I talk with the Scott Rossignol and David Degandi from eHealth Exchange about technical architectures and trust frameworks for health data access. We discuss the pragmatics of TEFCA roll-out overall, as well as health system comfort level with sharing data via document exchange vs FHIR. We dig into some of the challenges of managing FHIR connectivity across a wide ecosystem.
Discussion focuses on three architectures:
* "Facilitated FHIR" Architecture as proposed in TEFCA "QHIN Technical Framework 2.0 (Draft)"; this expects that dynamic client registration will allow point-to-point FHIR connectivity between any TEFCA participant organizations
* Network architectures that could reduce the need for point-to-point connectivity, potentially offering a near-term shortcut to implementations
* Adapter architectures that allow health systems to offer "QTF 2.0" functionality without waiting on the EHR to deliver this
