In this video Doctor O'Donovan explains what a Hiatus Hernia is, including causes, types (sliding and rolling), symptoms, treatment options and complications (e.g. oesophagitis, Barrett's Oesophagus and more).
A hiatus hernia is when part of your stomach moves up into your chest. It's very common if you're over 50. It does not normally need treatment if it's not causing you problems.
It is common to have no symptoms, but if you do have symptoms they can including heart-burn caused by acid reflux (GERD/GORD), belching, and feeling sick amongst others.
See a doctor if:
- your symptoms do not go away after 3 weeks
- your symptoms are very bad or getting worse
- medicines from the pharmacy do not help
In the video we cover:
00:00 - Introduction
00:26 - What is a Hiatus Hernia?
00:52 - Sliding vs. Rolling Hiatus Hernia
02:09 - Causes of Hiatus Hernia
03:21 - Possible symptoms of Hiatus Hernia
04:13 - Diagnosis methods
04:35 - Treatment options for Hiatus Hernia
07:01 - Complications
Treatment for a hiatus hernia can include:
1. Change your eating habits, for example, eat smaller, more frequent meals and do other things to help with the symptoms of GORD/GERD.
2. If you smoke, try to give up, as it can irritate your digestive system and make your symptoms worse.
3. Buy medicines from the pharmacy (ask the pharmacist what you should take to help with the symptoms of GORD).
4. If medicines from the pharmacy and changing your eating habits do not help, see a doctor, who can prescribe stronger medicines.
5. If stronger medicines do not work, a doctor can send you for further tests to find out if your symptoms are caused by a hiatus hernia. They might also prescribe medicines for long-term GORD.
6. A doctor might refer you to a specialist to check if you need surgery. This usually only happens if other treatments have not worked and you keep having very bad symptoms.

Hi Doctor, thanks for the video. I am 49 year old male, (good weight and fit) and for the last 3 months I had nearly weekly heatrburns (never had this condition before). They prescribed me 20mg Omeprazole for a month to treat the heartburn symptoms which was relatively effective as I only had one hearburn in that month. Midway through the treatment I also had an endoscopy and they found a 2cm hiatus hernia and bile reflux. They told me to take Omeprazole for another month to allow the oisophagus lining to heal. As I don't want to be on this med indefinitely, and as I am afraid of dependency and rebound effects, I agreed to take it every other day for another month and do diet changes. However I experienced 3 hearburn episodes within 3 weeks of taking the med every other day, while trying to figure out my trigger foods (becoming a pescetarian) and times of eating. My question is: should I take the med for another month daily, or keep trying to come off it (every other day) while continuing to figure out my trigger foods? If the oisophagus lining needs healing, should diet changes not allow that to happen naturally? I am really reluctant to take this med for longer as I feel it would be more difficult to come off the med the longer I take it, isnt that correct? In other words, howong should I be taking omeprazole for with the symptoms, conditions and circumstances I described? Also, would a surgery not solve my issue once and for all, and allow me to eat everything again as before? Thank you

PPI is great. I took it daily for 15 years before I came to the realization that it slightly depresses my mood. If I found myself singing in the car on the way home from work and then sitting down on the sofa with a big grin on my face thinking life is great then about an hour later the acid reflux would start -- because I forgot to take a pill that morning. No one told me this was a concern. So if you are taking PPI then keep this in mind. Don't take 15 years to figure it out like I did!

I have a large Hiatus Hernia and have shortness of breath when walking or doing simple tasks. I find that sitting in a reclined position makes breathing easier and is more comfortable. I often feel like there is a tightening around my diaphragm, like a belt is pulled to tight. I learned more from your video than I did from my doctor who took 51 x-rays and told me nothing that was going on. I really appreciate your help. Thank You.

What u need to do is 30-45 min after every meal take half a table spoon of baking soda in a glass of water.. do this for 30 days and ur pH levels will improve and ur reflux will have subsided maybe even completely cured.. if not continue until completely cured.. also if u have severe reflux at the beginning while u start the baking soda buy a water melon and make a water melon smoothie.. its the most soothing thing for the stomach total relief and natural to. I hope I've helped.❤

Glad to see your comment, especially the "belt is pulled to tight" feeling as described by you. Also, finding a bra that isn't tight is another challenge and if a company came out with one, they'd sell a lot after seeing so many of these videos with people searching for answers. Hoping for the best solution for everybody!

