Covid 19 and Acute Respiratory Distress Syndrome (ARDS) Diagnosis and Treatment
Dr. Xuanha White, DO talks about COVID-19 Pneumonia and ARDS.
ARDS, or Acute Respiratory Distress Syndrome, refers to a type of lung damage that can be caused by illness, trauma, or as a result of certain medical procedures. It's a potentially fatal condition in which the lungs suffer a serious injury that hurts their ability to provide oxygen to the body's organs. The condition causes fluid to build up in the lungs, which reduces blood oxygen levels to dangerous levels. ARDS is a serious medical issue.
ARDS is always caused by an injury to the lungs, whether from illness or trauma. It can affect people of any age. Approximately 190,000 Americans are diagnosed with ARDS every year. Patients with ARDS need supplemental oxygen and are usually on a mechanical ventilator to breathe. While there is no cure for ARDS, it isn't always fatal. 60% to 75% of people with ARDS survive with treatment.
"We have a substantial amount of experience supporting people with acute respiratory distress syndrome," says Dr. Xuana White, a Pulmonologist and critical care specialist with Integra Health in Temecula, CA. "But it's still a serious issue. Even though we can support the body to let it recover, the lungs still have to start the recovery process. ARDS has no magic bullet treatment.”
How does COVID-19 relate to ARDS?
The virus that causes COVID-19 attaches itself to cells in the upper airways when it enters the body. Inflammation sets off an immune response, leading to symptoms like cough, sore throat, and fever.
Sometimes, the virus gets past the upper airway, moves through the lungs, and ends up in your alveoli. As a result, COVID-19 can lead to ARDS, which usually sets in 8 days after the initial symptoms. ARDS is more likely to develop in people with COVID-19 if they have advanced age, diabetes, or high blood pressure.
What makes Integra Health unique in its treatment of ARDS?
At Integra Health, Dr. Xuanha White is very experienced in the use of evidence-based strategies along with her vast experience from the large amounts of patients she has treated to provide the highest-quality care to ARDS patients.
We combine evidence-based medicine with the experience we've gained from treating a lot of people in and out of the hospital to create a customized treatment plan for each patient. To maintain and improve lung capacity, we use medications.
The medications help with residual inflammation and symptoms like coughing and sputum production that can linger after a COVID infection. Patients are also enrolled in pulmonary rehab programs tailored to COVID recovery. Patients with more advanced disease need continuous monitoring with serial spirometry or PFT's.
If you or a loved one has been affected by COVID 19 and would like to assess your lung health after the infection, contact us at Integra Health for an evaluation with one of our lung specialists. Check out IntegraHealthPC.com to learn more.

Covid hit my entire family really hard within the last 2 weeks. First my son which is 22 yrs old, he suffered chills, fever, body aches, diarrhea and cough the first 6 days. I tried my best to help him with all that we had. We used medicine, vitamins, fever reducers and home remedies and nothing seemed to help. By day 7 his body could not take it anymore that’s when the shortness of breath started he could not breath. I called the ambulance and his heart rate was extremely high (224) and his oxygen levels dropped in the 80s. He was diagnosed with Covid pneumonia. He could not walk or barely talk. They keep him for 5 days until he was able to breathe on his on without the nasal cannula. They gave him steroids for his lungs and a few other meds as well. This was the most scariest experience ever. In the midst of all this going on my anxiety was through the roof. But I kept praying and I kept trusting God. Not only did I become ill but my 22 year old daughter( my sons twin sister) was hit with Covid as well. I suffered the same symptoms as my son and I am Diebetic so it was torture. I had a mild fever for 9 days straight. My body was burning and aching, the pain was excruciating. I could not sleep, eat, drink or think straight. I stayed in the prayer position in my knees for several days and I cried out to God for my family and myself. By the 9th day my daughter and I went to the Er and they kept us over night. I was diagnosed with C19 pneumonia, and my daughter only had C19. Going to the hospital was the best thing for us because at first we were hesitant but I just put my trust in God and allowed him to guide the way. They gave us fluids through IV which was much needed due to dehydration and they also gave Azithromycin along with Dexamethasone (lung steroids) and Albuterol for the shortness of breath. After all this I started to feel so much better. It’s day 15 and I’m feeling absolutely amazing. My son is doing great and my daughter is in her healing process as well. Glory be to God🙏🏾🙏🏾🙏🏾. I’m saying all this to say don’t give up God will see you through this, just trust him through it all. Sometimes it gets rough but we serve and awesome God who will not fail us🙌🏾🙌🏾🙌🏾.

What she failed to mention is what is aspirated. If refluxed stomach acid and pepsin (the primary digestive enzyme) is aspirated into the lung the digestion of our lung tissue occurs. In addition she failed to mention the importance of nasal breathing and the critical role the nasal mucosa plays in processing the air before it enters our lungs.

It would have been nice if she showed an X-ray of what ARDS looks like on imaging. When the Covid pandemic first started I was surprised at how much Covid lungs looked like pulmonary tuberculosis. The blotchy spots all over the lungs are quite different than your typical pneumonia that tends to settle in the lower part of the lobes of a person’s lungs. It looks like the person inhaled a bunch of poly fill batting.

@saptarshibhattacharya 2 года назад
@@user-cl6tm2oz3x Yes, I was put on a BiPAP machine in the hospital and home as well 1. Hospital - Acute Hypoxemic Respiratory Failure. BiPAP with FiO2 0.5. 8 days 2. Home just after discharge - Increased work of breathing with mild Type II Respiratory failure. BIPAP with FiO2 0.21 for 20 days 3. Home after 1 month of discharge - Acute Hypoxemic Respiratory failure with Type II compliance - BiPAP with FiO2 0.3, intermittently for 1 month The mode was set to T and the respiratory rate was set to around 25. (lung-protective ventilation strategy 1). For moderate to severe hypoxic patients, spontaneous breaths should be eliminated. A minimum gap of 5 cmH20 was kept between the IPAP and EPAP. IPAP as 10cmH20 EPAP as 5cmH20 For Type I Respiratory failure (ARDS) try to increase the EPAP and not the FiO2. Tidal volume - 6 to 7ml/kg Allow 50 - 70 ml as mask leakage value. For type 1 respiratory failure low tidal volume is sometimes used and an ABG test should be obtained. Make sure pCo2 is below 48mmHg. 3 - 4 mmHg is permissible hypercapnia For Type II respiratory failure, FiO2 should be kept such that the PaO2 level of the pt should be not greater than 60mmHg and higher tidal volume is used with a slight increase in respiratory rate. If there is underlying pathology within the lung parenchyma (such as GGO, consolidation) or has a history of pneumothorax then the RR should be below 20/min. Otherwise any RR below 30/min. Pulse rate, SpO2, RR, BP, CO2 levels should be monitored for 24X7. ABG test should be obtained daily Patient should be well awake to protect their airway
