the second time, and then not at all... Sometimes even the first dilation does very little... When it does help, it is usually just once or maybe twice. That is why the definitive "fix" is cricopharyngeus myotomy. Botox can be injected, but again, that is not the "permanent" fix that myotomy is...

As I have difficulty in swallowing food have had Xray and been diagnosed with prominent cricopharyngeus and mild dysmotility. My main problem is food getting stuck in my throat and causing my throat to close over and I cannot breathe - very frightening! What would the best treatment be? have been advised to have a widening of the throat, does this work, and has anyway else had this done with these symptoms.

Hi, I was doing McKenzie neck extensions where I hung my head on the back of a bed and let it hang I felt some pain inside the neck. Since then I initially had the thing stuck in my throat feeling, but now the problem is that every time I eat, I seem to get mucous coming up into the throat that can make me cough and at night I get some sort of reflux maybe into the lung not sure but have to sleep elevated. Is it possible that this is because this muscle is in spasm not closing the sphincter?

The solution for CPD, if symptoms are serious enough, is a cricopharyngeus myotomy. The muscle ring is divided so that it no longer surrounds the swallowing passage--going from an "O" to a "U." An alternative treatment is dilation, to try to stretch the muscle. This often loses its effectiveness before long. Botox has also been used for CPD, but in my opinion is not an ideal treatment. Is this condition more painful than Cricopharyngeal spasm? My "lump" began in May 2011 and escalated to severe pains in Nov/Des 2011. Did almost not sleep. I also had stabbing pain around the adams-apple in Cricopharyngeal Spasms. In April 2012 I got muscle relaxing. 0,5 mg morning/evening. The pain relived almost totally, but escalated after reducing to 0,25 two weeks later. I returned to 0,5 mg morning/evening. Does Cricoph.dysf. temporarely "go away" while using muscle-relaxing medics?

This kind of scenario is new to me, but I'm taken by your comment that food actually gets stuck in your throat. If food is sticking right at the cricoid cartilage (typically mid-neck in a woman) and has been for several weeks without progress, causing actual weight loss, you might ask your doctor about a videofluoroscopic swallowing study, to ensure there is no cricopharyngeus bar. CPD does not typically cause any sensation except when swallowing (especially solid food, pills, etc.), at which point a person is aware of lodgement at the mid-neck level. People with CPD and a Zenker’s often have trouble at night when saliva/food retained in the sac or pouch “squishes” back up to the level of the throat; they say they wake up at night and have to spit out “phlegm.” (Please consider this informational, not medical advice. What is the difference between cricopharyngeal hypertrophy and dysfunction? My ENT doctor suspected hypertrophy. I have a videofluoroscopic swallow study (VFSS) scheduled in a few months.

I suspect the words are used interchangeably though they are technically not the same. When the muscle fails to relax during swallowing, it creates an indentation in the barium stream which might be called "hypertrophy" but it is really just non-relaxation (dysfunction).

hi, can you give me info or an opinion on the following. when i looked into this subject, i came across an article that said it was possible for the esophagus to develop bulges in it, that could collect food. is this true? i ask because i regularly experience regurgitating small chunks of food but i 'know' that they arent stuff that has been down to my stomach and come back up again, but is food that has somehow lodged in my throat, but not in such a way as to disrupt my breathing.

My throat pain / lump in the throat feeling is always there, not just when I eat. It’s been months now, it really hurts. Please help me, I’m so scared. I feel it now, it started on the left side but now it’s in the front middle too & it really hurts & is SOOOO UNCOMFORTABLE!! It’s much worse at night, basically it get worse as the day goes on but I have given up all hopes at a normal life while this is happening. The feeling never goes away it is ALWAYS there, all I can do is sleep so I don’t have to think about it or feel it... scared? I am terrified!!!! 😓

