Big Daddy Kane once explained why they call written lyrics “Freestyle”. It’s because it’s literally “free of style” meaning that it’s not tied to any particular topic or beat. Off the top was called off the top. At some point people merged the two different meanings into “freestyle” but that’s why a lot of clearly written raps are labeled as freestyles.

Freestyle does not exclusively mean "off the top of the dome" rapping. Freestyle literally just means "free of style". You can rhyme in any situation to any beat and any topic. Just because it was written doesn't mean it's not a freestyle. It definitely can't be something that you previously preformed somewhere else or that you recorded on an album already. Improvised off the dome rapping is so rare that you can probably count on one hand the rappers who can actually do that. I'd argue that most freestyles are written and always have been.
