How do u get the sams hammer never got it

Im missing one of the side quests to activate the assault rifle..... and havent figured out where or how to make it show

I have all of these but the assault rifle I guess I need to do couple more side quests i done 23 out of 33 soo maybe I need to do them all

If you already completed these quests can you go back to even find these weapons??

I beat the game but the mutator hotspot is still locked
