This Doctor REACTS video is looking at the next episode of Chicago Med. This is starting a storyline all about a psychiatrists daughter presenting with psychosis, and how this is diagnosed,, how this should be handled by a family member who is also a doctor, and whether you can really hold people in hospital against their will. Also, how do we know whether the psychosis is caused by schizophrenia or something else?

I was a police officer. In the state where I worked, involuntarily commitment required two physicians, or a police officer and a doctor. The way the law was written, at the time, it was to allow a police officer to initiate the involuntary hold and for the physician to confirm the need 48-hour period; e.g. working in a low populated area on the late night shift, discover a mentally disturbed person, but there aren't two doctors available at the time. However, this wasn't specifically mentioned in the state statutes. Because of that, it allowed a doctor to simply get a police officer to sign it. I was frequently called by doctors to help put patients on a hold for very absurd reasons. For example, doctors didn't want to wake up another doctor that was on call, the doctor couldn't get another doctor to sign, etc. These cases were always challenging cause you have to deal with the doctor's egos, the laws, and the patient's safety. I always defaulted to the spirit of the law and refused to sign. In case anyone is curious, I never once had a single request by a doctor who had a patient that wasn't coherent, and was a threat to themselves or others.

Please tell me this law has been changed…. I knew before I even read your examples that that bullshit law would be abused CONSTANTLY. That is so f*cked up. Being in a psych ward VOLUNTARILY is traumatizing enough (you’re locked in, not allowed to go outside or anything, etc) but involuntarily… my god. I can’t imagine how much more trauma those innocent people have now.

My son was on risperidone when he was an adolescent/teenager. He has Asperger’s , and when he was younger, they would say he had a mood disorder. Now that he’s an adult, he has bipolar disorder. Risperidone was the best thing that ever happened to him at that time. The problem was after he grew out of its effectiveness. Since then it’s a nightmare.

Is he seeing a psychiatrist? There are loads of different anti-psychotics. Abilify, Rexulti, Quetiapine, just to name a few trade names from the top of my head.

Just an fyi, Asperger is not a recognized condition anymore as it's been proven to just be the form of autism the nazis considered as "useful" so it's now quite problematic nowadays

Aspergers was a term created by a nazi to determine who was able to live/die. In some places the label was removed as a diagnosis. It is just autism.

For the States, there isn't an "Advanced Decision," so an Advanced Directive would be the step that is legally binding. That being said it might be more limited with what can be on there compared to the UK.

in my State (CT) involuntary commitment guarantees you a date with the judge. If I recall correctly, there are at least two/three specialists testifying

Yeah it's really unfortunate that the disputing format is reversed in the US system. Where you said you have to prove that the person needs to stay in hospital, we definately have to prove that we do not. and it works just as well as you could imagine.... :/

This is really helpful for me right now thank you so much. My dad has schizophrenia paranoia persecution and he has chronic psychosis and I have been contemplating admitting him without his consent to the hospital because his symptoms have been getting worse. He is untreated, unmedicated and lacks insight to his own mental health.

Has Dr Elliott seen New Amsterdam S2E5 where there is child with anti-social personality disorder? She comes later again in another episode, I think in S3E6, where we see has she gotten better or not (not going to spoil it). Season 3 overall is when the writing of the main character goes do bad that it ruins the whole so, but the side characters, like Juliette are still soooo good. This is a side note if anyone reads it: If I remember the season 3, maybe that same episode or earlier, shows moment with the doctor who has ADHD that she admits that she misses Covid because that was when she shined. And I can so feel it. I can't survive normal life but things like that are manageable for me. That "small" confession she tells her nurse is so brilliant and shows the good writing of the characters - knowing the mental/neurological side so well... It's so sad that the writers drop the ball with the main character so badly, forcing it too hard onto things - pretty sure covid and many other things outside writer's power did hurt the show a lot.

I experienced hallucinations as a result of a ketamine treatments for my bipolar depression and it scared the hell out of me. If I never experience that again it will be too soon. It’s only the second time in my life when I felt “crazy”

Involuntary Commitment in the US has been in and out of the public eye, and thus the Hollywood eye, many times over the years. In recent years the conservatorships of several high profile individuals, like Britney Spears, has brought it back to the forefront and inspired a few episodes on various shows. Involuntary Commitment and Conservatorships will vary from state to state, and can vary wildly. In some states Involuntary Commitment can be done at the behest of either family or the government and it can turn into a pseudo-life sentence. In other states Involuntary Commitment can only be done by the State and requires a trial, either civil or criminal. In other States Involuntary Commitment isn't really a thing, because mental state cannot be used as a defense, but can be used as a measure of competency. You've got 50 States, multiple territories (both tribal and not),and the Federal Government to consider. Each with their own sets of laws, regulations and Constitutions. My favorite Political Science professor I ever had once put it this way. "You can't compare the U.S. to the U.K. It would be more accurate to compare the U.S. to the EU."

where I live in the US, if put on an involuntary hold, within one day (excluding weekends), you meet with a county investigator who can override the doctors and end the hold. it's just automatic that you get somebody who can advocate for you to override the doctors. the investigator checks in every few days or daily, depending on the situation. the investigator can end the hold, shorten the hold, or work with you to turn it into a voluntary hospitalization (which can, of course, turn involuntary again if you try to leave before doctors approve of it, and then again, you get a county investigator). commitment is a different thing that I'm not very informed about.

