Elden Ring DLC - Two-Headed Turtle Talisman Location 

This item is a Shadow of The Erdtree Talisman and can easily be obtained without fighting enemies. We start at the castle front site of grace heading southeast, continue down the mountain entering the Ellac River Cave site of grace. From the Ellac River Cave site of grace leave heading north all the way to the end finding the talisman sitting in a waterfall cave.
(from here you can also exit the cave to your right following the wall to enter another separate cave and obtain the nearby Rivermouth Cave site of grace)
