DOACs in the Anticoagulant Treatment Landscape 

If one pharmacist got it wrong doesn’t mean they all will. I run the anti coagulation clinic here and I find it alarming that a pharmacist would suggest holding warfarin for 7 days without initiating bridging therapy with lovenox to cover until the day of the procedure, and resuming warfarin after procedure.

Wow - some harsh words for pharmacists regarding holding DOACs before procedures. It's a shame that your pharmacists got this wrong, but we aren't all worthy of this generalization.
