ECTOPIC HEART BEATS - COULD THEY BE DAMAGING MY HEART?

Can ectopic heart beats, PVCs and PACS damage the heart?
Today I wanted to do a video to hopefully reassure patients why ectopic beats do not cause damage to the heart.
The heart is a pump and its role is to get oxygen rich blood to our organs. To function most effectively as a pump the heart has to beat at a certain optimal rate and in a regular rhythm. If the heart goes too slow or too fast it becomes less efficient and less effective. If it beats irregularly it beats less effectively. It therefore is reasonable to say that any heart rhythm disturbance makes the heart beat less effectively but only for the duration of the heart rhythm disturbance. So when we get an extra beat, what is happening is this:
Now as you can see it is not that there is no blood coming out of the heart during the extra beat. It is just that there is less blood coming out of the heart at that time but the next beat that comes along a little later and is felt as thud, more than compensates and essentially terminates that very transient period of inefficiency.
For damage to occur, there has to be a sustained lack of blood going around the body which would then mean that the cells within our organs would starve of oxygen and eventually die. In ectopics beats by definition are non-sustained. The period of inefficiency is so transient that it is simply not long enough to cause harm. The minute you get the thud, you know that the period of inefficiency is over.
Let me demonstrate this by using an analogy.
Imagine someone compressing my neck. Sure it feels uncomfortable but it is not dangerous, unless I compress the neck for a sustained period of time. If I compress the neck for less than a second and let go then it is highly unlikely that I will cause the person to die of suffocation. I could compress the neck as many times as I wish but I will not suffocate the patient unless I keep compressing for a sustained amount of time.
Ectopic heart beats by definition are non-sustained beats and therefore there is not going to be a period of sustained inefficiency and this is why they absolutely do not damage the heart.

God bless this man! I'm 61 at present and became aware of a fluttering sensation in my chest in 2021. I already had a Kardia device and it showed my heart was not in normal sinus rhythm, but nothing specific. So, I subscribed to the premium version and it told me they were PAC/PVCs. Needless to say I was extremely alarmed because my history with my Dad is Afib and ultimately a pace maker. He lived to be 91! I went to my cardiologist and they ran a holter for 24 hours and I had a about 3,000 ectopics out of roughly 100,000 beats. That is about 3%. My Doc said that unless the ectopic burden gets up to around more than 20% they don't even "get interested." He even said that an echo was optional at that point. Needeless to say I was relieved, but that nagging worry persisted as the fluttering continued. Some days I would just sit most of the day frozen in fear. I happened upon Dr Gupta who has honestly changed my life with his reassurances. I still have them and a few really give a kick, but I just carry on with Dr Gupta's words echoing in my head. I can successfully ignore most of them now.

It's unnerving at times, but I just keep saying "it's not life threatening" and try to carry on. Stay strong.

Been taking CoQ10 for close to 30 years. Last year all of a sudden my BP went to 90/40, I was in heart failure, and I needed my aortic valve replaced. In replacing it my surgeon found a 5mm hole in a leaflet of my aortic valve. I also had a bicuspid valve instead of a tricuspid valve. Although I went through a catheterization and a TEE the cardiologists did not find the 5mm hole that was in my leaflet of the valve. I recovered, but still wonder if CoQ10 helped me all through the years of having a hole in my heart.

I am 67 and was recently diagnosed with afib; my "ecotopic beats" are constant - I feel no discomfort from them, save for the knowledge that I have them. I had an echo showing NO abnormalities with my heart. I am a life-long athlete - runner for 47 years - I stopped this with the advent of afib, but continue to walk 10 miles/day. I am heartened to hear that you have been living with afib for 45 years; I have curtailed my physical activities (rowing, elliptical, running, etc) because of the CONDITION - not due to any discomfort that I have. I have a consult with Dr. Gupta on March 3, so hopefully I will get positive feedback that will allow me to resume my normally VERY ACTIVE lifestyle. Thanks for posting your history of afib... that alone, makes me feel more confident

Perhaps this comment could be helpful to someone. After experiencing 10-20 ectopic beats, as explained in a video, my cardiologist recommended taking 120mg of Magnesium citrate daily. I split the dosage, taking 60mg in the morning and another 60mg in the evening. Surprisingly, this almost completely eliminated my ectopic beats. I still occasionally experience one or two, but they are rare. It's important to note that this is my personal experience and may not work for everyone. The effectiveness could depend on your medical condition and other factors. For context, I am a 29-year-old active male, weighing 71kg and in good physical shape. All the best!