Me too,I'm scared it could be neurological its painful I dont know if it's my neck or my throat that hurts,I feel a lump and tightness for 2 years and 7 months just had a videofluroscopy I also feel dizzy,I've had brain scans done to see if I had a stroke nothing I'm scared

Koolkid. Omg almost 3 years?!?! That is terrible 😕😭 but you have given me hope I might not be going to die... The doctors have booked me for a gastroscopy, I’m worried it will agrivate my throat more. I’ve doubled my anxiety meds so now at least I can function without my symptoms scaring me to death all day. It’s getting more uncomfortable to swallow solids, like I feel pieces stuck in my throat after. I mainly just eat soup & ice cream now...

@vhanix18 IF the diagnosis is cricopharyngeus dysfunction (and your doctor would have to verify), it tends to get worse very slowly, often across months or even several years.

I can't eat any semi solid food...my left throat did not working, like banana,khichdi.boiled egg,jackfruit....what's the problem I can't understand...all ent doctors gave me reflux Medicine,nerve medicine but they did not work...then a slp suggest me a gastroenterologist...he diagnose this is esophageal problem....my gastroenterologist suggest me upper GI endoscopy and easophageal manometry ,and cbc blood test...

I breath in saliva water coffee food . This goes into my lungs and almost kills me Happens at least once a week. So water or fluid is in my lungs I gasp for air and cough puke and can't breathe at all for a minute at first acute water or food inhaled into my lungs makes breathing almost impossible and takes al long time to get my breath back. Followed by a panic from no air i panic and run outside my neighbor says put your hands up this seems to help but i really do not know.. This happens to me alot at least once a ;week. Its attack is followed by coughing up mucus and a headache for hours. I'm sure this is going to be the death of me as it has happened so often and always ruins my whole day...Not being able to breath is a horrible way to die my arms and legs get pain from no oxygen and then my brain is so deprived of oxygen...I have almost not got my breath back so many times it's a miracle i'm still here... Seems like it happens when i least expect it if i think of something and distract from breathing it happens like i cant forget to breath what is happening so its a throat spasm, How do you stop this from happening i dont think ill survive many more of these attacks feels like i almost die every time... No Air

My dysphasia appears to originate farther down the esophagus above the stomach. It is stress induced. The esophagus completely closes off. Even liquids and soft foods won’t pass. Of course, when it occurs stress is heighten even further. The saliva instantly thickens in preparation for vomiting. The pain can become excruciating. Breathing is fine during this whole period since the backup is farther down. Often it will catch me off guard and can begin before I’m aware of it with several bites already backed up. A number of times I’ve been on the verge of passing out. Everyone talks about dysphasia in the throat. But I don’t see discussion of the condition lower in the esophagus right above the stomach. Not sure how to proceed. 🤷🏻‍♂️

I have these symptoms seen two ent doctor's had X-ray CT scan barium swallow test and they claim nothing they can see but I know what I feel been feeling this way since November 2020 please help

IF difficulty swalling is much greater with solids / pills, etc. than soft or liquid consistencies; IF the location of the sense of lodgment is mid-to-low neck; and if symptoms slowly increase over time, CPD sounds likely. The key test is not CT or barium swallow. It is instead a videofluoroscopic swallow study, often referred to as a modified barium swallow or even "cookie swallow."

This is a long-used term for cricopharyngeus spasm (CPS)-different from CPD, which is featured here-but I don't use it. That’s because the term often used is not just “globus,” but “globus hystericus.” It's the connotation attached to the second term that I find offensive and inaccurate. I have problems with pills getting stuck and can feel uncomforatble in my chest over many hours. I was diagnosed as having a Schatzki ring. Does this cause pills to be stuck ? I sometimes have problems with food but find if I'm hit on my back it resolves. The pills get stuck the worse

I have never heard of either the small neck incision or the lazer one. I had a different type of procedure for Achalasia.

