McKenna is an amazing actress. From Gifted to Haunting of Hill House to Ghostbusters Afterlife to Handmaid's tale to Friend of the family to Bad Seed 2.
You won't be surprised to learn that after THAT scene where she's handcuffed to a hospital bed McKenna woke up the next day to find she had literally lost her voice and her arms were black and blue from all that thrashing about! I read that somewhere... McKenna Grace is like someone else said here an underrated actress, she's so unbelievably talented that it's frankly scary. Plus in this video you see her with one of the best actresses that she's worked with: Elizabeth Moss (she has actually said that aswell).
She absolutely knows. She didn't grow up in Gilead. She was a grown woman wen it happened. She grew up in the land of the free and the home of the brave. And she went above and beyond in her cruelty. I'm from South Africa. After the apartheid government fell, there were amnesty trials. Those who were found to have done wat the state forced them to were pardoned. Those who were found to have done things beyond mandate, into cruelty, were sentenced. Aunt Lydia didn't have to take out Janine's eye and permanently maim her for a few remarks, she didn't have to beat June with a shock stick for trying to help her up the stairs and she sure as hell didn't have to rub Emily's mutilation in her face and a whole host of other things. She knew exactly what she was doing.
Blood tests give a good clue. Is she ovulating? Are her eggs plentiful and healthy? If yes then there's no real reason to think infertility. Anyway the book deeply implies it was the older men who had poisoned themselves and the planet spraying India with a chemical fertility blocker. None of the commanders fathered any babies.
So according to the book, and mentioned when June sees Rose in the Hospital when Off warren was shot, once girls start their period they get regular Pap Smears to check and see if their fertile.
They don't have to determine fertility with a young woman before making her a handmaid. They assume that a young woman is fertile and she's sent into service, and she's given a year or two (with multiple postings) to see if she'll conceive. Most of the infertility in Gilead is male, but they would never admit that openly.