Unless you have thousands (Say, something like 20 thousands) ectopics a day, they're usually not considered more than annoying or discomfortable. Frequent runs of ectopics may be frightening, and they can be a sign of a heart close to get atrial fibrillation. As long as most of the beats are "normal beats", there's likely very little to worry about.

Did you know that a loose tooth crown or infected root of a tooth can cause PVCs? Someone went through 3 years of PVCs where their heart skipped a beat every 4-5 beats, they were in perfect health otherwise but while investigating the PVCs, they noticed during two of the Zio patches they wore for 2 weeks at a time, that the PVCs went down from 15-20% down to 0.1% over the 2 week period while on antibiotics. This was because of an infected root canal (no symptoms but seemed to be Loose crown > led to infected root canal > entire tooth removal. The entire tooth removal stopped the PVCs entirely. After some research, I saw other people in online blogs had been through the same. This should be published in a journal and doctors made aware that with especially generally healthy and younger people, the cause of PVCs could be hidden infection/inflammation. Happy to help you with this. Your channel is great, thanks for all you do.

I've had ectopic beats all of my life and I'm now 58 years old. Unfortunately, I got pulsatile tinnitus when I was 53 years old; possibly a result of jumping out of a plane at over 12,000 feet. That means I can hear my ectopic beats as well as feel them. That can be very disconcerting! I had various tests, etc. and my ectopics were considered benign. Two things I've found with my ectopics: * They never occur during exercise when I have higher heart rates. My resting heart rate is typically around 50-55 and I only ectopics when resting. * Going on a low carb diet appears to have significantly reduced ectopics. Might be coincidental, but that is what happened. For anyone out there that also has pulsatile tinnitus, which is an unfortunate combination to have with ectopics: * For me, when my blood pressure is *lower* my tinnitus is more obvious. At 120/80, I don't hear it much. When the tinnitus is louder I might hear ectopics more, then my blood pressure is coincidentally more likely to be 110/70. * Caffeine reduces my tinnitus a bit, but that is not a lot of caffeine. Just a cup of instant coffee. * Potassium supplementation (1.4g potassium citrate in 450ml water) reduces my pulsatile tinnitus a lot! The potassium is not enough to noticeably affect my blood pressure and yet somehow it reduces the pulsatile tinnitus. These days I rarely notice my ectopics.

@@alaskawanderings8518 I've never had a scan, but I was seen by a specialist and the diagnosis was considered benign. Not anything like an embolism, tumor, etc. The first occurrence was not long after doing a sky dive. The pressure on the way down caused me significant ear discomfort and hearing loss for a few weeks afterwards. I suspect the pressure might have damaged a blood vessel. Subsequently it became more persistent for longer periods and then became permanent. Very loud wooshing, so that i couldn't hear properly and then also had a bad episode of eptopics. Maybe give potassium citrate a go? My full dose is 1.4g twice a day in water and the result is miraculous. Less is not enough and more doesn't give additional improvement. The effects last for over 24 hours, but I once missed a weekend and was badly wooshing again by the Monday. Before anyone talks about potassium rich foods, potassium from foods did not work in the slightest. Only a supplement ever worked.

