Dr. Sean Mackey was such a pleasure to listen to. He brought the science, but he also brought a humble kindness and willingness to share his personal experience. I really enjoyed every minute of this episode.

As a Psychiatric Nurse Practitioner and Licensed Massage Therapist, I am quite naturally inclined to ask questions regarding the entire patient and not only what the patient is expressing as their primary complaint. How I wish you could dedicate another 3 hours to this topic as it has overlap with suicide, addiction, depression, homelessness, and many others. Another interesting topic is how we normally undermedicate acute pain which thereby leads to chronic pain. Great topic. Eloquently broken down in layman’s terms. I am forever a fan Dr. Huberman.

Hey Andrew, i am 36 year old (and formerly very active - much like the tennis player example mentioned here) living with degenerative disc disease, spinal stenosis and facet joint damage. Your channel has been great so far, as i have tried and failed with mindfulness approaches due to the ambiguity of their instructions. I greatly appreciate the scientific approach and information here, and would love a series on pain. Thank you for helping people

I really appreciate this one, thank you Dr Mackey and Huberman Labs. I turned 40 at the end of last year and missed playing competitive sports since I spent the last 10 years raising 3 wonderful children. I signed up for soccer and in the first game, I ruptured my Achilles tendon. I'm a healthy guy, cardio, kettlebell exercises, even cold plunges, however, this injury and it happening right as I try to do something I love was a 2 fold blow. The realization that I would need surgery and a long recovery was bad, but the emotional anchor of 'I cannot do the things I used to be able to do' is now sitting heavy on my mind. Getting old sucks. In my medical practice I see many patients that develop food sensitivity after acute gastroenteritis. Typically this is because the infection or food poisoning triggers small intestinal bacterial overgrowth or SIBO. When I do testing and treatment of SIBO, that often resolves the food sensitivities that are caused by the overgrowth in the microbiome.

Im so sorry I am going to share an experience of having toothache and was given highest legal dose opium Then found a GP - antibiotic - no pain I'm wondering what this says about standards treatment And I understand that injuries are on another level shoulder pain could be gall bladder issues

As a chronic pain patient battling with multiple syndromes, diseases and conditions, this had to be one of the most informative, compassionate podcasts I have ever watched. Hi Andrew. Great podcast. It might be interesting to maybe do a series on pain? Or maybe have several guests on the show? People who come to mind are Lorimer Moseley and Howard Schubiner. Would really love to see them on the podcast, as they have valuable insights and contributions regarding pain.

Yes these two suggestions are phenomenal! Please consider. I know I’ve been requesting Dr.Howard Schubiner to your podcast since the first IG live you did on pain. I also recommend him often to Arm Chair Experts podcast. It’s so important to have their work, experience and research in the mix of discussion as Luke was said in this interview we are in a public health crisis!

I love that food intolerance/gut issues and the body's response are discussed. Through elimination diets over the past 10 years, I have figured out that if I eat any cream, butter fat, or anything high in saturated fat, I get extreme joint pain in main joints exactly 27 hours later and erythema nodosum on my legs. Would love to join a study where saturated fat and inflammatory response is measured.

I live with more than five decades of complex chronic pain and this has been the absolute best summary of pain, approaches and treatments. Also, I value that dogma was mostly absent in this summary and even though I don’t quite agree with everything discussed (remember I have 24/7 clinical experience over 5 decades when I say this) I’m pleased that this kind of information is available to the public .

In nursing school, they taught us that pain is a signal usually to something happening in the body. It's our body's way of communicating with us. The question is, are we listening. It's better to listen to your body when it whispers before it has to scream.

As someone who also suffered from severe, chronic abdominal pain I wanted to share my story as well. I also tried the low FODMAP diet and while I saw minor improvements, the only thing that really stopped my pain was pain reprocessing therapy. For me it was more of the emotional response to the sensations, stressful life events, and fear of food was driving my pain persistence. I’m not trying to invalidate his experience that he bravely shared but I did want to share an alternative solution to the low FODMAP diet. That diet doesn’t work for everyone but that isn’t the ONLY course of action as he suggested.

