Meanwhile, if the patient does not drop the arm, but is in pain, that is indicative of Supraspinatus tendonitis as the abduction impinges the inflamed tendon at the acromion-coracoacromial arch. Just saying, there is more to this test that should have been pointed out to distinguish a tear from tendonitis.

I’ve had trouble performing this on patients who come to the ER for shoulder pain thinking they have a rotator cuff injury. They scream in pain with passive abduction and resist the movement-can’t hold it still till I let go of their arm.

The test not for infraspiatus which is an external rotator

The whole rotator cuff muscles are active to the same degree (Wattanaprakornkul et al. 2011) in abduction. What the authors in the study we provided found is that the supra- and infraspinatus were mainly affected in people with a positive drop arm sign.

Thanks for the compliment! You might wanna check out our video on "Shoulder Impingement Explained". There are a couple of hints on how to diagnose it, but it's really hard to determine a specific structure that is responsible for painin the shoulder, because tests are almost never very specific.

Hi, what would A/PROM be after a repair? for example a week (even though not advised)

Varies per individual. PROM is usually free although we see that patient's often have protective spams at around 90°. AROM often limited due to pain.

@@Physiotutors pain management is key. and small sessions throughout the day would be good. like 3x 10 minutes every day. but Rest Ice Heat. and then each week increase load of exercise and increase duration of each session.

any full thickness tear exercises?

if you have a complete tear, it is not smart to do excercises. a full thickness tear doesnt heal by itself and usually requires surgery, i highly recommend to see a doctor if you havent.

It really depends on how large the tear is, it can be a full thickness tear that is only 1cm across, or it can be massive 5+cm tear. If the tear is large enough to go across the entire supraspinatus, then you have lost functionality of that muscle and it will need surgical repair, unless the muscle has retracted itself in which case the best you can hope to do is compensate for the loss with the deltoid muscle. If the tear is small enough that the supraspinatus can still function, that there are some light resistance band exercises that can be done to strengthen whats left attached. Unfortunately, once there's a tear, it essentially becomes a zipper slowly opening up more and more, and the likelihood of the tear becoming worse is pretty high. Surgery to repair the tear is always an option but know that the repairs are fairly easy to re-tear. I'd recommend going to see a PT and see if you can cope without surgery, a lot of people can function just fine without them if the tears are not massive.

Adam Cook . I’ve seen a doctor and had both an mri, X-ray and ultrasound. The right supraspinatus is torn 13mm in length and 16mm width. Left is 10 mm in length and 20 mm in width. An ultra sound guided Cortisone injection has been recommended by the specialists. I think it must have been a skiiing impingement.

@@CaffeineJogger those tears are classified as medium in size and would likely have similar outcomes with conservative PT treatment when compared with surgery. I'll link this study if your interested in researching it www.ncbi.nlm.nih.gov/pmc/articles/PMC4855119/ Shoulder impingement is a likely suspect in the formation of those tears, but impingement could be caused by a variety of factors. Impingement would also not cause a sudden tear, but would make the shoulder more vulnerable, due to wear and tear over months to years, to a traumatic event to the shoulder such as a fall. The Cortisone injection will be helpful in reducing the inflammation in the shoulder if there is a lot and will help to reduce the impingement since the inflammation would be contributing to having less joint space, if there's not much inflammation the injection likely wont be that beneficial. Also the injections are only temporary solutions to the impingement problem, proper body mechanics and shoulder stability is more beneficial long term. Long story short, PT is a good option for you, they'll address shoulder/scapular strengthening and stability to help the glenohumeral head remain in the correct position in its socket. They'll also advance your exercise program appropriately so the risk of worsening the tear is minimized. A weak rotator cuff allows the head of the humerus to drift upwards resulting in one cause of impingement.

Hi, doctor! I think i may have a rotator cuff tear, im 21 yo, trying out chalistenics and after some time i felt pain in my shoulder. I did some tests i flund5in the internet: the lift arm behind the back test and it hurts (sharp pain) but i can do it, and also i can do the pointing down thumbs arm raise, but it also hurts. Should i go to get attention or just let it heal and rest? Thanks!
