Eh, I would say anyone who gets close to Subaru starts to understand that he's suffering a lot - Echidna knows the full details (and Roswaal understands to an extent as well), but while Rem saw him at or near his worst state, Emilia, Betty and Otto are all aware that something's wrong on the inside, I think. Even Puck and Ram, I feel, suspect something.

In the Kasaneru path, Subaru ends up a desensitized monster who isn’t even human anymore after killing himself over 100 million times because Echidna was constantly influencing his death loops for her own amusement.

Echidna is definitely my favorite character in Re: Zero. Would have loved to see more of her! She is neither an evil nor a benevolent character. She just lives and does things that satisfies her curiosity, so even if she does something heinous, its not like she finds sadness or happyness in that, she gaine knowledge which satisfies her thirst. Same goes when she does something good, she doesn't care about anyone, just her own satisfaction of greed. Kinda feel bad for her because no matter how strong she is, she is a pitiable character cursed who will never find satisfaction because there is no end to knowledge.