About 2 years ago, I had new symptoms added to my already really bad mental health (major depression, anxiety, chronified ED, cPTSD, AVPD). More like sliding into psychotic depression, dissiciation, word finding issues or being unable to speak at all, forced thoughts so intense that I could not sleep any more (where of course I don't know what of all these might have been causes by the sleep deprivation). I really felt like "going insane" and was utterly terrified to "lose myself". The doctor tried risperidone, but I did not react well. He exchanged it for quietiapine after just 1 or 2 days as my pulse went off the roof. Never had that before. I'm still on quietiapine (and others), and it really tends to lower my blood pressure even more than usual. I actually sleep with my mattrass a little higher at my feet since then. Without that, I can hardly stand getting up in the morning. And need more time after having taken the med. Meaning using the toilet doing the night is a wobbly-dizzy adventure. It's annyoing, but the alternative is insufferable, so it can't be helped.

I'm no longer surprised when someone in Europe comments on how hard it must be to keep track of all the different laws in all the different states; and it can be since Americans do frequently travel from state to state. But you also have to remember that most states in the U.S. are larger than most countries in Europe. Though there is an old saying among world travellers; "To an American 100 years is a long time; to a European 100 miles is a long distance".

I've never had psychosis, but I've done plenty of hallucinogens...and not all trips were smooth sailing lol It would be very frightening if that started happening without taking drugs first. It makes me almost grateful that of all my diagnoses, none of them have ever made me do that.

The food i Finnish mental hospitals is pretty good. We get the sam efood as prisons and schools and Finnish schools are famous for their lunches. Only foreigners dislike the food because it can be a little bland. My favorite food was salmon soup.

One of the complaints I have in my province, is that the appeal of the mental health certificates is limited to the patient, their lawyer, their doctors and the tribunal. Additionally, the tribunal is not held accountable for their decisions. There’s no transcripts and according to a colleague of my mother’s (a GP,) the tribunal she sat on had never canceled a set of certificates.

Its all a legal process and if a lawyer and a doctor is there then they have to legally have notes. Those are legal documents. The tribunal must keep records because its a legal process. Maybe its videoed? One way or another it will be recorded.

Writers seem to use the "gone off their meds" as a go-to when talking about characters with mental health issues 'going crazy' in an episode. Is someone with Robin's condition going to have to remain on medication for the rest of their life? How quickly after stopping their meds will a patient experience a psychotic episode? Dr. E: You rock that Burberry scarf.

Is someone with Diabetes going to have to remain on medication for the rest of their life? Is someone with MS going to have to remain on medication the rest of their lives? Is someone with Epilepsy going to have to remain on medication for the rest of their lives? Non compliance is common in schizophrenia, and seems to be a part of the illness. How long it would take for symptoms to reappear would depend on the medication/medications, and the person. It can vary from person to person.

@@sdigf3167 Epilepsy, especially one started in childhood, and 2 type of diabetes can change into type that you don't need medication anymore. But I get your point. And personally I have to eat ADHD meds probably my whole life, because it's that bad, even if generally people aren't supposed to have those forever and I would love to be able to function without. Meds to anything or for anyone depends on so many things that we never really know how long it will be.

its probably different for everyone and might be different if she had only just started but my ex would get some hallucinations if he missed a dose but not like a full on psychotic episode, not sure how long it would take for that but i think hes been off them for a few days before

@@piarateking8094 Sometimes there can be a reaction to stopping the medication itself which can mean that the person will experience a severe psychotic episode and take months to resolve. So its not always about the illness. Every person is different when it comes to what they experience. I know someone who is diagnosed with Bipolar with Psychotic symptoms (I can't recall the exact name of the disorder however this is the easiest way to explain it). What happened to him is he got frustrated when he went through a period of constantly being in bed and significant weight gain. He had no energy at all. Unfortunately he thought it was the medication when it really was part of his illness. So he had stopped taking the medication and initially started to gain excessive amounts of energy. It tooks months in his case and he got more manic over time to a point where he was spending huge amounts of money and had huge business idea's. He was experiencing psychosis but could mask his symptoms. His wife said he was starting to talk about being the next messiah at times. That became more pronounced. To us he seemed to incredible and have amazing idea's. He rarely slept and flew around the country. Then psychosis fully took over. He became terrified of the FBI wanting to capture him and monitoring him. He drove erratically around the country and we couldn't find him. He had been to the psychiatrist who thought he was okay but he was masking his symptoms. So I filed to have him assessed. The Police found him hiding under someones house terrified. He had died his hair. They took him to be assessed. He was lucid within days. The damage he did, well it took him a long time to pay back the money. He will have taken his own life if he wasn't found. This happened over months. Schizophrenia is different again.

It always blows my mind listening to docs, especially non-USA docs, talk about what mental health hospital floors are like. This is not a worst case scenario place. This is a place you go to when you actually have decent insurance and family who cares.

People make mistakes. Even doctors. He appears to genuinely wish he had done it differently. His student doctor was hoping to convince her to go to hospital voluntarily. The way it all happened didn't changed the fact that she wasn't well and needed treatment because if she didn't get it, or agree to it, she would have been likely to have harmed herself.