I am so grateful for this episode and learnt so much! Leaving with chronic pain can be discouraging. Thank you Huberman for inviting Dr. Mackey, both of you are extremely generous human beings. I hope that his platform will be made available internationally as pain, opioids and others are sadly a universal problem now. Some gems I enjoyed ‘’pain inhibits pain’’, 100%, the study on pain and love was very interesting :) I like that there were many concrete advice, the pacing was one of them because when you feel good, you feel invincible and forget about your pain but you end up paying for it. Also his simplification of the 6 catergories of solutions is great. I would love if you would invite Dr. Howard Schubiner one day, that would be a very interesting exchange about the specific psychological side of pain. Amazing work as usual Huberman, bless you 🙏

I m stoked to be one of the 2% who know that pain inhibits pain. Or, to be precise, acute pain shifts focus from the chronic one. And I first heard about this in House MD (he hits himself to diminish the intensity of his chronic leg pain), which is, in my book, one of the best doctor series of all time.

Thanks for revisiting kratom Andrew. I hope you focus more on its benefits for people in chronic pain rather than just its potential negative side effects for those not in pain. The only side effects that I’ve noticed in the few years that I’ve been taking it is that I’m tired for a day or two afterwards if I use multiple heaping tablespoons daily for weeks on end. Plus it can affect sleeping if taken within a few hours of bedtime though not nearly as severely as caffeine.

Andrew - would you do an episode on exercise and pain? I'm thinking about things like exercises designed to relieve pain, corrective exercises, the analgesic potential of different exercise forms (?), ways of preventing injuries or how to address and recover from exercise pains/injuries if they've been sustained, etc. Great episode, love the podcast! -Ontario, Canada

Very educational interview. I live in SoCal and used to be a medical sales rep (not in narcotics) and the physicians here in Calif were so harassed by the board they stopped seeing patients and prescribing opioids altogether. THEN, a law was passed that if a patient sees their physician for pain, and the physician denies to treat the patients pain with opioids if nothing else works, the patient can complain to the medical board and the doctor can go to jail. It’s absurd. In *all* other states the worst that could happen is the doctor loses his license (which is also horrible). But in California they can go to jail! I’ve seen so many outstanding doctors shut down their practices and move to another state. I’ve seen Harvard educated neurologists be harassed into retirement. Everything about how calif handled the opioid issue backfired, chased great doctors away and calif has egg on their face.(as usual 🙄). What exactly is a doctor supposed to do when backed into a corner like that? Because the board was asked this question many times and the only reply was crickets…

Nice episode. I actually had quite a hard time making a biologist understand that pain is a subjective experience constructed in the brain. That said, strange not having mentioned psychedelics, given that baseline stress and/or anxiety increases the perception of pain.

I was hoping you might cover shock and dissociation as they relate to pain. I understand that they would only obliquely fall under how to 'reduce & manage' pain, but they definitely are routes of pain mitigation, even if out of our control. I've always wondered what was happening physiologically in those states... Thank you for all you do! I am always educated, entertained, & inspired. 💗

Pain really does inhibit pain, I was in a motorbike accident and broke my wrist, 4 fingers, 4 bones in my hand, two rips, a collar bone and a whole in my leg just about my knee I couldn’t feel any pain in any of those injury’s only the pain in my back where I’d boke L2 on my spine in two places.