CPD alone causes mostly solid food to stick in the throat, as the muscle will not open properly to “receive” the food. But CPD can occur in tandem with reduced ability to “organize and send” the food. In this case, a person may also tend to aspirate liquids (a "sending" problem). A videofluoroscopic swallowing study can assess both your "receiving" and "sending" abilities. (Please consider this informational, not medical advice. I had balloon dilation of this muscle today during an EGD. The muscle was tight and spasms. I was experiencing dysphagia and choking sensation, frequent coughing. I pray this worked. Updates in 3 weeks.

If antegrade cricopharyngeus dysfunction (solid food / pill lodgement) dilation often works once or twice, but myotomy tends to eventually become the definitive solution.

​@Riley Watson, I only feel about 30% relief. I'm going for a second dialation soon. I can't find a surgeon close that will do a myotomy.

I am female, 39 years old. For the past 5 years I have been struggling with my throat. I've had 4 Botillium injections done with dilatation every time and it does not last and the frequency increases. My Surgeons think I am a candidate for Myotomy but I am very scared. But I can not live with the effects of the Chrico dysfunction and I get debilitating spasms too. Is it really that simple? 1 night in hospital and I can have a normal life?

This description, describes me perfect only worse... I can't eat anything unless it's like in mashed up form, like mashed potatoes... I feel like I can swallow my food but when I go to take a drink, I often 99% of the time get choked... I constantly have to drink after every bite... I have no insurance and the cost for a scope is $2,000 which I can't afford... Any advice or help out there that I can get or do? I don't know how much more I can handle of this...

Sherry Dinardi I've had your symptoms for years. A gastroenterology prescribed 10 mg citalopram which helped somewhat. Unfortunately I don't see that doctor anymore and have run out of that medication. Hopefully I can get a prescription from my new doctor.

Sherry Dinardi The only diagnosis I got was Gerd. After long treatments with anti reflux med which did not work, he prescribed citalopram which is an antidepressant. He said most of the serotonin in the human body is in the digestive tract not the brain.

David I will definitely talk to my Dr about this... I can't afford a scope at the moment... I seen that was a med for depression and I found that odd because I'm not depressed but I need to get answers I have been dignosed with bad acid reflux and currently take meds for that, it helps just not with my swallowing... He even thought maybe my sphincter muscles wasn't working but who knows

Keep in mind that cricopharyngeus *dysfunction* is an issue of the muscle’s inability to let go and allow the passage of food/ pills, etc. There is no medication or therapy for that problem. A botox injection can be used if the muscle is not fibrotic. Definitive repair in people who are symptomatic to an unacceptable degree is via cricopharyngeus myotomy. Cricopharyngeus *spasm* is a different story altogether, and sometimes people mix the two together.

Doctor I don't understand... I got stretched twice, first time 20mm and second 24mm but food still seems to stick to my lower throat..I take small bites and chew well ..my dr doesn't think is a muscles problem so why won't these dilations help much? What do you suggest I do?

Sorry to hear of your difficulty. Everything is of course driven by a very clear diagnosis. Let’s think for a moment about one of the possible explanations that may (or may not) apply to you: If the problem is solid food lodgment that you can actually feel at the mid- to lower neck, and if a videofluoroscopic swallow study (x-ray study of swallowing function using barium) shows what is called a cricopharyngeus bar, then dilation may help transiently for some, but most people only get true relief from a cricopharyngeal myotomy procedure. Again, first verify the diagnosis with your personal doctor. Ask if the dilation is being done for a stricture or muscle problem, and exactly where along the foodway the stricture or narrowing problem is. That information may bring better understanding. Well I went to two different specialist.. One said it was CP over active muscle because he saw on the motility test..and the other said it was just a stricture from my esophagram study ..I just want relieve that is more permanent because even with dialations perforation is at risk and I don't want to get higher than 25mm I'm a small woman not sure my esophagus would be able to handle it...maybe l can send you my information seek further treatment with you for a true diagnosis.