@@neozeusbabba567 Just a small point. Baking Soda is known as Bicarbonate of Soda in the UK. My Grandmother had a hiatus hernia and she used Bicarbonate of Soda to relieve the heartburn. My brother has a hiatus hermia and I suspect I've got one too. I have a curvature of the spine as does my brother - we were born with it, his is more pronounced than mine at least when we were younger and due to an accident a few years ago plus advancing age, mine is now very noticeable (to my embarrassment). I cough a lot and get breathless, some days I can barely walk across the livingroom to the kitchen, also feel like lindacassidy1787 like I've got a belt tightening round my middle. For me, the breathlessness and cough are the most annoying part of it as it restricts what I'm able to do. There is a technique you can learn to push the hernia back and I've been trying that lately and it does seem to help. There are videos on here that show how to do it. The symptoms are always worse after I eat. I'm not overweight, in fact I'm really skinny.
@Pythonaria
@Pythonaria 2 месяца назад
@@eveleengray9067 Same problem. To get the proper size, 32A I find the manufacturers make the back strap far too constrictive. Just because a lady has a small bust doesn't mean to say her back is narrow. Although I'm a natural skinny I do have fairly broad shoulders and a broad back. I got round that problem by purchasing bra extenders. They work like a charm so now my bras fit me and don't cut me in half.

About 3 years ago I was diagnosed with a hiatus hernia while a gastroenterologist did an endoscopy. Apparently it is a rolling type. I know I need to lose about 20 pounds which I hope will help. Thanks for sharing this video.

I have a sliding hiatus hernia I was put on medication but I worried about the long term effects I also suffer from bloating from certain foods which makes it worse, I have found if I don’t eat later than 6pm that makes a difference I also found if I sit around all day that makes it worse, I take Gaviscon advance when I do suffer with acid reflux sometimes I take it 3 times in a week and other times I don’t take it for months, diet seems to be the key and a good walk after a meal definitely helps, I also use two pillows at night, also started doing some stretching exercises occasionally, think that may help

I just found out I have a small hiatal hernia (via endoscopy) and I have all the symptoms you've mentioned (difficulty swallowing, acid reflux, heartburn, chest pain, etc.). All the doc said was take omeprazole (PPI). Seems like it's not a long term solution. What are people's thoughts on surgical treatment?

I have a hiatal hernia. Had it for years. I suffer from regular acid reflux, bloating, belching (which you cannot control and is embarrassing) and sometimes after a meal my stomach would feel like a washing machine and I would vomit. Doctor prescribed Omeprazole but not had to stop taking it as this used to make me vomit uncontrollably. I now take Rabeprazole, which does not make me sick. The doctor said that surgery is not an option (lack of NHS funding for minor ops), so I try and control it with diet. I have recently cut out certain foods such as fried, spicy, tomatoes, which was a trigger for me as too acidic. I have started having Greek yogurt, granola and fresh fruit for breakfast, a small lunch and I try to eat my last meal before 7pm. This seems to be helping. I am also actively making a point to lose weight as I am sure this will also be beneficial. I am so fed up with being in pain so I am praying these measures will help my symptoms. I do not want to take medication long term and I only take the Rabeprazole when I have an acid attack. I don’t think it’s good for you long term due to cancer risks

Thank you for a fantastic, informative video. Can a Hiatus Hernia give a high reading in the standard FIT test for colorectal cancer? My recent FIT test reading was 168, I understand normal is a reading below 10, and I had a virtual colonoscopy, which has just ruled out any colorectal cancer or large Polyps. They have found a Hiatus Hernia, so I wonder whether that could be the reason for the very high FIT test score?

I have hiatus hernia and was prescribed PPI medication. This only helped short-term. I am not sure why doctors prescribed this long-term. What I found is that that these drugs neutralize acid in your stomach and cause you to digest more slowly. That is exactly the opposite of what you want. Digesting food slowly means your stomach will still be full when you go to bed. Instead, I have used Betain HCL with my evening meal to speed up digestion so my stomach is more empty when I go to bed. You can't use this if you have ulcers. Unfortunately, anything you eat or drink before bed can be trapped in the herniated portion of your stomach and sit there when you go to bed. I found that medications I take before bed does not go into my stomach on its own. So I eat a little soft, non-spicy food after taking medication and wash it down with plenty of water to get the pills in my stomach. I also found another side effect from PPI medication is that it slows the absorption of other medication. This made my BPH medication useless to me because it wasn't getting into my system. The result was waking up every two hours to go to the bathroom despite taking medication for that condition. I do not know why doctors like to prescribe PPIs because they really only make things worse. However, apparently, they go by recommended treatments and apparently the people recommending this medication don't have GERD themselves, or they would know it doesn't work long-term, and may also be dangerous to take beyond 2 weeks.

I've had a sliding HH for over 30yrs and been taking a PPI for about that long, a couple of points that you made just need clarification. If you have a hiatus hernia you should stop eating at least 2 hrs before bed, if you don't then you're just asking for trouble! PPIs do not naturalise the acid in your stomach, PPIs reduce the acid produced in the stomach, and can take up to 5 days before they are fully effective, they are not an antacid! PPIs do indeed affect other drugs, it's always best to do your own research on drug interactions. And my top tip, I've been examined by numerous doctors over the years, most, simply do not understand, or are sceptical you when describe your symptoms, if you get a doctor like that change to another at the first opportunity and don't give up!

