Ok man, we had a patient we were taking to the ER the other day. It was some type of SVT being irregular, I chalked it up to atrial fibrillation. But super early on it looked JUST like classic SVT morphology. She wasn't symptomatic, and denied the shortness of breath she earlier called for. But, I just didn't want to look like a dumb ass. The vent rate went from 90-114. So I did report AFib with RVR at a lower rate. It didn't help that we had less than a mile im transport time. I feel like a huge dumby in those "wham bam thank you mam" scenarios

I just left the ER yesterday because of the abusive nature of Shasta Regional Hospital night shift. They were literally freaking out but not really helping me. I have congestive heart failure and now I have been going into tachycardia A-fib, my pulse rate would would go all the way up to the 150s per minute in my blood oxygen at 82. And all they did was give me a chemical bath with all these different chemicals which made it worse.

Wow, I almost wondered if I was the one you transported. I had an SVT event a year ago, but I did not call with shortness of breath. My neighbor drove me to the nearest fire station because I was feeling faint for quite a while, and it wasn't going away . I realized my heart rate was super high. Emt's transported me, rates were over 200 beats/min. Took a few hours, finally restored to normal. The follow up cardiologist appt never mentioned afib. He said I basically passed a stress test, since I had no other symptoms. Yesterday I took a friend's mom in to see a retina specialist because she has a big black spot in her vision. The Dr did say that blood thinners can contribute to (macula degeneration) or vessels bleeding in the eye. I asked her why she is on blood thinners, and she said because she has Afib. My boyfriend says I had Afib, I said no I had an SVT. Which is why I'm here. But I don't think my SVT was very regular which seems as though i might actually have had Afib? Now I'm going to have to cal cardiologist and make sure. I do experience weird heart beats. But never pain or shortness of breath.

What about differentiating AFib with a very high RVR (160s-200s) vs true SVT? If you can’t tell the irregular at that rate and can’t see P waves regardless?

I know this is a year old but, at high rates close to 200 bpm it’s really hard to tell. I’ve been in several situations where we say they’re either in SVT or Afib c RVR. If they have a history of afib or have been in afib then that’s what we go with.

Almost only when I’m at rest ( sitting on couch in the night or lie down on bed), recent week I noticed my heart just suddenly irregular ,then normal for a few mins, then “pump”irregular again- back and forth for like 10 mins. Considering whole day I’m normal , meaning I’m having AF?

I want to the hospital in the night thinking it was my asthma and I was coughing so much I figured let me get a corona test to make sure I’m don’t have the Coronavirus 19 so they did my blood pressure put me on a monitor and my heart was racing I’ve been going from 50 to 150 out of 92 down to 60 it was all over the place like a mouse was inside the machine well I am 63 and I am overweight so they gave me a ended up they gave me a ablation and I was put on a heart monitor for the next two days given blood thinners I have a med to control of my heart beats I’m just praying to God I’ll get better , and I don’t know if it was the right thing or the wrong thing I did but I’m glad to be alive

I woke up last night, with bad heart burn, took a tums and laid back down.. heart started pounding and was well over 200bpm, went into the bathroom and turned the shower to cold and got in fully clothed for just a few seconds, went back to my bedroom and by the time i was back it was under 150bpm, i laid down and it was under 100bpm within a couple minutes.. the whole episode lasted less than 10 minutes and the heart beat was very steady when it was really fast, very even i mean. Don't know what to do with this stupid virus going around, I cannot go to my Dr, but if I go to the hospital, I may end up getting coronavirus. This has happened to me several times before, but never quite this fast, we always assumed it was anxiety attacks....

This is a common phenomenon with GERD. I’m assuming you got evaluated by now since it’s been 2 years

By now you've probably learned that this can be fixed with a medical procedure where they go into your heart thru a vein and do something inside...sorry, can't remember the terminology.

how to treat Svt in outpatient? can we start with metoprolol 5 mg and add amiodarone 200 mg?

It's a normal rhythm variant where the heart speeds up during inspiration and slows during expiration. It's not harmful.

Hi! Thanks for the simple explanation. I got lost in 2 places. Under the SVT list on the left, there is Afib with RVR. But then on the right side column, Afib with RVR is ireg. ireg Rhythm vs SVT says rhythm is regular. 2. At min 4.5) secs, you mention multiply by 10. I guess that was also meant to be multiply by 6? Thanks!

If there are 50 big boxes on the rhythm strip, multiply by 6. If there are 30, multiply by 10.

On a 6 sec strip you count the qrs and multiply by 10, on a 10 sec strip you count the qrs and multiply by 6 to get ventricular rate. Hope this help.
