I enjoyed Ed's indignation at Greg Davies refusing to have a starter. Maybe a compilation of Ed getting exasperated with the guest's crimes against food? Or James getting worked up over guests lack of respect for sugar?
When you have the disease, it's really important to do that because of how ridiculed and stigmatized Type II diabetes is. Speaking from experience, there's likely a lot of pain behind it that Ed isn't expressing.

I absolutely love this stupid running gag, and I love that Ed gets genuinely annoyed when people tweet replies to his diabetes charity promotions with "is that because you fell in the Thames?" It's all the more magical because constant shit-stirring is something Ed normally does himself hahah
It's fine if he wants to play along with the in-joke on the show because James is his friend, but when randos do it as you describe it's them literally making fun of a disability in a context where it's totally inappropriate.