I had a severe Hiatus Hernia several years ago. I had three major ops to sort it out. As the bottom muscles of my gullet were burnt away by severe acid I had to have my stomach raised in order to connect with the remaining working muscles. I have often wondered what my stomach is like now. I also had to have my stomach meshed up as I developed five or more hernias popping out. I have been on Omprezole since the operations as acid still occurs, but I do not have any side effects. Could you explain what did happen with the above please. I should mention for the first two op 18 hours and 15 hours respectively, I was opened up on me stomach . Stuart Broome, age 75.

I had an endoscopy five years ago and was diagnosed with a hiatal hernia. He didn’t say what kind it was. I have been taking 20 mg of omeprazole every morning since then, and your video makes me concerned about the increased risk of cancer. I didn’t know that long-term use of omeprazole could cause that. Before I was on omep , my symptoms were hideous heartburn caused by eating absolutely anything. Even eating yogurt, ice cream, oatmeal, white rice, bread, would cause me heartburn. I had to sleep with my back propped up because of acid reflux. But the omep did get rid of the heartburn. Lately though, I’ve been suffering from horrible stomach gas after eating. My belly becomes so distended that it looks like I’m nine months pregnant. I can’t eat Indian food anymore, nor most Mexican food. I’m not able to drink beer anymore. I can’t have more than two glasses of wine because it starts to burn my throat. My defecation is normal. I’m almost never hungry nor have an appetite, but when I do start eating, I do have the physical desire to finish it. No sudden weight loss or weight gain. I’m getting another endoscopy done in August. I’m 55 years old, and I really want to get this taken care of because it is a major quality of life issue.

You listed mostly carbohydrates and dairy products, this would give many people symptoms in the digestive system, then look at the ingredients in those "products", except for oatmeal and rice they're very high processed "food's".. if one can call what we find in the supermarket "food". Hope you find relief, and maybe start with diet ..
@larrydirtybird

@@Research-1st Thanks for the reply. I was just using those as examples of foods that normally don’t cause heartburn. Or do they? Maybe they do and I learned something new today. But you got a good point. I still eat way too many processed foods.

Hellow Doctor, thank you for your outstanding presentation. I just got diagnosis of small sliding esophageal hiatal hernia after having had CT of abdomen and pelvis. I was diagnosed with acid reflex two years ago and was placed on omeprazole for four months which did not make a significant improvement. I have continued to have acid reflex twice a week despite my best effort. I also have been experiencing certain vague feelings in upper abdominal region which I absolutely cannot find a word to describe it. This experience comes and goes and prevents me from walking. I am wondering if the vague experience in my upper abdomen is related to the small sliding esophageal hiatal hernia.? I am really desperate on an answer about the feeling that happens in my upper abdomen.

Folks, if you are having hiatal hernia issues, stay on it! Keep pressing the doctors and get it fixed, if it is intolerable! I had a msg exchange with my doctor and I said "Hernia" in 2007. I was having massive attacks in my chest which started tests, every kind of test, blood, urine, treadmill, ultrasound, mri, CT scans etc. 12 - 13 doctor's saw me and did Nothing for 8 years. In 2015 I had a Massive Attack and my wife knew something was wrong. Went to ER and after another CT Scan was determined that I had given myself a gastric bypass. My stomach was inside my chest and folded over. Had 2 surgeries in 2015, 1 in 2020 and my 4th in 2022. First 3 were surgeons, 4th was aided by DaVinci machine. 8 years was a long time to go without the care I needed. Now I am left in chronic pain on my phrenetic nerve, so it just hurts to breathe. Every ache and pain makes me scared that it will turn into A Thing, like my chest pain before. Everytime my repair failed I get back into The System. My surgeons did not want to do the 4th surgery at all bc my stomach just won't cooperate! I was kicked over to the Bariatric Team to get a Ruex-en Y bypass just to illuminate my stomach entirely. But fortunately i didn't meet some requirements so they rejected procedure. So they did 4th with DaVinci machine. Since 2007 I have seen over 30 doctors, surgeons, specialists etc. I have had about 15 CT scans. It sucks, but stay Proactive. You know your body and when things just don't feel right. I had massive attacks in my chest. Couldn't figure out what caused them. They happened intermittently, all hours of day and night. The littke attacks were 3 - 5 hours and The Dooozeys hit were 8-12 Hours long and very painful. Good luck to all dealing with HH.

Trouble is H2 blockers dont have a lasting effect, and they now want me to use ppis BUT i used them before, and each one gave me different side effects the last ones esomeprazole really gave me a bad stomach...so i have a dilemma...and also whats the good of using a drug that cannot be taken for any length of time?..the acid reflux will not just go away...antacids do nothing...neither do H2blockers.