The sad part is, we still have the troubling symptoms. Taurine is helping a lot though (self-medicated)

Thank you for the video. I have been suffering with these for 5 years. On and off using the dare approach they have been bearable but since August I have them every day. I am beside myself and don’t know what to do. I don’t drink anything with caffeine, have quite a healthy lifestyle and always walking around for my job. I had a 5 day EKG, stress test, BP 24 hour test and echocardiogram in 2021 and everything came back ok but I didn’t have them as bad. I can’t use the DARE approach as I just can’t accept them. I just feel like I’m living in hell and I’ve tried looking for answers everywhere. Vagus nerve, stomach links for acid reflux (I suffer bad with this). I’m always coughing also because I get them. I also get chest pain and collar bone pain. When I don’t think about them I don’t really notice them until I get one then think wow I had 2 hours without feeling shit. I don’t get them in the morning after waking. Mainly after breakfast and throughout the day. I just don’t know what to do 😫 writing this in the hope that Dr Sanjay or someone can help me 🙏

I’ve had these palpitations exactly the way you described them for a long time now many years. Only recently within the last few months has it become very frequent and concerning. I went to see a cardiologist and they did a blood test. Turns out I was very deficient in vitamin d. 50,000 iu of d2 once a week seemed to help for only a day or two but they would return. Then I stumbled on your videos about magnesium and I started taking 225 mg of magnesium citrate once every two days. I can say with confidence that that did the trick! I am so grateful to you. They said it would have been there since birth and referred me to surgeon. I knew that wasn’t right because I had just had a heart Cath and nuclear stress test. Went to my cardiologist and the echo showed fluid in the pericardial sack. She said she didn’t know what caused but my body would reabsorb it. I’ve been having the hard PVCs again. Could that cause my aorta to regurgitate and look like PDA and put fluid in my pericardial sack. What can I do. They say every test shows nothing but normal. Thank you.

It doesn't happen at night and when I wake up it is normal but then I worry when it will start and it does. I can do the running up 5 flights of stairs in less than a minute so I know I don't have a diseased heart. The odd thing is when I do excessive exercise it beats normally while I do it. I recently started getting more of these at night a couple of months ago. They seemed to appear after taking some medication I was prescribed by my family doctor, which I've since stopped taking. I purchased a Kardia mobile 6 and it is picking up occasional and single PVCs as well. My PVCs seem to mostly be at night while sleeping and are waking me up. It feels like my heart is skipping on and off constantly in my sleep. I have an upcoming appointment with my cardiologist (who I havent seen in several years). My past EKGs and echos at cardiologist were normal other than possible mild mitral valve regurgitation (cardiologist didn't seem too concerned about it). You do not address multiple PVC's such as bigiminies, trigiminies (sp?). They can last up to 30-45 minutes before going away. Very infrequent for me, and I have had numerous evaluations with 12 wire while they occur and I am told to "go home and relax" you are going to be just fine. Echo cardiograms, PET scans show no ischemia. Holter monitor shows less than 1% PVC's over a 2 week period, but when they do occur, it is not a single beat as you discuss here. It would seem wiser to me, for you to discuss multiple beats rather than a single beat? BTW these occur so infrequently, and my tests have been so good overall, that they do not believe a visit with an EP is warranted. 😊

Thank you, Dr. Gupta. Do PVCs, etc. signify a risk for, or can they lead to/develop into, or create pathways for, something more aberrant, like atrial flutter and fibrillation? Also, please comment: if at one level of TSH they are mostly absent but at a lower level of TSH occur more frequently. Finally, is there a frequency of ectopics that requires/would benefit from intervention?

I've personally found that if I get an ectopic at 'the wrong time' then it can cause tachycardia. It happens very rarely, much more rarely since I started a low carb diet, which may be coincidental. For me, the wrong time was an ectopic that happened whilst I was lifting a heavy weight or otherwise suddenly exerting. It didn't happen during exercise such as cycling.

Except for MRI, Electro study, and exercise test (all my doctors said were not needed), all were OK. The only big problem I had was VITAMIN D DEFICIENCY (mine was 16 while normal numbers need to be around 45-70.). My doctor put me on 4k UI D3 every day for a few months and then 2k UI a day for 5-6 months. In about 1-2 months period, my ectopic burden of PVC, PAC, doublets, and triplets (a total of 700 extra beats per 24H houlter) reduce to 8-10 beats a day, most of the days being simple beats. I was low in vitamin D because my work and life are mostly indoors for years and years....and I am not a very active guy. I am 50 years old and overweight. I used to be very active, with great muscular mass, and I used to make about 40 km per day on my bicycle and also 10-15 km per day walking. Every day for more than 20 years. Now, in the last 10-12 years I am just a couch potato...sadly! Ohh, and yes, my anxiety (I had attacks every evening) went down A LOT!

