They return everything, it's all for show, it's why we see her wear the same rags year after year. All those clothing hauls she had done and she didn't wear any of it. She was walking around last year with a giant price tag on a necklace. It's all returned.
People want their picture took with her because she is a great person to be around. Why the hell wouldn't anyone want their picture took with her, she is the best person there is.

Doubt it. She’s been doing this since she was in her early teens. At this point she is a medical mystery but I suspect she will last another 5-7 years.