Hello! I’m speculating that I might have a hiatal hernia but I’m not 100% sure. I do plan on going to see a Gastroenterologist but I’ve delayed because I have zero luck with doctors as they can never seem to find what’s wrong. My question is, aside from acid reflux, I have pain around my left shoulder blade along with a constant burping urge and the burping is what drains me the most, is that a common symptom? It’s as if gas builds up to the left of my solar plexus and I need to burp it out, only to have it build back up within 20 seconds. It’s seriously a very poor quality of life since it’s accompanied with major discomfort.

I also have squeezing pain in esophagus after eating as well as the feeling that I have a big lump in my throat. Cannot stand wearing tight clothes like a tight bra. I feel like food gets stuck in the herniated portion of my stomach. Anti gas pills seemed to help the burping at first, but now I mostly try to flush the trapped food into my stomach with water. Stay away from spicy food at night. Tomato sauce is bad. Try to have an empty stomach before going to bed. A reclining bed helps (or wedge pillows). You can try wooden blocks under the head of your bed to provide some slope. Anything to keep food or gas from flowing back into the herniated portion of your stomach.

Years ago I felt like my sternum was shiting side to side and I had a lump there. Finally after many doctors, one was concerned and ordered an endoscopy in which they put me out to perform. When it was over, they woke me up and put me in a room. When the doctor came in he said" just chew your food a little better. Later I went to the emergency room as I thought I was having a heart attack. They did all tests for a heart attack and then the emergency doctor said it looked like I had a hiatal hernia and I told him the doctor didn't tell me anything but to chew my food better The emergency doctor looked mad and gave me things to help out. I still have strong heartburn everyday that only omeprazole can take away. The problem now is my son has superbad acid refux and has to take 2 omeprazole. They just keep telling him its just acid reflux and watch your food . He drinks alot of water everday because he works out everyday but it started before he worked out he's down to lettuce bread and certain cereals. That's it! I read there was a surgery to fix it but he said the doctors just keep saying it's just Gerd I am so angry he won't push the situation more. The acid is so bad and now he's 27. I recently got told I have a hiatus hernia after having an endoscopy. I get lots of nausea I guess it could be connected to it or inflammation that they found in my duodenum. I also get a discomfort in my tummy and feel as if there is a bulge. I feel it inside me but when I am examined no doctor can feel anything if that makes sense. This feeling is to the left side not far under my rib cage. I actually thought it might be a hernia as in another issue not from the hiatus hernia if that makes sense. I am pretty sure I am waiting to see gastroenterologist but I am in the uk so we both know that might take sometime to happen! My gp suggested the pain sensation I am getting was from a hernia but nothing showed on ct scans or ultra sound. I guess I just need to patient and see what the gastroenterologist says! I haven't been seen by a doctor for this issue but my dad had GERD so I figured that's what I have. Last night I was bloated and that turned into extremely bad chest pain under my right breast and back pain between my shoulder blades. It felt like a heart attack it was so bad. If I can get myself to burp it will start to subside but then it comes back. This lasts anywhere between 3 hours to 10 hours. I've even had it happen for 24 hours before. It's really scary not knowing what I have. My dad's Omeprazole was helping but I have to take it like the day before for it to work and I never know when this will happen. Last night it was so bad I couldn't walk and could barely stand up, though pacing my kitchen was helping to get me to burp which is something my doctor told me to do after I gave birth to my son. I had a C-section and they think air got trapped in my stomach when they closed me up so they told me to walk around to get the air to come out. My son is 10 now so I doubt that is the same issue.

Doctor researching for more than 14 years and checking out Surgical solutions for HH got me thinking so let me not use medical terminology here to much so people can understand what im writing here so .Why mess with wrapping your stomach or inserting dangerous magnetic discs or even using mesh .When the problem lies in muscles .Nerve that control muscles gets damaged from pressure sometimes along with muscle than that muscle atrophy if you have HH for long time . Nerves as well as muscles can be repaired . I had my arm almost ripped off it was hanging by the skin after a bike accident and Surgeons took some of my working nerves from my leg and fixed my arm. Took some time for muscles to start working again but I do not see reason why is that not being done for HH repair surgery. If anyone can explain me this I would be most thankful.

Its much easire for them to do surgery that prevents you from ever burping or throwing up so you don't get acid reflux. The side effects are not so good for you, but is cheaper than repairing nerves. I heard people can't drink anything carbonated after they "fix" your HH because of severe bloating it causes, and no foods that can cause gas. Your life sucks, but they saved money.

This way they did not get a dime from those millions and I am disabled for over 14 years. Had to close my business and eventually money ran out. So I donated 1 million to the hospital and got him brand new luxurious home with a g wagon to top it off. I was that grateful because I understood if they did not do it all that I have would be spent and I would not be able to make more since I would be disabled.