I did well during the stress test, performed the entire 9 minutes but my heart doc says my recovery is not good and I have a 'dead spot' at the bottom right of my heart. He would need to do a heart cath to see if it's a blockage or a calcification. Of course, he started out giving me all of the risks of the 'invasive procedure' and scared the bajeebers out of me. Now I'm not sure if I should do the heart cath or not. Of course I want to get down to the bottom of these chronic (I mean all day for years) ectopics. There are PVC's PAC's and other types. I can’t even stand up or walk anywhere without my hr going up and my irregular beats going to fast and dizziness … so scary… :( anyone have any advice I’d love it.

Doc Gupta, pls kindly give me some advice . I saw the cardiologist a year ago & he says I got a tiny leaky valve & ectopic heart beats & my arteries are healthy with zero clogging. But I am still getting tired easily , shortness of breath & light headed . I’m Celiac too. Otherwise all is fine . My GP thinks it’s all anxieties and starting to see me as a mental health patient which truly makes me feel like I’m not being heard. My thyroids are fine , just got that test done. But she did say I’m low on iron. So I’m eating red meat once a week but not much improvement. I’m going to start Maltofer Iron pills from tomorrow. I don’t smoke or consume alcohol or even caffeine. Pls advise me if you can , what should I do next? Thank you very much . Chong

Thank you so much. Just like many others here I have been experiencing these for a few years. I get absolutely terrified no matter how many tests I’ve done and how many times my doctor says not to worry. The anxiety and fear I’m going to have a heart attack consumes me daily. I get them at the oddest times some days very frequent others barely at all. I experience them most at night and wish I knew for sure what was causing them so I could work towards control them. I’m scared to work out because I’m scared I’m going to fall out cause they are back to back and sometimes the beats are so strong it takes my breath away or makes me cough. It’s the craziest thing ever and just started out of nowhere about 3-4 years ago.

I am 46 and I have had ectopic beats since the age of 40. I have anxiety and panic disorder . There ectopic beats are uncomfortable sometimes. I definitely thought o had a heart condition or A fib . I have had stress test. Echocardiogram and many ECGs and it’s always fine. Even though I am reassured by my doctor and nurse friends that it’s not deadly it feels like something is wrong. Rational mind does not kick in. I do try and watch videos like this to calm my mind. Thank you for this ❤

Same as what I am going through. I had a stress test and had 1 ectopic about 12 mins and then I had a stress echo and had 4 ectopics at rest, none during. My anxiety often gets the better of me. But I swear mine are caused by gas in my stomach

Isn't that the same as afib I have like heart rhythm and I need an ablation done or pacemaker but they tried to shock my heart into Rhythm six times I think that might have damaged my heart but now I'm on for blood pressure and arrhythmia medicines I'm on defaite alike Artisan Metropole blah and lisinopril 40 mg doctor don't think that 40 mg of lisinopril make your joints hurt but I got news for them it absolutely does when I was taking 20 didn't happen my take 40 it does happen but anyway I'm trying to figure out which is the safest way to go should I have the ablation done with all the blood thinners you have to take or should I get a pacemaker that will restore my rhythm my heart rates 160 because of a Reglan shot irregular shot is medicine for nausea she was worried that I might be nauseous because I'm missing and I had take taking some Pepto-Bismol that morning early but I was fine but she gave it to me anyway and I didn't think anything of it but it caused my heart rate to go to 160 and two days later it was still at 160 jumping all over the place and I had to go in the hospital three different times for before they got a medicine which is the feta lied to get it to come down thank God for defend alive because my heart rate stays about 55 now or 40 which is too well but at least it's not 160.

Dear Dr. Gupta, Could you please make a video about (Long) Covid and the heart? What are your thoughts on this? Inflammation as a cause? Therapy? As many individuals suffer from heart arrhythmia after covid, I think this would be very helpful. Thank you for your consideration.
