En escena tenemos a un chico, Nemorino, que ha sido engañado por un charlatán que se hace pasar por doctor, Dulcamara. Éste, le ha dicho que con el elixir que le ha vendido todas las mujeres caerán rendidas a sus pies por sus mágicos efectos al beberlo en abundancia. Aunque él sólo pretende el amor de su amada Adina, ella, al ver que ahora todas las mujeres lo quieren por marido lamenta mucho el haberlo rechazado cuando él le declaró su amor... se sorprende ella, el charlatán y hasta el mismo Nemorino de los resultados del fármaco.
Al verla tan triste y apenada, Dulcamara el charlatán, pretende venderle ahora a Adina el mismo medicamento, que no es otra cosa que simple vino embotellado, para que a ella también la pretendan todos los hombres de todos los estratos sociales; pero ella, menos incauta que Nemorino, le dice durante este dueto que su elixir no sirve... que la receta del amor es su mirada y el elixir son sus ojos, llevando el mensaje principal de toda la ópera y que embellece hasta el límite la hermosísima música de la ópera bufa italiana.