Once you develop long term chronic pain issues such as fibromyalgia there is also a problem that other issues may develop that are then ignored & dismissed by medical practitioners. I intially I had an FM diagnosis. Over the years my symptoms of Hashimoto's, lupus sle, cholecystitis with an inflamed scarred gall bladder and cervical cancer were dismissed & took years to diagnose, luckily it was a slow growing cancer. They found endometriosis when they operated for the cancer, it had been undiagnosed. My symptoms for the first years that I had developed hypersensitivity pneumonitis were written off as FM as well. It became chronic & the lung inflammation & scarring had almost killed me by the time it was discovered. For years I'd been using all those "pain management techniques" to cope after being told over and over my pain didn't mean there was harm to my body, or that it was psychosomatic.

Hey Andrew, you said that you read all of the comments so I thought I might suggest here some ideas for future episodes in case you'll read this: 1. Stretches for after workout, rest and recovery. The importance of warm up and stretching, active vs passive stretches, the difference between stretches for releasing muscles and stretches for splits and are middle splits even possible due to the design of the pelvis-femur. Also, how to stretch with massage/lacrosse ball, foam roller, are they necessary or can self massage be sufficient, oils and ointments you can use and treat areas. Basically how to enhance and maximize Recovery Day. The being (rest) instead of the doing (workout). 2. I dance on extremely high heels (8") and started walking and running lately as a sport and it started to hurt my feet a little, so would appreciate a segment for those who exercise their legs and feet a lot, in order to avoid injury or future problems. I understand that standing on high heels shortens the leg muscles? So if you could also elaborate on how to best choose quality shoes: daily use, training in the gym, running on trail, on road, high heels if at all etc. 3. Cats and dogs and their influence on human relaxation and why we find them adorable and comforting. Allergies and stuff related to pets in general. Pregnant women and toxoplasma. I think you could definitely be a cat person, and also, cats are more viral on the net than corona at its peak. 4. Breathing exercises - alternate breathing exercises, pranyama, ujjayi breath technique, all kinds of interesting yogic stuff, just to make us aware of whats science-based and whats bull5h1t. 5. Intelligence - what is it, how can it be measured, can it be enhanced, if it cant be enhanced how does the flynn effect exist, and if we are not debating intelligence but just effective learning process of individuals, what can you do to improve it neurologically? Eat fish and learn a new language? Play an instrument? Crossing the body midline for strengthening the conncetion between the hemispheres? Read that Einstein' corpus callosum was a thing, as well as changes to the brain after learning a musical instrument. Also, can you tell an intelligent person's iq from brain oscillations? As in, how it behaves when a high iq person does intense thinking or resting, vs a person who has average iq? What happens under gamma? I understand that long time meditators experience that a lot. And where do we go when we delta and where does self-realization lies. 6. Longevity diets - okinawa and greek islands' 100+ year olds who live happily and in good medical shape, how does this miracle happen and does it have to do with living by the sea and having a nice breeze and sun light. 7. Geriatric episode - how to treat the elderly, signs of aging to look for, dementia, alzheimer etc. Do crossword puzzles and coloring mandelas actually help them?! We all (or most) have parents and they happen to be older than us :/ 8. Death. A bit morbid, and perhaps you will need to let this type of content to mature, but it feels like death is everywhere. What is the process of dying, what happens to the body, near death experiences, how to deal with the grief, loss, trauma, pain, guilt, and all the negative feelings associated with it, for the people left alive. 9. Hair skin and nails - can you explain about the structure of skin and hair (I know you did that on hair loss but maybe a refresher), the types of skin and fundamental skin care routines, skin and scalp diseases and treatments, sunscreen - chemical and physical, foods that can help with skin protection (lycophene?) as well as general nutrition, hydration. 10. Emotional well-being, empathy, strong relationships, volunteering, compassion, toxic relationships and how to avoid/handle them or being "that" kind of person. Understanding one's problems. Journaling as a means to process emotions, aside from therapy. Cbt? 11. Seasonal things - like treating of allergies in spring, sunburns in summer, sluggishness in winter, and being scared of school come septmber. 12. Holistic approach to taking care of your mental, physical and emotional well-being. With all the protocols set aside, what would be the *key components* for people with less structure in life to maintain an overall sense of feeling good. If you could take from all the protocols so far the most fundamental needs and have them as the basics for building up and nourishing all the others - what would they be. 13. Elaborate on the concepts of bdnf, carb cycling, body recomposition. Didnt know where to stick those. Your channel is wonderful and its just unbelievable to me that this body of work made of 2-3 hour long videos are probably just the 5% tip of your actual scientific research iceberg. In a world where people can barely handle a 1 minute video, thats truly amazing. Thank you for making science available and comprehensible to the public ^-^ P.s havent watched the video yet so i apologize if some things here are answered

