Lidocaine is 2% Lidocaine, always move the decimal point over one ( 2.0 > 20 and it is mg/ml ) 20 * 1.8ML ( always 1.8ml in a cart ) = 36mg per cart. this how they converted it to find the mg per cart most LA are 2% except Prilocaine and articaine in this case its 4% so now 40* 1.8ml in one cart= 72mg per cart Lidocaine is a gold standard though and is a common LA unless there are contradictions for any reason which I cant recall contradictions for it other than maybe to its vasoconstrictor but there is a plain version of Lidocaine too a 1:50k and 1:100k
