This Doctor Reacts video is on the final episode of the INCREDIBLE new series on Apple TV, called The Crowded Room. Tom Holland's portrayal is based on the real case of Billy Milligan and is all about dissociative identity disorder and whether it can be a defence for a violent offence in the courts. There is so much to talk about, and on this video I focus on fusion therapy, insanity, what happens in court and the importance of endings in therapy, both for the patient and the professional.

This is one of the best shows I’ve seen in a long time. The entire cast was incredible especially Tom holland, he really gave it his all

I’ve been looking forward to this video! I really enjoyed the series but a few bits did get too real for me and I had to take a few breaks. I suppose that is testament to its good portrayal of childhood abuse/trauma though!

I just watched The Crowded Room and thought it was really good. I didn't get the poor reviews from the critics, but at least viewers gave it a high score. So I ended up stumbling across your reaction, which I enjoyed and found interesting from the view of a psychiatrist. So, I wondered what your reaction to the game 'Hellblade:Senua's Sacrifice' would be,it's about psychosis. That's if you haven't reacted to it already. Wellp, they lost me at this episode. Integration as a goal is such a frustrating approach. It feels like annihilation. And the idea that we aren't "whole" just because we have DID is awful. I am not a sum of my parts. Integration isn't just merging parts together nor can/should it be pursued as some sort of ultimate goal. Communication, not integration.

I think it's reflective of when the show is set - in the 70s or 80s when DID was still called MPD and people were really just beginning to explore potential treatment/management options

I agree, it is frustrating, but for some people, integration is desirable. I personally would like to live as a functional system but other systems want to be single. Heck, there’s disagreement in my own system regarding which one to work towards. Yes, it’s not the only option, but we still have to acknowledge it’s a healthy one

It's a reflection of the time period. It's what they did in the 70s. They're being accurate to the time period. I'm sure if Rya knew it was harmful she wouldn't have done it. Would've gone against her character

This is an absurd take and “it feels like annihilation” sounds like you both don’t understand the point of integration and like you don’t want to lose your maladaptive coping mechanism. If it’s not possible for someone, sure, it can’t be forced, as Dr Elliot said, but insisting that that’s a bad goal or that it’s “annihilating” your alters (I assume that’s what you mean) suggests you don’t understand what it actually is.

So I had, what I thought was a friend but turned into a scumbag stay with me to help him out. It was a few months back. He went to University for psychology and told me that you have a fake British accent. Also, I am apparently stupid and gullible for watching your videos because criminal pathology or forensic psychology isn't actually a career 🙄😒 I did google search you and proved you are in fact British and have psychology credentials. I also showed him various definitions and profiles of renown individuals in your field. Of course he did not believe me. Silly dumbell, he was kicked out of my apartment shortly afterward for a few reasons, but this argument was the catalyst.

I can assure you that I am British and also while I'm a psychiatrist not a psychologist, forensic psychology is a very rewarding career I am work alongside many forensic psychologists in my day job.

Alters are literally the result of dissociating and fracturing your singular self into parts. You’re not a broken person, but you’re also not multiple whole, fully developed people.