I’d say dancing with high heels (or wearing them at all) is something that is incredibly destructive not just to your feet, but to your entire posture as a human. If it’s something absolutely required for the thing you love doing most, it might be one of those trade-offs that you have to accept (like CTE for football players).

Thank you for this. Really helpful. I'll add to the requests for more on fibromyalgia. Potential questions: What is the best current understanding of what it is and what is going on in the brain? "Fibro fog": is it a side effect of pain, or there something else going on with the brain concurrent with the pain signalling issues? Is there any reliable research on effectiveness and safety of cannabis, acetyl L-carnitine or other supplements for folks with fibro? And please, more on the art of pacing! That is so much harder than it sounds.

I am enjoying this. Chronic pain is something that my son is experiencing and refuses to take any painkiller tablets or analgesics. His one is neurological, which affects his spine and travels to his whole body.

I have Morton’s neuroma - just started having symptoms the past few weeks - and knowing that being on my foot is not causing any harm or tissue damage was such a relief. My dogs thank you as much as I do. I can power through some short walks with them now! But, yeah, it does hurt. A lot.

Thanks for another informative episode! As I was listening to this, I was wondering if the subject of breath work for pain management (such as Lamaze breathing) would be discussed. I'm wondering if there have been any studies on various types of breath work as a behavioral intervention for pain. When I experience intense pain due to severe endometriosis mouth breathing and moaning are a natural way to deal with the pain, trying to avoid it in front of people only makes things worse.

There’s a kind of shame (pain) you need to overcome to tune into this, cause you have to admit that you have some sort of pain & we don’t like want to think of that pain, we don’t want to remember it, cause that’s painful 😅 this episode made me just realize that life is nothing but pain management, it never goes away completely, sometimes it’s in your face, some times it’s in the background. Sometimes it’s a hangover, other times it’s a failed relationship. We try to forget it, fill our lives with less pain. And to even reduce pain takes pain, going to the gym, saving up and not upgrading to the latest gadget you love so much. There’s an ice pack on my knee from a 10k run I just finished while listening to this episode. Life is truly pain 🥳😅😩🤕💀

I am new to your podcast but have listened to several episodes and really enjoyed each one. This was one of the best discussions about pain I have ever heard. I appreciate the topic and Dr. Mackey's work! As a chiropractor I have to say that it got a little awkward when you asked about chiropractic. He obviously dodged and then didn't really have anything positive to say about it. Bringing up vertebral artery dissection was completely irrelevant. Also, he made it sound like there have been no positive studies about chiropractic and pain which is not true. That was the only brief moment in the podcast where he showed his bias. In my 20+ years in practice I have worked with many pain management doctors and have enjoyed a very beneficial teamwork approach to helping patients. Again, just one awkward moment in an otherwise excellent and important podcast. Thank you for work!

Question: Dr. Sean Mackey mentioned that in recent years there has been a shift in the scientific community going away from the concept of "the pain matrix". Is it possible to get the names of some key articles surrounding this subject? Keep up the good work, great podcast.

Love the fact that prostaglandin and noceceptors are mentioned. The variability between people. The increase in norepinephrine explains a lot of why snri's are used.

