This patient had undiagnosed median arcuate ligament compression (MALS) of his coeliac axis leading to high flow and pressure in the SMA collaterals. He presented with a bleed from a ruptured aneurysm or pseudoaneurysm of the Inferior pancreatico duodenal artery. I embolised this with Glubran and lipiodol in a ration of 1:6 with good angiographic result.
