You want to hang a picture or a tchotchke on your wall and your wall has lead paint. What do you do?
If you’re not going to use Velcro or Command strips to hang pictures on your wall and you have chosen to sink an anchor, there’s the proper way to do this without spreading a ton of lead dust all around.
First, take a one-gallon Ziplock bag and tape it to the wall. I taped the corners in to make sure the bag will stay open.
Wear gloves as you don’t want lead dust on your hands.
Use a damp paper towel to help catch any debris and drill a new hole. These walls are cement so I’m unable to just tap a nail in… it will destroy the wall.
Drop the paper towel into the bag. Use another damp paper towel to wipe the wall where you just drilled the hole and make sure all the dust is removed. Carefully clean off the drill bit with the wet paper towel.
Drop this paper towel into the bag. Remove your gloves and drop them into the bag. Remove the tape and drop the tape into the bag. Seal the bag and throw it away.
Set the anchor carefully with a hammer, drill in the screw, and hang your picture. All done!

Does simply wiping drill bit with damp paper towel effectively remove all lead dust? Is lead dust hard to remove from skin/hair/clothing/surfaces or very relatively easy?

I should have shown cleaning my drill bit with soap and water, specifically detergent (one that contains phosphate is best, (Cascade Dishwasher detergent, or Electrolsol), or TSPl work well. Lead dust isn't hard to wash off, just don't use your kitchen sink. A bathtub or if you have a set tub, this works well. You want to keep the lead dust away from anything you eat.

How can putting a nail in the wall damage it significantly?

Property units built before 1978 have many different substrates used for interior walls. This varies from lathe and plaster, first-generation drywall that has plastered different layers on top of rock lathe, aka gypsum board, to today's style of drywall. Drywall is often found when the property unit has been abated for lead paint and these are just a few substrates. I lived in an old 1932 house with first generation drywall that had a layer of cement plastered on as the first coat with a plaster layer on top of that and lead paint. I figured out early on If the nail was bigger than a small pin nail that I carefully drove in, I had to worry about the plaster blowing out or cracking and creating fissures in the cement layer that came back to haunt me later when I had to fix the issues. When nailing into a wall, most often nails would simply bend over because the wall substrate was so hard. Lead paint dust issues aren't derived from the act of driving a nail into the wall, it's the damage that is created to the substrate, then removal of the damaged area that creates the hazard. Use stick-on picture hangers as much as possible instead of hammering a nail into the wall.

I recently signed a rental a agreement for a home built before 78'. The realtor says there is a chance of lead based paint, but she isn't sure. The house has been rented on and off for five years and has been painted over during the time. How many times it was painted over, I'm not sure. Am I at a health risk staying in a home like that? Also, if I hang up pictures and frames with thumb tacks or nails, is the level of dust a health risk? And does the hole itself emit lead based particles or is it just the dust that is a health risk factor?

When was the house built? The likelihood of lead paint varies over the years. If the house paint is in good shape and not peeling and cracking the lead paint would be buried behind the layers. It’s sealed and/or encapsulated so you should be in good shape. When you know there’s lead paint underneath you take precautions. When making any change to the substrate the goal is to keep the dry dust down to a minimum. Depending on the age of your house, putting a nail or tack into the surface can really do some damage to the wall. Some lath and plaster will crack around the nail creating the dust. Some nails simply won’t go in… My house. If you suspect you have lead paint underneath all the layers you never dry sand your walls. Always wet sand. It’s the lead dust that’s the health risk and for a fun added note, lead dust is invisible to the human eye.

There are also lead chech kits for at home when you're not sure. Like from 3M.