Hello , I love your show and I have a very urgent quick question..my right eye , all of a sudden, has a bunch of floaters and one very black very clear "string"...causing vision issues...this just happened in the last hour or so...who or what type of eye dr should I see for this..I dont want to just go see someone whoe prescribes glasses... thank you !

Such a good episode, I appreciate especially the part of refered pain. One comment : you forgot to talk about the one profession that is specialized in this kind of pain - osteopath! Would be great to have one on the show. Thanks for all your amazing work!

this is a great episode! I started my journey in 2009 and I did this exact experiment I had to figure it out on my own being that the medical community did not know anything about my gut issues said it was all my head. But yeah, I found out gluten soy corn syrup anything that's considered manufactured. I was able to get my issues completely under control with like a modified Paleo diet could be dairy for a while and now I've introduced some dairy back and I'm fine with it. It's pretty fascinating prior to the change in diet, I had chronic IBSjoint pain continual sinusitis and very cystic breasts. After cleaning it all out, my body is much happier, especially considering the inflammation in the joints being that I have a lot of sports injuries and quite a few surgeries, fascinating, subject and they're definitely needs to be more research done on it.

Hi Dr.Huberman, thank you for quenching our thirst in the science field! I was just wondering about the elimination diet you two talked about, in Australia the Royal Prince Alfred Hospital put together what they call a "Failsafe diet"(popularized by Sue Dengate) that has really impressive results in various issues, going from ADHD, autism, chronic pain, chronic fatigue, skin issues, mental health etc. It's quite fascinating to dig into that and to realize how food is interacting with our biochemistry in a very individualized way. Can't find any ressource or practicioner yet here in France, I wonder if in the US those foods/body interactions via the failsafe lens is known ? I find that's quite interesting. Thank you again for fulfilling our interest in science 🤓

😮Bell Pepper allergy. I luckily found (through a nice waiter) that I was allergic to “Bell Pepper” after having the worse stomach pain of my life. I had a tomato soup for lunch which turned out to be (1/2 tomatoes and 1/2 Bell Peppers). I started testing myself with some foods that had Bell Pepper and always had really bad stomach pains that lasted from 3 hours to 24 hrs +- and no pain killer helped. My life changed for a much better and happier life after removing Bell Pepper from diet. ❤

I'm so excited to see your research! It's the only thing that touched my interstitial cystitis, fibro, abdominal adhesions and kidney stone pain. I was shocked, but happy I was able to function and be a part of society. I also was on suboxone, bupenorphone, and methadone, I was always in pain but did what I had to to survive when my pain was too much for me to handle. With meds I was high but always still in pain. Being labeled, not caring if I lived. Cannabis doesn't come anywhere close for my physical pain, but together, I handle my ptsd, agoraphobia, anxiety and bipolar. Along with therapy. (I always promote talk therapy) Cannabis kept me from walking into traffic when it was bad and nothing else was around. I was so lucky, and I'm so grateful I found a reputable vendor right off the bat, 8 years ago. This has been the first year I've had to deal with things that would really push me over the edge before krat0m, but I'm able to deal now, not hide. Please keep up the great work ❤️🙏💪

One more thought relating to the statement, “ after a gut infection it can change the genetic expression related to sensitizing you to food antigen. You get an infection, and your gut no longer response properly to a normal food item.” I have a son with autism, can an immunization cause some type of gut infection to change the genetic expression related to sensitizing the child to antigens. It was clear to me that my son could not eat many foods, but test never showed a problem. My son is now 27 but I still would like to prevent autism. The gut biome is key to the answer.

When I stub my toe, I instantly step on it with the other foot and hold it down hard. For some reason, it helps. My kids thought I was nuts when they would stub their toe and I'd say quickly, "step on it!" It works though... that pressure helped reduce pain totally.

If I may: For a person who did not experience either at all or the fullness of natural and optimal attachment relationship (a strong foundational sense that they belong just by the fact of existence), to suddenly be exposed to a drug or situation that offers the "feelings" of acceptance and love is consuming. That one who developed coping strategies, even or especially subconsciously, will always be at risk for addiction at random moments no matter how beautiful and fulfilling their life looks from the outside.

About the nutritional pain. Mckey said that it's difficult/impossible for everyday people to self diganose what's causing the stomach pain and issues. I'm really super hyper allergic to lactase. In Finland were are supposed to have amazing free medical care. But my condition being so rare the doctors were unable to diagnose it for 7 years. I don't take: "We don't know. Eat pills." answers from docs. Finding it took so long long. Basic elimination diet. Where you drop milk products and then grain. However they toss that lactase to so many products here in finland "Just in case" I'm also allergic to lactose. thankfully I can eat well matured cheeses at least. 😋 Lesson here: don't give up trying to find what causes your stomach issues. All health starts in the gut. And there is virtually no condition that you can't affect with diet in some manner.

Typically in many cases Chiropractic is covered by insurance. Massage therapy can change lives wonderful ways! Also Reflexology is extremely effective! Seen these modalities work wonders for the past 15 years! Get references.

I've got the pain. I try to ignore it. Wakes me up heating pad helps to put me to sleep if I wake up I turn it back on so I can go back to sleep. My parents told me when I was a kid your too young to have pain. Years later I had a root channel fixed my neck pain. So I think there is always a reason. You just need to figure out and find the why. I had carpultunnle surgery and it messed my shoulders up. Still haven't figured that one out its been 10 years. I think I have arthritis in very small amounts threw out my whole body. I'm the same with onion and garlic. Bad food poisoning when I was 14 or so. Felt like death wonder how close I was. Thanks for the info.

I have been living with chronic pain for years. While I have never received an official diagnosis, I'm fairly certain about the cause. I've undergone three challenging spine surgeries and six nerve decompression surgeries on my neck and arms due to a car accident. I took OxyContin and another pain medication for 10 years, and getting off the opiates was the most challenging experience I've ever been through. Later, I discovered I had neuropathy in both of my feet. Interestingly, when I stand up, my nerve pain completely goes away. The pain is intense when I sit or lie down. I'm determined not to go back on opiates. Kratom helps with my nerve pain, but I prefer not to take it more than once a day. I've been using Kratom daily for 2 1/2 years. Recently, I started taking gabapentin, which helps enough for me to sleep pain-free. Living in Southeast Asia, where there are no pain management doctors, makes the situation more challenging. Your videos have been incredibly helpful. Thank you.

I’ve taken kratom for awhile now and I’m cutting down as I realize it’s probably not as good as I thought. I want to know is it healthy, especially neurologically in moderation? One to two eighth doses a day. I respect ALL of your work, you’re open minded and you discuss controversial topics that NEED to be conversed. Thank you Dr Huberman!

I take it in small amounts for a year or two now. Some constipation as a side effect. It does help me. I try to take as little as possible, but with degenerative discs/neuropathy in my feet I’m doing the best I can

I am just getting to the Aspirin part of the podcast where Huberman increases the Aspirin when in pain. Someone please get to him the message that he should not do this due to the fact that it can increase his risk of a GI bleed. Aspirin is not an NSAID that should be advised for use for anyone outside of their PCP and most importantly their Cardiologist. Please someone get the message to him that - it is an anti platelet NSAID and not and anti inflammatory NSAID!

In full disclosure, I wasn't very interested in this topic, so I almost skipped this discussion, but I played it in the background just for kicks and some noise, and I'm so glad I did; I learned things I didn't even know I wanted to know about. Dr Mackey so effectively communicated the information you asked him about, respectfully navigating the controversial or less 'conventional' interventions, and even in your '10-questions-in-1' ambush I MEAN format haha. I appreciated how you both intuitively knew when to pause to break something down for clarity or to back it up to recap the information presented. His long standing dedication to to improving the understanding and managing pain for all is heartening too. Good stuff.
